PATIENTS AND METHODS: Data about 21 patients were reviewed retrospectively to determine age, clinical symptoms and findings, preoperative screening, operative findings, surgical history, length of hospital stay, and postoperative complications. RESULTS: There were 14 men and 7 women (mean age, 65 +/- 11.3 yr). Preoperative ultrasonography of 17 patients showed a gallbladder stone in 14 patients, adenomyomatosis plus stones in 2 patients, and a polyp in 1 patient. There were 5 patients with acute cholecystitis and 16 patients with chronic cholecystitis. Gallbladder wall thickening was noted in 3 of the 12 patients who had abdominal computed tomography. Frozen section examinations were done in 5 patients. Radical cholecystectomy was done in 1 patient because of suspected carcinoma. CONCLUSIONS: It is difficult to diagnose XGC preoperatively or intraoperatively, and the definitive diagnosis depends exclusively on pathologic examination.Item Neuroendocrine Tumour of the Gallbladder Diagnosed after Cholecystectomy(2022) Tasci, Halil Ibrahim; Coskunoglu, Esra Zeynep; Turk, Emin; Karagulle, Erdal; 0000-0003-2269-4798; AAJ-2989-2021Gallbladder cancer is a rare but aggressive malignancy. Neuroendocrine tumour of the gallbladder make up 2-3% of all the gallbladder tumour. A 67-year female patient underwent laparoscopic cholecystectomy because of symptomatic cholelithiasis and the histopathology revealed a neuroendocrine tumour of the gallbladder, stage pT2a. The patient's imaging study for metastasis workup were normal. A radical cholecystectomy procedure was planned as the tumour stage was pT2a. Postoperative chemotherapy and/ or radiotherapy were recommended. The patient, who had comorbidities, was refused both surgery and other treatment alternatives. The patient's one-year clinical, laboratory, and radiological follow-up did not reveal any findings of recurrence or metastasis. There is no standardised staging system for neuroendocrine tumours of the gallbladder since the number of such cases is quite limited. Guidelines are also insufficient. Multi-centred and large studies are needed in order to develop standardisation in treatment, prognosis, and factors affecting survival.Item A Retrospective Evaluation of the Epithelial Changes/Lesions and Neoplasms of the Gallbladder in Turkey and a Review of the Existing Sampling Methods: A Multicentre Study(2018) Ozgun, Gonca; Esendaglı, Guldal; Akarca, F. Material and Method: With the participation of 22 institutions around Turkey that were included in the Hepato-Pancreato-Biliary Study Group, 89,324 cholecystectomy specimens sampled from 2003 to 2016 were retrospectively evaluated. The numbers of adenocarcinomas, dysplasias, intracholecystic neoplasms/adenomas, intestinal metaplasias and reactive atypia were identified with the review of pathology reports and the regional and countrywide incidence rates were presented in percentages. Results: Epithelial changes/lesions were reported in 6% of cholecystectomy materials. Of these epithelial lesions, 7% were reported as adenocarcinoma, 0.9% as high-grade dysplasia, 4% as low-grade dysplasia, 7.8% as reactive/regenerative atypia, 1.7% as neoplastic polyp, and 15.6% as intestinal metaplasia. The remaining lesions (63%) primarily included non-neoplastic polypoids/hyperplastic lesions and antral/pyloric metaplasia. There were also differences between pathology laboratories. Conclusion: The major causes of the difference in reporting these epithelial changes/lesions and neoplasms include the differences related to the institute's oncological surgery frequency, sampling protocols, geographical dissimilarities, and differences in the diagnoses/interpretations of the pathologists. It seems that the diagnosis may change if new sections are taken from the specimen when any epithelial abnormality is seen during microscopic examination of the cholecystectomy materials.
