Evre 1A2-1B1 serviks kanserinde parametriyal tutulumu etkileyen faktörler ve düşük riskli hasta grubunun seçilmesi

Abstract

Amaç: Evre 1A2-1B1 serviks kanserli hastalarda parametriyal tutulumu etkileyen faktörlerin belirlenmesi ve düşük risk faktörlerine(tümör boyutu ≤2 cm , DSİ≤ % 50, LVSİ negatif) sahip erken evre (1A2-1B1) serviks kanserli hastalarda parametriyal tutulumun ve konservatif cerrahinin değerlendirilmesi. Gereç ve yöntemler : Araştırmada mayıs 2007 ve haziran 2013 yılları arasında Başkent Üniversitesi Ankara Hastanesi Jinekolojik Onkoloji Anabilim Dalında opere edilen 168 evre 1A2 ve 1B1 serviks kanserli hastalar değerlendirilmiştir.13 hasta (%7.7) evre 1A2 ve 155 hasta evre 1B1 (%92.3) idi.Hastaların ortalama yaşı 49.5(29-80). 132 hasta(% 78.6) skuamoz hücreli , 23 hasta (%13.7) adenokanser olup 13 hasta (% 7.7) adenoskuamoz hücreliydi.Ortalama takip süresi 31.5 ay (3-75 ay)’dır.Bu hastalara uygulanan cerrahiler ise ;139 hasta(% 82.7) Tip3+bso+BPPLND, 14 hasta (%8.3 ) Tip 3+USO+BPPLND, 2 hasta (%1.2) TAH+BSO+BPPLND, 7 hasta(%4.2) Radikal trakelektomi+BPPLND, 4 hasta (%2.4) Parametrektomi+1/3 Üst vajenektomi+BPPLND, ve 2 hastaya (%1.2) Geniş Konizasyon Konizasyon +BPPLND şeklindedir. İstatiksel analiz SPSS 17 paket programı kullanılarak yapılmış olup tüm testlerde istatistiksel önem düzeyi p< 0.05 olarak alınmıştır. sonuç: Düşük risk faktörüne sahip(tümör boyutu ≤ 2 cm, DSİ≤ %50 LVSİ negatif) 26 hasta bulundu ve bu hastaların hiç birinde parametriyal tutulum , lenf nodu metastazı, hastalığa bağlı ölüm ve rekürrens izlenmedi.Litaratürdeki çalışmalara paralel sonuçlar bulundu.Erken evre düşük risk faktörüne sahip hastalarda konzervatif cerrahi yaklaşım umut vericidir ve halen devam etmekte olan prospektif çalışmaların sonuçları bu yolda ışık tutacaktır. Objective: The aim of the study is to identify early stage (1A2-1B1) low risk (tumour size ≤ 2 cm, stromal invasion ≤50 %, and LVSİ negative) cervical cancer patients and to evaluate parametrial involvement and the role of less radical surgery(cone, simple trachelectomy, simple hysterectomy) Material and Methods: We evaluated cervical cancer patients operated in Baskent University Ankara Hospital at Gynecologic Oncology Division between the years 2007- 2013.We identified 168 patients as stage 1A2 and 1B1.13 patients was stage 1A2 (7.7 %) and 155 patients was stage 1B1 (92.3 %).Mean age was 49.5(range , 29 -80).Histologies were ;132 patients (78.6 %) squamous cell, 23 patients (13.7 %) adenocancer, 13 patients (7.7 %) adenosquamous cancer respectively.Mean follow-up time was 31.5 months(range , 3-75 months).139(82.7 %) patients was performed Radical hysterectomy+BSO+BPPLND, 14 patients (8.2 %) was performed Radical hysterectomy+USO+BPPLND, 7 patients (4.2 %) was performed radical trachelectomy +BPPLND, 4 patients (2.4 %) was performed parametrectomy+upper 1/3 vagenectomy+BPPLND and 2 patients(1.2 %) was performed Conization +BPPLND .Statistical analyses are done by using SPSS 17. Results:We identified 26 patients with early stage , low risk factors and there was no parametrial involvement, lymph node metastases , recurrance and death of disease in these patients.And these results was similar with retrospective studies in litarature.Conservative surgical approach is hopefull in low risk-early stage cervical cancer patients and the results of ongoing prospective randomised studies are waiting for safety and feasibility of conservative management in these patients.
