Item New Concept Design of an Insulin Pen for Visually Impaired or Blind Diabetius Mellitus Patients(2015) Ucar, Tuba; Kocak, Onur; Akpek, Ali; ABI-1247-2020Today, diabetes mellitus (diabetes) emerges as an increasingly important health problem because of the problems and frequency that it cause all over the world. Especially type 2 diabetes mellitus is increasing rapidly in all of the developed and developing society with the rapid changes in lifestyle. As of the year 2013, the number of the diabetes patients are 382 million. And this number is expected to reach 592 million increased by %55 in 2035. The main reason of this population growth due to the obesity and physical inactivity is increasing beacuse of urbanization and aging. In our country, according to result of the studies of Turkey diabetes apidemiology (TURDEP-I), type 2 diabetes mellitus %7.2, frequency of impaired glucose tolerance (IGT) %6.7 was found. In the studies of TURDEP-II that is published recently has been seen significantly increase of 2 type diabetes mellitus at age of 20 and over. Uncontrolled diabetes leading to hypoglycemia causes development of complications that affect all systems of the body. (cardiovascular system-eyes-kidney-nervous system) The risk of lower extremity amputation in diabetes patients is 25 times higher than without diabetes. Especaially in developed countries, the most common cause is visual loss and blindness. Methods: We used the short TROND protocol by QTRAC to measure axonal excitability parameters (strength-duration time constant (SDTC), rheobase, etc.) in 12 healthy subjects and 14 DM patients with PNP and 10 DM patients without PNP. The short TROND protocol was performed before and after 20 min of deep hyperventilation in healthy subjects and patients with DM. Also, venous blood pH and partial pressure of O-2 and CO2 were recorded before hyperventilation (HPV) and after 20 min of HPV. A "hyperventilation score" was evaluated before and after HPV. Results: When the values of DM with PNP group and control group before HPV were compared, SDTC and latency were statistically significant. Comparing the values of the excitability parameters after HPV showed statistically significant changes in the SDTC, rheobase, and refractoriness at 2.5 ms in controls and DM patients without PNP. HPV resulted in no changes in SDTC in DM patients with PNP. Conclusion: The results of this study suggest that patients with DM and healthy subjects have different responses to HPV, and pH changes have different effects on diabetic PNP compared with healthy controls and DM patients without PNP. All rights reserved.Item Consensus Statement of Endocrinology, Cardiology, and Nephrology(ENCARNE) Experts on Prevention, Diagnosis, and Management of Cardiovascular and Renal Complications of Diabetes(2021) Altay, HakanAn array of medical practitioners, including endocrinologists, family physicians, internal medicine specialists in nephrology and cardiology, unceasingly investigate, diagnose and treat over 8 million diabetic patients in Turkey. Apart from routine glycemic regulation, several frequent coexisting comorbidities such as obesity, hypertension, dyslipidemia, and their associated complications should also be promptly managed. Due to the concomitant occurrence of complications, the involvement of additional specialties in the precise management of such conditions becomes indispensable. Owing to the ever-expanding knowledge about the prevalence and clinical manifestations of diabetes, various international medical societies publish annual diabetes guidelines, which makes it too cumbersome as well as challenging for the practicing physicians to follow these comprehensive guidelines in clinical practice. There is an unmet need for an easyto-read and concise document for all physicians working for diabetes management for a standardized approach for better management of diabetes and improved patient care. This consensus report was prepared collectively by the Society of Endocrinology and Metabolism Turkey, Turkish Society of Cardiology, Turkish Society of Nephrology, Turkish Society of Hypertension and Renal Diseases to prevent cardiac and renal complications of diabetes, to timely detect these complications by using pertinent measures and to develop, implement and monitor strategies for managing them effectively.Item Sodium-glucose cotransporter-2 inhibitors in heart failure patients: an appraisal of recent cardiovascular outcome trials(2020) Kocyigit, Duygu; Kocyigit, Alime S.; Hussain, Muzna; 33059407Although initially introduced as a novel oral glucose-lowering agent class, cumulative evidence from randomized controlled trials (RCTs) have led sodium-glucose cotransporter-2 inhibitors (SGLT2i) to become a component of primary and secondary prevention from atherosclerotic cardiovascular disease in patients with type 2 diabetes mellitus (T2DM). Dapagliflozin, one of the agents of this class, was investigated in 2019 in patients with heart failure (HF) independent of being diagnosed with T2DM at baseline. Since then, many other studies are being undertaken in this class of drugs. Herein, we aimed to review the RCTs, their subgroup and post-hoc analyses that examined the effects of SGLT2i on cardiovascular outcomes (including HF-related outcomes) in patients diagnosed with HF that were published until June 2020. We also summarized the ongoing trials that aim to assess the impact of SGLT2i on cardiovascular outcomes in patients with HF and listed available guideline recommendations regarding the use of SGLT2i for cardiovascular disease management.
