Sağlık personeli ve hastaların, hasta hakları konusunda bilgi düzeyleri ve uygulanan müdahale yöntemlerinin etkinliğinin değerlendirilmesi

Abstract

Başkent Üniversitesi Hastanesi ve Bağlı Sağlık Kuruluşları’na başvuran hastaların ve çalışan personelin hasta hakları ve yasal mevzuat konusundaki bilgilerinin belirlenmesi; bu konu ile ilgili farklı müdahale yöntemleri uygulayarak sağlık bilinci geliştirmede bireylerde olumlu yönde değişim sağlamak, bu araştırmanın temel amaçlarıdır. Bu araştırma müdahale tipi epidemiyolojik bir araştırma olup, 2006 yılında personel ve hastaların hasta hakları konusunda bilgi düzeyi belirlenmiş ve 2007 yılında müdahale yöntemleri uygulanarak, müdahalelerin etkinliği değerlendirilmiştir. Veri toplama aracı olarak anket formu kullanılmıştır. Müdahale yöntemleri olarak, personele eğitim verilmiş ve eğitim kitapçığı hazırlanmış; hastalara ise hasta hakları konusunda broşür ve hasta danışmanları aracılığı ile bilgilendirme yapılmıştır. Araştırmaya toplam 1240 personel ve 988 hasta katılmıştır. Katılım oranı: müdahale öncesi grupta personel ve hasta için tam iken, müdahale sonrası grupta hastaların örneklem sayısının %72’sine ulaşılmıştır. Hesaplanan bilgi puanı personel ve hastalar için müdahale öncesi ve sonrası gruba göre değerlendirilmiştir. Müdahalenin personelin bilgi düzeyi puanını 4,9 kat etkilediği ve müdahale sonrası grupta bilgi düzeyi skorunun artmış olduğu görülmüştür. Hastaların müdahale sonrası grupta bilgi düzey puanları artmış olmasına rağmen müdahalenin istatistiksel açıdan gruplar arasında farklılık oluşturmadığı saptanmıştır. Araştırmadaki incelenenlerin hasta hakları ile ilgili ülkemizdeki mevzuatı(hasta hakları yönetmeliği) okuma durumu değerlendirilmiştir. Müdahale öncesi personelin %12,6’sı yönetmeliği okurken, müdahale sorası %25,2’sinin okuduğu görülmüştür. Müdahalenin personel üzerinde yönetmeliği okuma durumuna 2,3 kat etki ettiği saptanmıştır. Tüm hastaların %8’nin yönetmeliği okuduğu görülmüştür. Hastalar açısından müdahale yöntemlerinin yönetmeliği okuma durumunu etkilemediği saptanmıştır. Araştırmada kullanılan müdahale yöntemleri personel ve hastalar da bilgi düzeyini arttırmakla beraber devamlılığının da sağlanması önem taşımaktadır. The main objectives of this research is to determine the knowledge level of hospital employees and patients who applied to Başkent University Hospital and Baskent University Research and Application Centers about awareness of patients’ right and legislation; and to provide positive change in the development of individuals’ health consciousness by implementing different intervention methods on this subject. This research is a field trial (intervention) study. The knowledge level of the hospital employees and patients about patients’ rights is determined in 2006 and after 6 months of implementing the interventions the efficiency of the interventions is evaluated in 2007. The questionnaire is used as a data collection tool. The intervention methods comprise employee training course and training book, brochure about the patients’ rights and information to patients by patients’ advisers. 1240 staff and 988 patients are participated in the research. The participation rate is 100% for the employee and the patient before the intervention while the rate is decreased to 72% after the intervention for patients. The calculated knowledge scores for groups employee and patients are evaluated according to the before and after intervention. The intervention makes an increment of the knowledge score of employee 4.9 times more and the knowledge level score of the group increases after the intervention. Although the information level scores of patients increase after the intervention, it is not statistically significant. It is observed that the legislation is read by the 12.6% of the employee before intervention while the 25.2% of the employee read it after the intervention. The intervention on the employee affects the reading status of legislation 2.3 times more. It is seen that the 8% of all patients read the legislation. It is confirmed that the intervention methods do not affect the reading status of patients. Intervention methods used in this research seem to increase patients’ and employees’ knowledge levels. However, ensuring its continuity is also very important.
