Item An Evaluation of the Relationship Between the Mesiobuccal Canal Configuration, the Interorifice Distance, and the Root Lengths of the Permanent Maxillary First Molars with Cone Beam Computed Tomography(DIAGNOSTICS, 2024-12) Ozemre, Mehmet Ozgur; Karslioglu, Hazal; Kamburoglu, KivancBackground/Objectives: This study aimed to investigate the relationship between the mesiobuccal root canal configuration (MB RCC), the interorifice distance (IOD) and the corresponding root and other root lengths of the permanent maxillary first molars; Methods: Cone beam computed tomography (CBCT) images were acquired between 2020 and 2023 for different purposes unrelated to this study. Overall, 1550 CBCT images were retrospectively evaluated. A dentomaxillofacial radiologist with 15 years of experience evaluated the CBCT images and performed the measurements; Results: According to the MB RCC, there was no statistically significant difference between the Vertucci type II and Vertucci type IV groups in terms of the mean age and sex distribution (p = 0.694 and p = 0.273). There was no statistically significant difference in the IOD between the MB RCC groups (p = 0.755). Moreover, according to the MB RCC, there was no statistically significant difference between the Vertucci type II and Vertucci type IV groups in terms of the mesiobuccal, distobuccal, palatinal, and mean root lengths (p > 0.05); Conclusions: There was no association between the IOD and the type of RCC in the maxillary first molars. New studies conducted by collecting data from different centers to explore the different morphological features of maxillary first molars and detect their anatomical differences will provide more reliable and accurate results.Item Characterizing The Unseen: Clinical And Radiographic Perspectives On Unilateral Condylar Hyperplasia(JOURNAL OF STOMATOLOGY ORAL AND MAXILLOFACIAL SURGERY, 2024-12) Ergezen, Ezgi; Salyut, Ardakgul; Akdeniz, Sidika Sinem; Cubuk, SecilObjectives: Unilateral condylar hyperplasia (UCH), marked by progressive condylar growth, occurs between ages 11-30 with unclear etiology. This retrospective study aims to clarify the clinical, radiographic, and demographic features of UCH to improve its diagnosis and treatment. Material and methods: Data for all patients included in this study were retrieved from the archive. Inclusion criteria were the diagnosis of UCH supported by a positive bone scan above 10% of condylar uptake in SPECT/ CT, the patient's history of progressive facial asymmetry, symptoms confirmed by the orthodontist during follow-up, and clinical and radiological evaluation. Demographic characteristics, along with volumetric and linear measurements, were statistically evaluated. Results: Forty-one patients were included in this study with 70.73% female predominance. Patients were classified as transverse and vertical type condylar hyperplasia (CH) according to their prominent asymmetry characteristics. Eighteen patients were classified as vertical type CH and 23 patients were classified as transverse type CH. Twenty had left-side condylar overgrowth, and 21 had right-side overgrowth. Condylar volume difference was significantly higher in vertical type CH patients compared to transverse type CH (p = 0.004). No correlation was found between growth rate (bone scan uptake ratio) and condylar volume difference in patients with vertical type and transverse type CH patients (p = 0.205). Conclusion: Particularly in cases that exhibit a pronounced vertical growth tendency, volumetric and linear measurements can guide the timing and treatment planning process. Accuracy was evaluated using the difference between planned and executed IPR. Anxiety and discomfort levels experienced by the patients were evaluated using a questionnaire of 17 questions.ResultsThe accuracy of IPR was high in groups 2 and 3; however, it was low in group 1 with the executed IPR significantly less than the planned amount. On quadrant-level, executed IPR was significantly less in the upper left quadrant in group 1, and significantly more in the upper right quadrant in group 2. The difference between planned IPR and executed IPR was significant for teeth 11, 21, 32, 33, and 43 in group 1, indicating deficiency. The average difference between planned IPR and executed IPR was 0.08 mm for group 1, 0.09 mm for group 2, and 0.1 mm for group 3. Anxiety and discomfort levels did not differ between the methods, but a negative correlation was observed between age and discomfort and anxiety levels.ConclusionsThe overall accuracy of the 2 motor-driven IPR methods was found to be better than the hand-operated system. Maxillary central incisors and mandibular canines were more prone to IPR deficiency when hand-operated abrasive strips were utilized. Patients were similarly comfortable with all 3 methods, and discomfort and anxiety levels decreased with age.Clinical relevanceMotor-driven methods have proven to be more effective when compared to the hand-operated ones by means of precision, speed, and patient comfort. If the clinician favors a hand-operated method, it may be advised to perform slightly more IPR especially on mandibular canines and maxillary central incisors.Item Effect Of Ultrasonic And Sonic Instrumentation On Shear Bond Strength Of Resin Composite To Enamel: An In Vitro Study(AMERICAN JOURNAL OF DENTISTRY, 2024-04-12) Adiguzel, Ruhsan Muduroglu; Mustafa, Abdelrahman; Oduncuoglu, Bahar Fusun; Colak, Mew Yavuz; Gulsahi, Aye; Arhun, NeslihanPurpose: To assess and compare the effects of sonic and ultrasonic instrumentation on shear bond strength (SBS) and investigate the influence of adhesive system application modes (etch&rinse/self-etch) on SBS. Methods: In this experimental study, 45 extracted sound human molars were selected. Mesial and distal surfaces of the teeth were abraded until 90 smooth enamel surfaces were obtained. Specimens were divided into two groups, with half (N= 45) conditioned in etch&rinse mode and the remaining (N= 45) in self-etch mode using Tetric N Bond Universal. Composite resin discs were produced on these surfaces by filling Teflon molds. All specimens were aged via thermocycling. Each group was further divided into three more groups (n= 15) according to the type of periodontal instrumentation (ultrasonic, sonic or none). SBS values were recorded and analyzed using two-way ANOVA. Fracture sites were examined under a stereomicroscope. Results: The type of periodontal instrumentation had no significant effect on SBS values, whether sonic, ultrasonic or their absence (P= 0.945). The type of adhesive system mode had a significant influence on the shear bond strength values. Etch&rinse groups had significantly higher shear bond strength values than self-etch groups (P< 0.001).Item Evaluation Of Gap Formation For Different Adhesive Agents In Primary Teeth With Optical Coherence Tomography(EUROPEAN ORAL RESEARCH, 2024-05-23) Uyar, Didem Sakaryali; Asena, Leyla; Tirali, Resmiye EbruPurpose This study aimed to evaluate gap formation between the tooth surface and restorative material in terms of microleakage by using optical coherence tomography (OCT) for self-etch and selective-etch applications of two different universal and one self-etch adhesives. Materials and Methods Sixty non-caries, primary molar teeth were divided into six groups; self-etch and selective-etch application ways of two different universal and one self-etch adhesive systems (n:10). After Class-V cavities were prepared, every tooth was distributed randomly in groups to apply adhesion procedure and then, all cavities were restored with polyacid-modified composite resin. Microleakage was evaluated by measuring the gap between the tooth surface and restoration by a blind researcher with Image J Software from OCT images. During statistical analysis, the significance level was accepted as p<0.05. Results According to the statistical analysis of the measurements obtained by Image J Software, selective-etch groups showed less gap formation than self-etch groups for each tested adhesive (p<0.05), and self-etch adhesive without etching showed significantly highest gap formation among all groups (p<0.05). Conclusion Universal adhesives with a selective-etching step might be preferred over self-etch adhesives for long-lasting polyacid-modified composite resin restorations in primary teeth. However, obtained results should be considered with another prospective clinical study for long-term prognosis.Item Evaluation of the Consistency of Two Interproximal Reduction Methods in Clear Aligner Therapy: A Preliminary Study(Başkent Üniversitesi Diş Hekimliği Fakültesi, 2024-04-06) Ergun, Pelinsu Gulec; Ozcirpici, Ayca Arman; Kocabalkan, Azize Atakan; Tuncer, Nilufer IremObjective: To compare the consistency of two interproximal reduction (IPR) methods in terms of the amount of planned and performed IPR during clear aligner therapy (CAT). Methods: Thirty-four patients who received IPR using hand -operated abrasive strips (Group 1, 20 patients, 162 teeth) and motordriven 3/4 oscillating segmental disks (Group 2, 14 patients, 134 teeth) during CAT were included in this preliminary study. The consistency between the planned and performed IPR amounts was evaluated within and between groups for teeth and quadrants. Results: In Group 1, the amount of IPR performed on teeth numbers 22 and 43 and in the upper left quadrant was found to be statistically less than that of planned. On the other hand, the amount of performed IPR was statistically higher on tooth number 44 and in the upper right quadrant, whereas it was statistically less on tooth number 33 when compared with the planned amount in Group 2. The inconsistency between the planned and performed IPR amounts were statistically significant only in Group 1 and for teeth numbers 11, 21, 32, 33, and 43. No significant difference was found when the same parameter was compared between the groups. Conclusion: The consistency of IPR was found to be better with the motor -driven oscillating disk system than with the hand -operated IPR strip system.Item Factors That May Increase The Risk of External Apical Root Resorption During Orthodontic Treatment Retrospective Clinical Investigation(2024) Kaya, Burcak; Gulsahi, Ayse; Turkyilmaz, GizemPurposeTo determine the correlation between external apical root resorption and malocclusion-related variables in patients treated with fixed orthodontic appliances.MethodsIn all, 103 patients aged 12-15 years and treated with edge-wise appliances either without extractions or with four premolar extractions were included in this retrospective cohort study. External apical root resorption was assessed in the pre- and posttreatment panoramic radiographs of these patients for incisors, canines, premolars, and first molars. A total of 2332 teeth were evaluated in 206 panoramic radiographs obtained from 103 patients. The gender of the patients, duration of orthodontic treatments, presence of premolar extractions, Angle classification, overbite, overjet and amount of crowding were assessed. Wilcoxon signed-rank test, Mann-Whitney U test, and Kruskal-Wallis test were used for statistical analysis of the data.ResultsStatistically significant (p < 0.001) root resorption occurred in all examined teeth during orthodontic treatment. The degree of root resorption observed in the premolars was significantly greater in premolar extraction cases than in nonextraction cases. All Hb values were derived from complete blood count tests obtained on the first day of C-CRT. The receiver operating characteristic (ROC) curve analysis was used to scrutinize a possible connection between pre-treatment Hb values and RIT status.Results Two hundred and twenty three patients were included in the study and RIT was diagnosed in 46 (20.6%) patients. The Hb cutoff in ROC curve analysis that separated the patients into two groups was 12.05 g/dL [Area under the curve (AUC): 82.7%; sensitivity: 72.9%; and specificity: 71.3%]. RIT was significantly more prevalent in the Hb = 12 g/dL group than in its counterpart (41.9% vs. 7.3%; p < 0.001). In multivariate analysis, Hb = 12, anemia, pre-C-CRT MMO < 41.4 mm, and masticatory apparatus doseV58 Gy < 32% groups were found to be independently associated with significantly increased rates of RIT.Conclusion Low pre-C-CRT Hb and anemia status are novel biological markers that independently predict higher RIT rates in LA-NPC undergoing C-CRT.
