Item MRI Findings of Lumbosacral Metastasis from Occult Follicular Thyroid Cancer: Report of A Case(2014) Coban, Gokcen; Yildirim, Erkan; Gemici, Kazim; Erinanc, Hilal; https://orcid.org/0000-0002-9057-722X; https://orcid.org/0000-0003-1401-6356; 23129029; P-7533-2014; ABI-3856-2020; AAL-1268-2021A 63-year-old female was admitted to our hospital with bowel and bladder incontinence. Magnetic resonance imaging (MRI) showed a 13 x 12 x 12 cm mass invading the posterior regions of the L4, L5, S1 and S2 vertebrae with broad paravertebral soft tissue invasion. A Tru-cut biopsy of the mass was performed. The histopathological examination revealed metastatic follicular carcinoma of the thyroid. Thyroid functional tests were within the normal limits. Thyroid sonography revealed a heterogeneous, ill-defined, 14 x 9 mm hypoechoic solid nodule in the right lobe of the thyroid gland. On thyroid scintigraphy, an area of focal hyperactivity was detected in the right lobe at the nodule localization. Total thyroidectomy was performed, and the primary tumor pathology was determined to be follicular thyroid cancer. To our knowledge, only a few cases of lumbosacral cord compression as the initial manifestation of follicular thyroid carcinoma have been reported in the literature. We aimed to discuss the MRI findings of tumors in this age group with lumbosacral localization.Item Predisposing Factors in Posterior Circulation Infarcts: A Vascular Morphological Assessment(2015) Coban, Gokcen; Cifci, Egemen; Yildirim, Erkan; Agildere, Ahmet Muhtesem; 0000-0002-4010-2883; 0000-0002-9057-722X; 0000-0003-4223-7017; 25666230; P-7533-2014; ABI-3856-2020; AAB-5802-2020The aim of the study is to assess the effect of shape, diameter, elongation and deviation criteria of basilar artery (BA), convergence angle and diameter variations of vertebral arteries, and concurrent chronic diseases on posterior circulation infarcts. Between January 2010 and May 2013, 186 patients who underwent brain and diffusion magnetic resonance imaging (MRI) with suspected cerebrovascular accident and were diagnosed with posterior circulation infarct and 120 infarct negative control subjects were included in this case-control retrospective study. Vertebral artery (VA) and BA diameter, right (R) and left (L) VA angles at the level of bifurcation, and BA elongation-deviation, and shape of BA were assessed in a total of 306 subjects. Ischemic lesions in the posterior circulation were classified according to their anatomical location and vascular perfusion areas. No significant difference was noted between the control and patient groups with respect to BA diameter (p = 0.676). The most effective risk factors for posterior circulation infarcts were as follows: BA elongation of 2 or 3, BA transverse location of 2 or 3, increase in left VA angle, and history of hypertension, hypercholesterolemia, and diabetes mellitus. Our results suggest that prominent elongation and deviation, C and J shape of BA, and increased L VA angle may be the predictors of at-risk patients in posterior circulation infarcts. Reporting marked morphological BA and VA variations detected at routine brain MRI will aid in selection of patients. Timely detection and treatment of at-risk patients may be life-saving.Item Recurrent Childhood PRES(2015) Donmez, Fuldem Yildirim; Agildere, Ahmet Muhtesem; 0000-0003-4502-106X; 0000-0003-4223-7017; 25894844; AAE-5528-2021; AAB-5802-2020The purpose of the study was to review the recurrent posterior reversible encephalopathy syndrome (PRES) and emphasize the possibility of repeated attacks on the basis of particular clinical situations. 32 children, diagnosed with PRES were included in our study. The recurrent cases were determined; their radiological features such as involved localizations and clinical information such as presenting symptoms, underlying diseases and clinical prognosis are retrospectively assessed. Of the 32 children (8 months to 18 years old; mean age 11), four of the patients had recurrent episodes of PRES. They had different underlying diseases. One had Chediak-Higashi syndrome, one had ALL, one had chronic renal disease on hemodialysis and one was a renal transplant recipient. Three of the children recovered with no residual neurological deficits, one of them passed away due to multiorgan failure. Three of them had high blood pressures at the time of either one or both PRES, and the renal transplant recipient had also high blood levels of Tacrolimus. Recurrent PRES is encountered most commonly because of repeated increasing blood pressures due to various underlying diseases, immune system disorders or side effects of the treatments.Item Oral 30% Glucose Provides Sufficient Sedation in Newborns During(2017) Eker, H. Evren; Cok, Oya Yalcin; Cetinkaya, Bilin; Aribogan, Anis; 0000-0003-0866-7339; 0000-0003-3004-2626; 0000-0002-7901-0185; 0000-0002-4419-5693; 27999970; AAF-1346-2021; AAI-7998-2021; AAI-8769-2021; S-8336-2019Newborns are often sedated during MRI but sedation itself creates adverse events and management is more challenging in this environment. Oral glucose/sucrose administration has been studied in newborns during painful procedures; however, its effectiveness in keeping newborns sleepy and motionlessness during painless procedures has not been demonstrated. The objective of this study was to describe effectiveness of oral 30% glucose administration by comparing with intravenous midazolam sedation for newborns during MRI. One hundred twelve ASA II-III newborns who required care in the ICU and were scheduled for MRI with sedation were included. Group I received 30% glucose solution orally with 0.5-1 ml increments up to 2 ml/3 kg doses and group II received intravenous 0.1 mg/kg midazolam with 0.05 mg/kg repetition. The procedure was considered satisfactory when MRI images were not disturbed by patient movement after oral glucose or intravenous midazolam administration. The efficiency of the techniques, additional dose and rescue sedation requirements, blood glucose levels following oral 30% glucose suckling and presence of adverse events were recorded. Demographic data was similar between groups. The efficiency of the procedures were similar between groups (78.9%, in group I and 66.1%, in group II). The blood glucose levels were within normal range in group I whereas transient desaturation and apnea occurred in 8 neonates in group II (p = 0.006). Oral 30% glucose administration for newborns during MRI is as effective as standard sedation protocol with midazolam. Thereby, we recommend and support the integration of this safe and reliable technique into routine practice for newborns during MRI.Item Lumbar Multifidus Muscle Changes İn Unilateral Lumbar Disc Herniation Using Magnetic Resonance Imaging(2016) Altinkaya, Naime; Cekinmez, Melih; https://orcid.org/0000-0003-1348-8167; https://orcid.org/0000-0001-9658-9005; 26377578; AAM-5169-2021To assess multifidus muscle asymmetry using the cross-sectional area (CSA) and perpendicular distance of the multifidus muscle to the lamina (MLD) measurements in patients with nerve compression due to lumbosacral disc hernia. In total, 122 patients who underwent microdiscectomy for unilateral radiculopathy caused by disc herniation, diagnosed by magnetic resonance imaging (MRI), were evaluated retrospectively. Posterolateral or foraminal disc herniation at only one disc level, the L3-4, L4-L5, or L5-S1 region, was confirmed using MRI. Subjects were divided by symptom duration: 1-30 days, (group A), 31-90 days (group B), and > 90 days (group C). There were 48 cases in group A, 26 in group B, and 48 in group C. In groups A, B, and C, the median MLD differed significantly between the diseased and normal sides (P < 0.05). The MLD increased on the diseased side with symptom duration by lumbar disc herniation. The diseased side MLD was 5.1, 6.7, and 7.6 mm in groups A, B, and C, respectively (P < 0.05). The cut-off values for the MLD measurements were 5.3 mm (sensitivity = 62.3 %, specificity = 55.5 %; P < 0.05). In groups A, B, and C, the median CSA of the multifidus muscle was not significantly different between the diseased and the normal side (P > 0.05). The MLD measurement correlated significantly with multifidus asymmetry in patients with lumbar disc herniation.Item MRI Findings in Childhood PRES: What is Different than the Adults?(2016) Donmez, F. Y.; Guleryuz, P.; Agildere, M.; 0000-0003-4502-106X; 0000-0003-4223-7017; 25293448; AAE-5528-2021; AAB-5802-2020Posterior reversible encephalopathy syndrome (PRES) is a clinical scenario with convulsion, vision abnormalities, altered mental status, and headaches in the presence of an underlying etiology, and the diagnosis can be made by support of radiological studies. In this study, we evaluated the magnetic resonance imaging (MRI) findings of PRES in children and compared our findings with that of the known features in adults, and reviewed the possible pathophysiological reasons that may cause the difference. A total of 29 children (13 male, 16 female, aged 1-17 years, mean age: 10 years) diagnosed as having PRES were retrospectively reviewed. Clinical records were analyzed for the clinical symptoms and the underlying etiology. MR images were evaluated for the distribution of lesions, contrast enhancement, diffusion restriction, and hemorrhage. Presenting symptoms and underlying etiologies were variable. Frontal lobe (66 %) edema was almost as common as parietal and occipital involvement. Cerebellar involvement was present in almost half of the patients (48 %), which was more frequent than in the adult patients. Contrast enhancement is another finding that was found to be more common in children than in the adults (39 %). Four patients had diffusion restriction (15 %) and four patients had hemorrhage (%15), which are almost the same frequency as in the adults. The increased incidence of cerebellar involvement may show that the posterior circulation in children is more vulnerable than the adults. The contrast enhancement in children, which is seen more commonly than in the adults, may show that the pathophysiology in children may be more commonly related to blood-brain barrier breakdown, which can support the theory of the toxic endothelial injury.Item Management of MRI-Detected Benign Internal Mammary Lymph Nodes(2022) Gunes, Gozde; Crivellaro, Priscila; Muradali, Derek; 0000-0003-0991-1380; 35924122; G-4374-2017Introduction In this retrospective study, we aimed to evaluate benign internal mammary lymph nodes (IMLNs) in terms of frequency, number, size, long axis/short axis (L/S) ratio, intercostal location, presence of fatty hilum, and stability using breast magnetic resonance imaging (MRI) and discuss the findings by reviewing existing literature. Methods This single-center study consisted of 130 women between the ages of 24 and 76 years, who had at least two breast MRI examinations in our institution, with the latest exam performed between January 1, 2019 and November 1, 2019, were eligible for the study. MRIs of the study group were independently reviewed by two radiologists. Results IMLN was detected in 31.1% of the 427 MRIs, with a total number of 256 nodes. The most common indication for breast MRI was high-risk screening (66.2%). The median number of nodes per patient was 1 (range: 1-6). The median follow-up time was 19.5 months (range: 6-141 months). None of these benign nodes showed significant interval growth. Mean L/R ratio of the nodes was 1.9. One hundred and four nodes (n = 104, 40.6%) had a L/S ratio less than 2 and 43.2% (n = 45) of the nodes with a L/S ratio less than 2, had a long axis measuring less than or equal to 3mm. IMLN of patients with breast implants had the largest mean long axis. The fatty hilum was identified in 34.3% (n = 68) of the 256 nodes. The size of the lymph nodes where fatty hilum was visualized was significantly larger than the ones where fatty hilum was not visualized (p < 0.001). Fatty hilum could be visualized in only 2.7% of the nodes with a long axis smaller than 3 mm. Conclusion IMLN is a frequent finding on breast MRI. We have shown that benign IMLNs might be large sized in specific cases like patients with breast implants. When small sized (<= 3mm), they are more likely to be rounded (1/S ratio <2). The fatty hilum that is a feature of benignity might not be visualized in nodes less than or equal to3mm.Item Is it Possible to Differentiate Types of Breast Implants by Imaging in the Era of Implant-associated Lymphoma?(2022) Celik, Levent; Gunes, Gozde; 35410617Objectives: Breast Implant-Associated Anaplastic Large-cell Lymphoma (BIA-ALCL) has been recognised in recent years, and there is extensive ongoing research. Although the exact mechanism and cause are still unclear, we now know that the disease is more associated with textured implants. To the best of our knowledge, no previous studies investigating the radiological differential of various implants have been conducted. In this essay, we aimed to demonstrate dicriminating in vitro and in vivo imaging features of variuos types of breast implant devices using mammography, ultrasound, and Magnetic Resonance Imaging (MRI). Methods: Five different implant devices from various manufacturers with various surface textures, including smooth, micro-textured, regular macro-textured, lightweight macro-textured, and polyurethane-coated were used. In vitro mammography was performed with a digital mammogram (Amulet Innovality, Fuji, Japan), and in vitro and in vivo sonography were performed with Esaote MyLab9 using a 7.5 MHz linear probe. In vitro MRI was performed with a 1.5T magnet (Symphony TIM upgrade and Aera, Siemens Healthcare, Erlangen, Germany) with a 7-channel breast coil (Sense coil, Innova, Germany). MRI studies included fat sat T2 Weighted Sequences (T2WS), non-fat sat T2WS, and silicone only sequences. Results: Each imaging technique had different contributions to dealing with this challenge. Mammography and MRI were limited to identifying the capsule's double bands. We could only differentiate the lightweight macro-textured implant on the mammogram as the borosilicate microspheres were represented by tiny, round lucencies within the gel. Ultrasound imaging with the proper technique was very helpful in identifying the surface. The inner capsule (implant shell) was identified as parallel double echogenic bands on the in vitro sonogram. Bands of the smooth implant were better delineated compared to the textured implants. The double echogenic bands of the polyurethane-coated implant were not even identified individually. The reverberation artifact caused by the smooth implant was the main discriminating in vivo sonographic feature of smooth implants. The hyperintense polyurethane-coated capsule was identified on fat-saturated T2WS and non-fat-saturated T2WS via in vitro MRI. The tiny hypointense microspheres of the lightweight implant were also identified on the silicone-only sequence of the in vitro MRI. Conclusion: In this study, we have shown that breast implant material and type may differ with the help of in vitro and in vivo imaging characteristics on different radiological modalities. These different imaging features could be used for recognising and labelling the implant type, especially macro-textured implants that are reported to be more associated with breast implant-associated anaplastic large-cell lymphoma (BIA-ALCL) compared to other types. We believe evaluating these imaging characteristics during daily practice will help radiologists become aware of the implant type and possible complications or diseases associated with that type.Item MRI evaluation of progressive supranuclear palsy: differentiation from Parkinson's disease and multiple system atrophy(2019) Eraslan, Cenk; Acaer, Ahmet; Guneyli, Serkan; Akyuz, Esra; Aydin, Elcin; Colakoglu, Zafer; Kitis, Omer; Calli, Mehmet Cem; 30373485Objectives: To evaluate the magnetic resonance imaging (MRI)-derived parameters in differentiation of patients with progressive supranuclear palsy (PSP) from patients with Parkinson's disease (PD), multiple system atrophy (MSA), and control subjects was aimed. Methods: Thirty-three patients [mean age, 65.21 +/- 4.75 years; PSP (n = 9), MSA (n = 8), PD (n = 6), and control subjects (n = 10)] who have undergone cranial MRI were included in this retrospective study. MRI-derived parameters including areas of midbrain and pons, midbrain area-to-pons area (M/P) ratio, widths of middle cerebellar peduncle (MCP) and superior cerebellar peduncle (SCP), MCP/SCP ratio, magnetic resonance parkinsonism index (MRPI), cerebral interpeduncular angle, and length of midbrain tegmentum were compared in patients with PSP, PD, MSA, and control subjects through the analysis of variance and Kruskal-Wallis tests with Bonferroni correction and Mann-Whitney U test. Results: The length of midbrain tegmentum, midbrain area, SCP, and M/P ratio were found to be lower, while cerebral interpeduncular angle and MRPI were higher in patients with PSP. Pons area, MCP width, and MCP/SCP ratio were found to be lower in patients with MSA. For PSP, cerebral interpeduncular angle has a sensitivity of 100% and specifity of 90%, and MRPI had a sensitivity of 88.9% and specifity of 100% for PSP. Discussion: Several MRI-derived parameters can be used in differentiation of patients with PSP from patients with PD, MSA and control subjects. The cerebral interpeduncular angle and MRPI, which demonstrated higher values in patients with PSP, were more significant for PSP than the other parameters.Item Bilateral renal leiomyoma with 5 year follow-up: Case report(2015) Goren, Mehmet Resit; Erbay, Gurcan; Ozer, Cevahir; Goren, Vinil; Bal, Nebil; 26664510Renal leiomyomas are exceptionally rare benign tumours of the kidney. Although the renal leiomyomas usually do not metastasize, the differential diagnosis between renal leiomyomas and malign lesions (leiomyosarcoma or renal cell carcinoma) cannot be done by radiological examinations, but is possible by histological examination. Surgery is the preferred treatment. After surgery, the prognosis is excellent without recurrence. Although uterine leiomyomas can be multicentric, renal leiomyomas have been single lesions. We report an incidentally detected case of bilateral renal leiomyoma in a 50-year-old woman with a 5-year follow-up. We also review the literature and discuss clinical, radiological and histological features of renal leiomyomas.
