Item Intravesical BCG Immunotherapy: Sepsis and Multiorgan Failure Developed After Traumatic Catheterization(2014) Cicek, Tufan; Togan, Turhan; Erinanc, Hilal; Ugur, Murat; Gonulalan, Umut; Cifci, Egemen; 0000-0003-1401-6356; AAL-1268-2021Intravesical Bacillus Calmette-Guerin (BCG) instillation is a prophylactic therapy using for treating bladder cancer to prevent tumour progression and recurrence. Both local and systemic complications can arise after the installation. Although local complications are common, this therapy is generally well tolerated. Systemic complications are rarely than local complications but can be fatal. We report a case who died from severe complications such as sepsis, pneumonia, renal failure and granulomatous hepatitis after receiving the first maintanence installation of intravesical BCG immunotherapy for bladder transitional cell carcinoma.Item Transpaeritoneal Laparoscopic Simple Nephrectomy: A Single Center Experience(2014) Cicek, Tufan; Sen, Erhan; Gonen, Murat; Gonulalan, Umut; Kosan, Murat; Ozturk, Bulent; 0000-0003-1944-1722; AAJ-8576-2021; AAA-7140-2021Purpose: In this study, we planned to evaluate the efficacy and perioperative outcomes of transperitoneal laparoscopic simple nephrectomy at Baskent University Konya Application and Research Center. Material and Methods: The demographic and perioperative data of patients who underwent transperitoneal laparoscopic simple nephrectomy at Baskent University Konya Application and Research Center Urology clinic between February 2005 and November 2013 were analyzed retrospectively. Operative time, perioperative laboratory results, the durations of hozpitalization and follow-up, perioperative complications and demographic characteristics of each patient were recorded. Results: Overall, 58 patients were included. 27 patients were male (46%) and 31 patients were female (54%). The mean age was 45.3 +/- 17.12 (6-79). The mean operative time was 104.4 +/- 36.5 (40-185) minutes. The mean hospitalization duration was 2.7 +/- 0.7 (2-5) days. The mean preoperative and postoperative hemoglobin levels were 13.7 +/- 1.74 and 12.4 +/- 1.73 mg/dl, respectively. Postoperative hemoglobin levels were statistically significatly lower than preoperative hemoglobin levels (p<0.001). 4 patients (%6.9) needed blood transfusion. 5 patients had grade I, 4 patients had grade II and 2 patients had grade III complications according to modified Clavien classification system. Conclusion: Transperitoneal laparoscopic simple nephrectomy seems to be a safe and efficacious treatment option as having low complication rates.Item A Rare Epididymal Mass: Leiomyoma(2014) Cicek, Tufan; Coskunoglu, Esra Zeynep; Demir, Canan Cicek; Akbaba, Kaan; Uzunkopru, Resit; JBF-7113-2023; AAK-4857-2021Leiomyoma is a benign smooth muscle tumor. It rarely rises from epididymal structures and usually is encountred in adults. The patient, who had undergone total epididymectomy for right scrotal mass at another center, was diagnosed with Epididymal Leiomyoma. He was admitted to our clinic for further evaluation and follow up. The previous eididymectomy specimens obtained and refered to our pathology department for confirmation of diagnosis. Microscopy revealed a benign neoplastic lesion, consisting of spindle cells. Which form interlacing bundels. Mitotic figures and tumor necrosis was not. Observed. The previous diagnosis is confirmed as the tumor showed immunohistochemical smooth muscule actin and Desmin posivity Whereas S-100 was not expressed with in the tumoral cells. This case report also overviews epididymal leiomyoma with recent literature findings.Item Prostatic Melanosis: A Case Report(2014) Cicek, Tufan; Erinanc, Hilal; Gonulalan, Umut; Sen, Erhan; Ozturk, Bulent; 0000-0003-1944-1722; 0000-0003-1401-6356; AAJ-8576-2021; AAL-1268-2021Prostatic melanosis is characterized by melanocytic proliferation in prostatic stroma and the epithelium. The pathogenesis of the lesion is uncertain. In literature it has been reported as a case associated with prostatic adenocarcinoma before. Primary and metastatic malignant melanoma should be considered in differantial diagnosis of the lesion.Item Renal Pseudoaneurysm after Micropercutaneous Nephrolithotomy(2014) Cicek, Tufan; Istanbulluoglu, Okan; Yildirim, Erkan; Buldu, Ibrahim; Kaynar, Mehmet; Ulas, Huseyin; 0000-0002-9057-722X; 0000-0002-6957-9060; ABI-3856-2020Postoperative bleeding due to pseudoaneurysm formation is the major cause of percutaneous kidney interventions. Micro percutaneous nephrolithotomy is a one step procedure that used mostly lower pole stones. There are small series that reports technical feasibility and safety of microperc using the All-seeing needle. Here we state the first report of pseudoaneurysm and its treatment after microperc in the literature.Item Laparoscopic Transperitoneal Adrenalectomy: Experience with the First 10 Patients(2014) Cicek, Tufan; Karagulle, Erdal; Turk, Emin; Demir, Canan Cicek; Kosan, Murat; 0000-0003-4766-3373; AAJ-5609-2021; JBF-7113-2023; AAK-4857-2021Purpose: In this study we aimed to assess the outcomes and complications of laparoscopic transperitoneal adrenalectomy operation performed in our clinic Materials and Methods: Medical records of patients operated with laparoscopic transperitoneal adrenalectomy between February 2007 and November 2013 at Baskent University Konya Application and Research Center Urology Clinic are examined retrospectively. The demographic characteristics, preoperative and postoperative laboratory results, operation times, complications, and duration of hospital stay and follow- up were recorded. Results: A total of 10 patients underwent laparoscopic transperitoneal adrenalectomy. Seven of these were male and 3 were female. The mean age was 48 +/- 17.4 (27-71) years. The operation time was 104.4 +/- 36.5 (40-185) on average and the mean amount of blood loss was 43.5 +/- 121.2 ml. The average mass size was 41.6 +/- 23.8 (15-90) mm. The mean duration of hospital stay was 3.8 +/- 1.3 (2-6) days. The patients were followed for an average of 45.8 +/- 28.7 (4-85) months. The operation was turned to open technique in two patients. Mean preoperative and postoperative hemoglobin levels of the patients were 13.38 +/- 1.91 and 11.6 +/- 1.7 mg/dl, respectively. Postoperative hemoglobin level was on average lower than the preoperative level (p< 0.001). One patient required blood transfusion. Conclusion: We suggest that the complication rate of laparoscopic adrenalectomy would be lowered by selecting appropriate patients, more detailed assessment of adrenal functions, and increased surgical experience. Laparoscopic transperitoneal adrenalectomy is regarded as a safe and efficient treatment method.Item The Role of 5α-Reductase Inhibitors on Prevention and Hormonal Treatment of Prostate Cancer(2014) Ozturk, Bulent; Cicek, Tufan; https://orcid.org/0000-0003-1944-1722; AAJ-8576-2021Aim: Although testosterone generates the main part of serum androgens, the main prostatic androgen is dihydrotestosterone. Dihydroestosterone is produced from testosterone by 5 alpha-reductase. Dihydrotestosterone has some roles on different diseases as benign prostate hyperplasia and prostate cancer in human. We evaluated the role of 5a-reductase inhibitors on the treatment and prevention of prostate cancer. New Findings: Recently, pure antiandrogens targeting androgen receptors or medical and surgical castration are used in hormonal treatment of prostate cancer. However, these treatments reduced the tumoral mass and the activity of androgen receptors, prostate cancer reactivated in 18-30 months. New drugs that affect the different levels of androgen-androgen receptor pathway are needed to increase the affectivity of treatment. One of these drugs is 5 alpha-reductase inhibitors. There two wide clinical trials on 5 alpha-reductase inhibitors as dutasteride and finasteride. The lower incidences of prostate cancer in patients with 5 alpha-reductase inhibitors were reported in these trials. On the other hand, it is reported that more aggressive tumors were seen with 5 alpha-reductase inhibitors in comparison with placebo. Conclusion: Clinical trials with finasteride and dutasteride are encouraging. Patients often present with penile deformity, recurrent urinary infections, urinary incontinence, serosal discharge from the accessory urethra, and difficulty in urinating. Urethral duplication is most commonly grouped according to the Efmann classification. It has 3 main types as Type I, II, and III. There is no consensus on its therapy. There are non-surgical solutions, including follow-up without therapy, as well as many surgical options, including urethral reconstruction. Anatomical urethra and the external sphincter should absolutely be delineated when a surgery is contemplated. We herein report the case of a two-year-old male patient referred to our clinic with recurrent urinary infection, bilateral hydronephrosis and difficulty in urinating who was diagnosed with urethral duplication. We discuss our findings with review of the relevant literature.Item Skin-to-Stone Distance Has No Impact on Outcomes of Percutaneous Nephrolithotomy(2014) Gonulalan, Umut; Akand, Murat; Coban, Gokcen; Cicek, Tufan; Kosan, Murat; Goktas, Serdar; Ozkardes, Hakan; https://orcid.org/0000-0002-4010-2883; https://orcid.org/0000-0002-0784-9926; https://orcid.org/0000-0002-7277-449X; 24481206; P-7533-2014; AAH-1052-2020Objective: Skin-to-stone distance (SSD) is a stronger factor than body mass index in predicting the success of shock wave lithotripsy. We aimed to evaluate the impact of SSD on outcomes of percutaneous nephrolithotomy (PCNL). Materials and Methods: The medical records of 1,280 patients who had undergone PCNL between April 2007 and February 2012 were evaluated retrospectively. 192 patients who had had preoperative non-contrasted computed tomography and single renal access were included the study. According to this median SSD value, patients were divided into two groups: group 1 (SSD <= 94 mm) (n = 92) and group 2 (SSD >94 mm) (n = 90). The groups were compared according to operative and postoperative parameters. Results: We found no significant differences between the two groups with regard to stone-free rate, operation time, fluoroscopy time, hospitalization time, visual analog score of pain, stone burden, transfusion rates and complication rates. On the other hand, the mean body mass index of group 1 was significantly lower than that of group 2 (p < 0.05). Conclusions: In this retrospective review of patients undergoing PCNL, we found that SSD has no impact on operative and postoperative outcomes. These results were in accordance with the safety of PCNL in obese patients. (c) 2014 S. Karger AG, Basel
