Behçet hastalığının denge sistemine olan etkilerinin video baş savurma testi ile değerlendirilmesi

Abstract

Giriş Behçet hastalığı (BH) küçük damarları tutan bir vaskülit olup birçok sistemi etkilemektedir. Yapılan çalışmalarda BH’da vestibüler sistemin etkilenmiş olabileceği bildirilmiş olup, bu çalışmada BH ve normal bireylerin işitme ve denge sistemlerinin incelenmesi amaçlanmıştır. Materyal ve Metod Bu çalışma, Başkent Üniversitesi Tıp Fakültesi Girişimsel Olmayan Klinik Araştırmalar Merkezinden (KA17/136 proje nolu) etik kurul onayı alınarak yapılmıştır. 31 adet Behcet tanısı olan hasta ve 31 sağlıklı birey çalışmaya dahil edilmiştir. Bütün katılımcılara vestibüler sistem bütünlüğünün değerlendirilmesi için saf ses odyometrisi, Video Head Impulse test (vHIT), Post Head Shake Nistagmus test ve baş dönmesi engellilik anketi (DHI) uygulanmıştır. Veriler uygun istatistiksel yöntemler ile değerlendirilmiştir. Bulgular Behcet hastalarında işitme ve denge sistemlerinin belirgin şekilde etkilendiği görülmüştür. vHIT sonuçlarında BH’nın %12,9’unda sol lateral kanalda, %25,81’inde sağ lateral kanalda overt ve covert sakkad gözlenmiştir. Post head shake nistagmus testi sonucunda BH catch up sakkad gözlenen hastalarda nistagmus varlığı izlenirken, odyometre sonuçlarında ise hastalarda işitme kaybı izlenmiştir. DHI sonuçlarında ise Behçet hastalarında elde edilen skorlar anlamlı düzeyde yükselmiştir.Sonuç vHIT BH’nda vestibüler sistemi değerlendirirken diğer testlere pratik bir alternatif oluşturabilecek kısa ve güvenilir bir testtir. Behçet hastalarında nörobehçet gelişmeden vestibüler sistemin etkilendiği görülmüştür. Introduction Behcet’s disease (BD) is a vasculitis, which involves all small vessels, and influences the multiple systems of the human body. Latest studies mentioned the impact of BD on vestibular system, as well. This study aims to compare the vestibular system results of BD patients and healthy individuals. Material and Method This study was held after the approval of local ethical committee (Baskent University Medical Faculty non-invasive Clinical Studies Center, K17/136 project number). 31 patients, which were diagnosed with BD, and 31 healthy individuals were involved to the study. All the subjects were evaluated via pure tone audiometry, video head impulse test (vHIT), post head shake nystagmus test (pHSNT) and dizziness handicap inventory (DHI) to check for vestibular system integrity. The data was analyzed according to proper statistical method. Results The vestibular system was significantly affected in Behcet patients. In vHIT, 12.9% of patients had overt and covert saccade on left lateral semicircular canal and 25,81% of patients had overt and covert saccade at the right lateral semicircular canal. In pHSNT, catch up saccade and presence of nystagmus were revealed in patients and patients had also hearing loss in pure tone audiometry. DHI results of patients were found significantly high.Conclusion vHIT, which is a short and reliable, may be an alternative test to assess the vestibular system. The affection of the vestibular system was revealed in the patients before the neuro-Behcet appearance.
