    Perceptions and Experiences of Privacy Among Persons With an Ostomy Receiving Treatment and Care A Mixed-Methods Study

    PURPOSE: The aim of this study was to determine ostomy patients' experiences and opinions on protection of privacy during treatment and care. DESIGN: Mixed-methods, descriptive correlational study. SAMPLE AND SETTING: The study sample comprised 80 patients with an ostomy receiving treatment and care in a university hospital in Turkey. METHODS: Data were collected using a form developed by the researchers based on literature review. The survey consisted of statements that examined demographic characteristics, experiences, and opinions of patients with an ostomy. RESULTS: For participants, 64.8% (n = 35/54) defined privacy from a physical dimension, 64.1% (n = 25/39) suffered from ostomy-related psychosocial discomfort, 48.7% experienced ostomy-related discomfort mostly during nursing care (n = 19/39), 48.7% (n = 39/80) were uncomfortable when their ostomy was seen by others, Approximately 56.3% (n = 45/80) thought that they disturbed other patients and their families during ostomy care, and 46.3% (n = 37/80) stated that healthcare professionals discussed information about their ostomy when other patients and their families were present. In addition, 21.3% (n = 17/80) opined that the physical conditions of patient rooms failed to provide privacy. Analysis revealed no significant relationship between education levels and reading the Patient Rights Regulation (P = .129). Analysis also found no significant relationships between age (P = .086), marital status (P = .382), educational background (P = .143), hospitalization experience (P = .427), and satisfaction with perceptions of privacy when receiving ostomy care. CONCLUSION: Patients with an ostomy reported physical and psychosocial problems concerning privacy, felt uncomfortable about the way information on their ostomy was discussed, and thought that physical conditions were not suitable for ostomy care. We recommend that healthcare professionals be provided with more training on psychosocial problems and privacy for patients with ostomy and that institutional policies and procedures be developed to address patients' problems.

    Validity and reliability of the Turkish version of the Patient Dignity Inventory

    Objective The aim of this study was to investigate the validity and reliability of the Patient Dignity Inventory (PDI) in the Turkish society, which was developed to evaluate dignity-related distress in palliative care patients. Methods One hundred and twenty-seven adults with advanced cancer hospitalized in several clinics of two university hospitals were included in the study. The patients whose Palliative Performance Scale score was at least 40% were recruited to study. The data were collected with a patient demographic form, the Turkish version of Hospital Anxiety and Depression Scale (HADS-TR), and the Turkish version of the PDI (PDI-TR). The PDI-TR was finalized and back-translated after translating into Turkish and obtaining 10 expert opinions. Exploratory and confirmatory factor analysis, internal consistency, concurrent validity, and test-retest reliability analysis were performed. Results The Cronbach's alpha coefficient of PDI-TR was 0.94. Factor analysis resulted in a five-factor solution, and all items were loaded on factors. Factors were labeled as symptom distress, existential distress, self-confidence, dependency, and supportive care needs and accounted for 68.70% of the overall variance. The model's normed fit index, comparative fit index, and X-2/SD were found between acceptable range (0.90, 0.93, and 2.64, respectively). A positive and strong correlation was found between subdimension scores of HADS-TR and the total score of PDI-TR (r = 0.70 for anxiety subdimension; r = 0.73 for depression subdimension). The test-retest reliability was conducted with 32 patients within the sample two weeks after the first application, and no significant difference was found between the two application scores as the result of paired-sample t-test (p > 0.05). An intraclass correlation coefficient of test-retest reliability was r = 0.855. Significance of results PDI-TR was found to be a valid and reliable tool in palliative care patients in Turkish society.

    Awareness of Skin Cancer, Prevention, and Early Detection among Turkish University Students

    Objective: The aim of this study was to determine the awareness about skin cancer, prevention, and early detection among university students. Methods: This descriptive cross-sectional study was carried out with 404 students in a university located in Ankara, the capital city of Turkey. A 35-item questionnaire was used for data collection. Results: Less than half of the students (37.9%) had knowledge about skin cancer mostly through the internet (24.5%) and media (24.1%). Half of them aware of the risk factors; mostly as avoiding direct exposure to the Sun between 10 am and 4 pm (45.3%); smoking and alcohol (38.4%); having fair skin color (34.9%); and ultraviolet light exposure (25.7%). Only one-third of them (32.9%) are knowledgeable about skin cancer signs and symptoms, such as a change in color and appearance of the nevus/moles (24%). The majority of the responders (77.3%) did not know about screening tests for skin cancer and only 18 (4.5%) students were practicing skin self-examination. Conclusions: This study showed a lack of knowledge about skin cancer, prevention, and early detection among university students and reported the need for educational interventions to raise awareness in this target group.

    The effect of structured education to patients receiving oral agents for cancer treatment on medication adherence and self-efficacy

    Objective: This study was conducted to examine the effect of structured education on medication adherence and self-efficacy through the use of the MASCC Oral Agent Teaching Tool (MOATT) for patients receiving oral agents for cancer treatment. Methods: This quasi-experimental study has been conducted at two hospitals; 41 patients were included in the study. Data were obtained using a questionnaire, medication adherence self-efficacy scale (MASES), memorial symptom assessment scale, and a follow-up form (diary). Patients were educated through the use of the MOATT at a scheduled time; drug-specific information was provided along with a treatment scheme and follow-up diary. Phone interviews were completed 1 and 2 weeks after the educational session. At the next treatment cycle, the patients completed the same questionnaires. Results: Majority of the patients were receiving capecitabine (90.2%; n = 37) as an oral agent for breast (51.2%; n = 21) and stomach cancer (24.6%; n = 10) treatment. About 90.2% of patients (n = 37) stated that they did not forget to take their medication and experienced medication-related side effects (78%; n = 32). The total score of MASES was increased after the education (66.39 vs. 71.04, P < 0.05). Conclusions: It was shown that individual education with the MOATT and follow-up for patients receiving oral agents for cancer treatment increased patient medication adherence self-efficacy.

    Effects of pre-operative individualized education on anxiety and pain severity in patients following open-heart surgery

    Objectives: Individualized education is important for preparing patients for the operation both physically and psychologically. This study investigated the effects of pre-operative individualized education for open-heart surgery patients on post-operative anxiety and pain severity. Methods: This quasi-experimental study was conducted in a university hospital between January and October 2014 and involved 109 patients. Data were collected through a form developed by the authors, the State-Trait Anxiety Inventory (STAI), and the visual analog scale (VAS). Patients underwent STAI 1 day before the operation to identify their sources of anxiety and educational needs, and then individualized education was given accordingly. 1 day after the operation, STAI was used to measure patients' state anxiety level, and VAS was used to measure their pain level. The effect of demographic variables on differences in anxiety and pain was investigated. Results: The average age of the participants was 59.62; 69.7% were male, 92.7% were married, 49.5% graduated from primary school, and the majority (71.6%) had coronary heart disease. The most common sources of anxiety reported the participants included lack of knowledge, being away from family, risk of death, and pain. An analysis of the participants' pre-operative mean scores for state anxiety displayed statistically significant differences (P < 0.05) according to age groups and gender. No significant difference was detected between mean pre- and post-operative state anxiety scores. There was a statistically significant relationship between mean pre- and post-operative state anxiety scores and mean pain scores. Conclusions: The individualized education is given to patients before surgery was found to have potential effects on their post-operative pain levels.
