    Plantar Fascia Coronal Length: A New Parameter for Plantar Fascia Assessment

    The effects of gender and various anthropometric variables were previously reported as significant predictors of plantar fascia thickness. Although a strong correlation between either the body weight or body mass index (BMI) and plantar fascia thickness were not demonstrated, a moderate relation was stated. We retrospectively investigated the role of gender, height, weight, and body mass index on plantar fascia thickness at the calcaneal origin (PFCO) and 1 cm distal from the calcaneal origin (PF1cm) and the coronal length of the plantar fascia at the calcaneal origin (CLPF) in healthy subjects. The PFCO, PF1cm, and CLPF were retrospectively measured from magnetic resonance images of 100 healthy subjects. The gender, height, weight, and body mass index of the participants were also noted. Gender was a predictive factor for the length of the CLPF. The subjects with a BMI >25 kg/m(2) had a significantly greater PFCO, PF1cm, and CLPF. Height was mildly and BMI and weight were moderately related to the PFCO. However the CLPF showed a better correlation with height, BMI, and weight than that of plantar fascia thickness. CLPF better reflected the role of weight, BMI, and height than its thickness. It is a new parameter that could be valuable in the evaluation of plantar fascia disorders.     Association of Body Mass Index with Depression and Alexithymia

    Objective: Association between body mass index (BMI) and mental disorders had long been investigated. Recent researches declare a relation between obesity and depressed mood as well as clinical depression. On the other hand mental disorders such as alexithymia are also found to be correlated with an increased risk of obesity. Therefore we aimed to evaluate depression and alexithymia together among normal, overweight and obese adults in order to assess what kind of an association is there between BMI, depression and alexithymia. Methods: We enrolled 100 adults, aged between 18 to 67 from department of endocrinology outpatient clinic whom had normal levels of TSH, fT3 and fT4 thyroid hormones and formed three groups according to their Body Mass Index (BMI). Individuals who had had BMI between 18.5 and 24.99 formed the normal, BMI between 25 and 29.99 formed the overweight and BMI 30 and above formed the obese groups. All three groups were evaluated by Composite International Diagnostic Interview for clinical diagnosis and also given Toronto Alexithymia Scale-26 item (TAS-26) as well as Beck Depression Inventory (BDI). Results: There was a positive relation between BMI and current depressive episode and this finding was stronger in women. There was no such relation with lifelong depression. TAS-26 total and subscale scores revealed no difference between three groups. As BDI total scores got higher TAS-26 scores were higher. Hence there was a strong positive relation between depression and alexithymia. Conclusion: Our findings indicate that higher BMI increases current depressive episode. No association is found between BMI and lifelong depression. Depression and depression severity are strongly associated with alexithymia. Contradictive with literature, alexithymia and its severity are not found to be related with BMI. Following overweight and obese individuals is important to offset the adverse outcomes of depression.

    The Nutritional Risk Screening 2002 Tool For Detecting Malnutrition Risk in Hospitalised Patients: Perspective From A Developing Country

    Background/Aims: To verify the validity of the Nutritional Risk Screening (NRS) 2002 test in a Turkish population. Materials and Methods: We prospectively investigated 2566 patients at a tertiary referral hospital. Nutritional status was screened using NRS 2002, and the length of the stay (LOS) was the main outcome measure. Hospital stays >10 days were accepted as prolonged LOS. NRS scores >= 3 were accepted as indicating risk for malnutrition. Statistical analyses were performed to determine the independent risk factors for malnutrition risk and prolonged LOS. Results: The mean age of patients was 56.6 +/- 16.9 years. According to the NRS 2002, 964 patients (37.6%) were without risk, 1320 (51.4%) warranted surveillance and 282 (11%) were at high risk for malnutrition. Malnutrition rate was the highest in the intensive care unit (22.01%). Prolonged LOS was seen in 24.4% of patients. Intensive care unit stay [odds ratio (OR): 0.585; confidence interval (CI): 1.45-2.22; p<0.001] and an NRS score >= 3 (OR: 0.88; CI: 1.87-3.13; p<0.001) were independent risk factors for prolonged LOS. Conclusion: Improving healthcare outcomes while avoiding preventable healthcare costs is an important goal of healthcare provision in developing countries. NRS 2002 was predictive of LOS, and thus, of patient prognosis. Further community-based studies are warranted to assess the impact of NRS 2002 on reducing healthcare costs.
