Tip 1 diyabetes mellitus tanılı erişkin hastalarda VHIT , C-VEMP , O-VEMP ile vestibüler sistem değerlendirilmesi

Abstract

Amaç: Bu çalışmada amacımız tip 1 Diyabetes Mellitus (DM) tanılı erişkin hastalar ve kontrol grubu katılımcılarda VHIT, C-VEMP, O-VEMP testlerini karşılaştırarak, diyabetli hasta grubunda periferik vestibüler sistemin etkilenip etkilenmediğini incelemektir. Yöntem: Bu çalışma 2020 yılında, 18-50 yaş arasındaki Başkent Üniversitesi Kulak Burun Boğaz Anabilim Dalı’na başvuran 21 kontrol grubu ve Endokrinoloji Bölümü’ne başvuran Tip 1 DM tanılı 20 hasta grubu katılımcısıyla Otoloji kliniğinde yürütüldü. Katılımcılara, detaylı Kulak Burun Boğaz muayenesinden sonra timpanometri, VHIT, C-VEMP, O-VEMP testleri yapıldı. Bulgular: Çalışma kapsamındaki 41 katılımcının 23’ü kadın, 18’i erkekti. Sağlıklı kontrol grubun yaş ortalaması 31,14±7,38, hasta grubunun yaş ortalaması ise 29,8±8,50 olarak hesaplanmıştır. Tip 1 DM hastalarının kontrol grubuyla C-VEMP P1, N1 latanslarını karşılaştırdığımızda hasta grubun P1 ve N1 latanslarında uzamış sonuçlar elde ettik. Sonuçlar arasında istatistiksel olarak anlamlı bir fark olduğu saptanmıştır (p<0,05). Buna ek olarak C-VEMP amplitüdlerinde ve VHIT, O-VEMP testlerinin hem latans hem de amplitüd sonuçlarında gruplar arasında istatistiksel olarak anlamlı bir fark saptanmamıştır. Sonuç: Hasta grup C-VEMP latanslarındaki gecikmeler doğrultusunda sakkül makülası ve/veya inferior vestibüler sinir yolağının etkilenmiş olduğunu düşünmekteyiz. Literatür bilgileri ve sonuçlarımız değerlendirildiğinde tip 1 DM hastalarında vestibüler sistem değerlendirilmesi ile ilgili daha kapsamlı çalışmalara ihtiyaç olduğu görüşündeyiz. Objective: The aim of this study is comparing the VHIT, C-VEMP, O-VEMP tests of adult patients with type 1 diabetes mellitus (DM) and control group volunteers were examined whether the peripheral vestibular system is affected in the diabetic patient group. Methods: This study was conducted in the Otology clinic in 2020 with 21 control group who applied to Başkent University Department of Otorhinolaryngology and 20 patient group participants with Type 1 DM who applied to the Department of Endocrinology. After a detailed Ear Nose Throat examination, Tympanometry, VHIT, C-VEMP, O-VEMP tests were performed to all participants. Results: Of the 41 participants in the study, 23 were women and 18 were men. The average age of the healthy control group was 31,14±7,38 and the average age of the patient group was 29,8±8,50. When we compared C-VEMP P1, N1 latencies of type 1 DM patients with the control group, we obtained prolonged results in the P1 and N1 latencies of the patient group. A statistically significant difference was found between the results (p<0,05). In addition, no statistically significant difference was found between the groups in the amplitudes of C-VEMP and the latency and amplitude results of VHIT and O-VEMP tests. İn Conclusion, we think that the saccule macula and / or the inferior vestibular nerve pathway are affected due to the delay in the patient group C-VEMP lattices. When the literature information and our results are evaluated, we think that more comprehensive studies are needed on the evaluation of the vestibular system in type 1 DM patients.
