    Ethical Discourse of Medical Students on the Phenomenon of Death: A Qualitative Study

    The aim of this study is to discover medical students' ideas on the phenomenon of death; produce information on how these students interpret the value-related problems regarding death that they come across in different units of hospitals; and assess this data in ethical terms. This study included a qualitative research in which 12 focus group interviews were conducted with 92 fifth- and sixth-year medical students. Data obtained from interviews were assessed using a thematic content secondary analysis. The main themes were specified according to the medical students' statements and were reviewed under the contexts of the "dying process"; "effects of death"; "attitude and behavior of health professionals"; "seeing a dead body/looking at a dead body"; "accepting death"; and "forms of expressions of death." Medical students' encounters with death in different units of hospitals leads them to question their values and familiarize themselves with the borders of their areas of profession.

    In Vitro Evaluation of E. coli And S. epidermidis Adhesion on Dressing Materials Used in Hypospadias Repair

    Background An in vitro study was performed to evaluate the adherence of Escherichia coli and Staphylococcus epidermidis to different dressing materials. Methods Hydrophyl gauze swab (GS), sterile flax swab (FS), and their nitrofurazone (Furacin (R)) absorbed forms (GSF and FSF), sterile paraffin-covered gauze with chlorhexidine acetate (0.5 %) (Bactigrass (R), BG), and semipermeable transparent film gauze (Tegaderm (R), TG) were used in the study. A 1 cm(2) of each material was placed in culture plates, and E. coli ATCC 25922 and S. epidermidis ATCC 12228 were suspended in each plate. After 24, 48 h, and 1 week of incubation period, colony counts were calculated in colony-forming units (CFU) per square centimeter unit. Bacterial adhesion was compared between groups, and each dressing material was also compared for different incubation periods (24, 48 h, and 1 week). Results The colony counts were significantly decreased in GSF and FSF groups (Kruskal-Wallis test, p < 0.05). There was no difference for E. coli adhesion in GS, FS, BG, and TG groups (p > 0.05). The significant difference detected for S. epidermidis adhesion in the first 24 to 48 h could not be detected at the end of a 1-week period. There was no statistical difference in groups when compared for 24-, 48-h, and 1-week periods of incubations (Wilcoxon signed-rank test, p > 0.05). In BG group, E. coli adhesion was increased, and S. epidermidis adhesion was decreased without a significant difference at the end of a 1-week period (p > 0.05). Conclusions Nitrofurazone absorbed sterile gauze materials have less bacterial adhesion. In vitro adhesion of E. coli and S. epidermidis did not differ between the 24-h and 1-week period. E. coli adhesion to BG can be increased if the duration of dressing prolonged up to a 1-week period. Level of Evidence: Not ratable.

    Prostatic Melanosis: A Case Report

    Prostatic melanosis is characterized by melanocytic proliferation in prostatic stroma and the epithelium. The pathogenesis of the lesion is uncertain. In literature it has been reported as a case associated with prostatic adenocarcinoma before. Primary and metastatic malignant melanoma should be considered in differantial diagnosis of the lesion.

    Clinical and Cytologic Features of Antibiotic-Resistant Acute Paronychia

    Background: Acute paronychia usually is treated as a bacterial infection, but antibiotic-resistant acute paronychia may be caused by other infectious and noninfectious problems. Objective: We sought to describe the clinical, etiologic, cytologic, and therapeutic features of antibiotic-resistant acute paronychia. Methods: A retrospective review of medical records and cytology was performed in 58 patients (age, 1 month-91 years; 36 children and adolescents [62%] and 22 adults [38%]) who had antibiotic-resistant acute paronychias. Results: Causes of paronychia included bacteria (25 patients [43%]), viruses (21 patients [36%]), fungi (5 patients [9%]), drugs (3 patients [5%]), pemphigus vulgaris (3 patients [5%]), and trauma (1 patient [2%]). Diagnostic cytologic findings were noted in 54 patients (93%); no diagnostic cytologic findings were present with drug-induced (3 patients) or traumatic (1 patient) paronychia. The most common predisposing factors were the habits of finger-or thumb-sucking (14 patients [24%]) and nail-biting (11 patients [19%]). Complications included id reaction with erythema multiforme in 3 patients (5%). Limitations: Limitations include retrospective study design from 1 treatment center. Conclusion: Antibiotic-resistant acute paronychia may be infectious or noninfectious. Cytologic examination with Tzanck smear may be useful diagnostically and may prevent unnecessary use of antibiotics and surgical drainage.

    Renal Pseudoaneurysm after Micropercutaneous Nephrolithotomy

    Postoperative bleeding due to pseudoaneurysm formation is the major cause of percutaneous kidney interventions. Micro percutaneous nephrolithotomy is a one step procedure that used mostly lower pole stones. There are small series that reports technical feasibility and safety of microperc using the All-seeing needle. Here we state the first report of pseudoaneurysm and its treatment after microperc in the literature.

    A Rare Cause of Small Intestinal Obstruction: Obturator Hernia

    Obturator hernia is an uncommon disease and its diagnosis is challenging. It is more prevalent in thin, multiparous, elderly women. In this case report a 93-year-old woman patient with a right-sided strangulated obturator hernia was discussed with review of the relevant literature.

    First Trimester Maternal Lipid Levels and Serum Markers of Small-And Large-For-Gestational Age Infants

    Objective: To investigate if first trimester lipids, sonographic parameters and serum markers are related to small-and large-for-gestational age (SGA, LGA) infants. Methods: This study was conducted at Baskent University Adana Research Center between December 2009 and July 2011 and enrolled 433 women. Blood samples were drawn to measure fasting blood glucose, serum triglycerides, cholesterol, very low-density lipoprotein, low-density lipoprotein, high-density lipoprotein, f beta-hCG and pregnancy associated protein-A (PAPP-A) at the first trimester. Crown rump length and nuchal translucency were measured as suggested by the fetal medicine foundation. Results: LGA group was significantly taller (p = 0.016) and SGA group had significantly greater BMI (0.025). SGA fetuses were born at a significantly earlier gestational age (p = 0.001). Univariate analysis revealed that LGA group had significantly lower cholesterol (p = 0.038) and LDL levels (p = 0.041). PAPP-A was significantly lower in SGA Group compared with LGA Group (0.027). After controlling for age, parity, height, pre-pregnant BMI, weight gain during pregnancy and fasting blood sugar, none of the lipids, serum markers or sonographic parameters was related to LGA. PAPP-A was the only parameter significantly associated with SGA after multivariate analysis (p = 0.008). Conclusion: PAPP-A was significantly associated with SGA after controlling for confounders.

    The Relationship Between Thyroid Volume and Malignant Thyroid Disease

    The present retrospective study aimed to investigate the relationship between thyroid volume and prevalence of thyroid cancer. We investigated the data of 3,850 patients who underwent fine-needle aspiration biopsy (FNAB). Biopsy results were evaluated as diagnostic or nondiagnostic, and diagnostic results were classified as benign, malignant, and indeterminate. We included 2,672 patients who underwent FNAB firstly in our hospital and evaluated as diagnostic biopsy except subgroup of indeterminate. We obtained cytologic data, levels of serum thyroid-stimulating hormone (TSH), and thyroid volumes of those patients retrospectively. Among 2,672 patients with thyroid nodule, 2,562 (95.9 %) patients had benign cytology and 110 (%4,1) patients had malignant cytology. There was no correlation between the malignancy and gender (p = 0.935), and patients with malignant cytology were younger (52 vs 59, p < 0.001). Also, TSH levels were higher in patients with malignant than benign cytology (p = 0.017). Median volume of right part, left part, and total thyroid for patients who had malignant cytology was significantly lower than patients who had benign cytology (8.3, 7.1, 15.9 vs 10.8 ml, 9.0 mml, 20.6 ml, respectively, p <= 0.001 for all parameters). The results demonstrated that thyroid cancer prevalence was higher in patients with low thyroid volume. According to our results, thyroid volume should be considered as a risk factor for malignancy in the evaluation of thyroid nodules.

    Fertility Outcomes of Patients with Early Stage Endometrial Carcinoma

    AimThree to five percent of endometrial carcinoma patients are younger than 40 years and may desire fertility. Conservative treatment can be employed in these cases. We aimed to review treatment outcomes of patients who were diagnosed with endometrial carcinoma and who wanted to preserve their fertility. Material and MethodsWe reviewed nine patients who were diagnosed with early stage endometrial carcinoma and wanted to spare their fertility. The patients were followed up at Baskent University Adana Research Center from January 2004 to December 2011. ResultsIn all patients the carcinoma presented as polyps, which were resected by hysteroscopy. After being informed about both surgical and medical therapies, four patients preferred surgery and five preferred medical treatment. The mean number of in vitro fertilization trials after conservative treatment was 3.25. One woman, who was on medroxyprogesterone acetate, delivered a healthy term baby from a fresh cycle. Another woman, who was on dydrogesterone, got pregnant from a thawing cycle, which later ended up in a missed abortus. Of all the patients who chose medical treatment, three had surgery at the end. One woman developed an ovarian tumor during the follow-up; one woman had a recurrence of endometrial carcinoma on dilatation and curettage for missed abortus and one woman tried in vitro fertilization several times and could not get pregnant, thus decided to have surgery. Two women had stage IA endometrial carcinoma and one had stage IIB ovarian carcinoma. ConclusionConservative treatment of endometrial carcinoma is safe in most cases. However, patients should be well-informed about the risks of conservative treatment because delaying definitive treatment sometimes worsens the prognosis.

    Effects of Amifostine on Endometriosis, Comparison with N-Acetyl Cysteine, and Leuprolide As A New Treatment Alternative: A Randomized Controlled Trial

    To assess the effects of amifostine, N-acetyl cysteine (NAC), and leuprolide as a scavenger in a rat endometriosis model. This is a prospective randomized animal study. Setting The Animal Laboratory of Medical University. Animals 40 rats were used for transplantation of an autologous fragment of endometrial tissue onto the inner surface of the abdominal wall. After allowing 3 weeks for growth, laparotomies were performed to check the implants. Then animals were randomized into four groups: Group I amifostine (200 mg/day loading dose after 20 mg/kg/day, p.o.); Group II NAC (200 mg/day, p.o.); Group III leuprolide acetate 1 mg/kg single dose, sc; and Group IV (controls) no medication. Three weeks later, implants were evaluated morphologically. Serum and peritoneal TNF-alpha levels were evaluated. The transmission electron microscopic examination of the peritoneal samples and ovaries was also performed. Leuprolide acetate, amifostine and NAC caused significant decreases in the mean implant areas and significant decreases in serum and peritoneal TNF-alpha levels. On comparing all groups, these reductions were higher in Group II. According to the transmission electron microscopic findings, leuprolide seems to be protecting normal structure of peritoneum best when compared to the other groups. Amifostine, NAC and leuprolide caused regression of endometriosis in this experimental rat model by a yet unsettled mechanism.
