    Fixed Orthodontic Retainers: A Review

    Orthodontic retention is defined as maintaining teeth in optimal aesthetic and functional position after treatment. Despite the necessity of retention phase and the factors influencing the stability of the teeth after orthodontic treatment was discussed by the orthodontist for a long time, it is accepted that a retention phase is essential for stability of orthodontic treatment results nowadays. Therefore, the application of a suitable retention method is important both for prevention of relapse after orthodontic treatment and for increasing patient satisfaction. Removable appliances had been used for many years for retention purposes. Later, fixed retainers were introduced to prevent relapse as having a number of advantages, such as better aesthetics, no need for patient cooperation, effectiveness, and suitability for lifelong retention. However, their need for precise bonding technique, fragility, and tendency to cause periodontal problems by weakening oral hygiene are some of their disadvantages.

    Appraisal of the relationship between tooth inclination, dehiscence, fenestration, and sagittal skeletal pattern with cone beam computed tomography

    Objectives: To examine the relationship between sagittal facial pattern and dehiscence/fenestration presence in conjunction with buccolingual tooth inclination by using cone beam computed tomography. Materials and Methods: The study was carried out on the cone beam computed tomography scans of the following three groups of patients (n = 20 in each group): Class I, Class II, Class III. Buccolingual tooth inclination, buccal dehiscence/fenestration presence, and lingual dehiscence/fenestration presence were evaluated on each tooth. Analysis of variance, Kruskall-Wallis H, Scheffe, and chi-square tests were used for statistical comparisons. Results: Differences (P<.05) were observed between the groups for inclination of upper incisors and all lower teeth except for second molars. Dehiscence prevalence in the upper buccal and posterior buccal regions was higher (P<.05) in the Class I group when compared with the other groups. Lower buccal and anterior buccal regions showed higher (P=.0001) dehiscence prevalence in all groups. No difference was observed in fenestration prevalence between the groups. The upper buccal and anterior buccal regions showed higher (P=.0001) fenestration prevalence in all groups. Conclusions: Orthodontists must consider concealed alveolar defects in treatment planning to avoid gingival recession or tooth mobility.

    An unusual case of invasive cervical resorption after piezosurgery-assisted en masse retraction

    A 16-year-old patient sought orthodontic correction for profile improvement and labially inclined maxillary incisors. She had Class II malocclusion, protrusive maxillary and mandibular incisors, and increased overjet and overbite with an American Board of Orthodontics discrepancy index value of 25. She was treated with maxillary premolar extractions and miniscrew-supported en masse retraction assisted with piezoincisions. Extraction spaces (7.5 mm per side) were closed with maximum anchorage in 10 months. Total treatment time was 23 months. Twenty-seven months after debonding, a pink spot was noted at the buccocervial region of the left central incisor. Radiographic evaluation on cone-beam computed tomographic scans revealed a severe case of invasive cervical resorption on both central incisors, around which the piezosurgical cuts had been made. Treatment proceeded with a nonintervention approach and the affected teeth were reinforced with a lingual retainer.

    Versatility of high resolution ultrasonography in the assessment of granulomas and radicular cysts: a comparative in vivo study

    Objectives: To evaluate and compare the diagnostic potential of high resolution ultrasound with periapical radiographs (PR) and CBCT in assessing granulomas and radicular cysts. Methods: This study included a total of 33 teeth from 33 patients with periapical lesions. Subjects were distributed among three groups. A consisted of teeth that were extracted. B consisted of teeth treated with root-canal treatment followed by apical surgery. C consisted of teeth treated with root-canal treatment only. Pre-treatment PR, ultrasound and CBCT images were obtained for Groups A, B and C and 6 month post-treatment PR and ultrasound images were obtained for Groups B and C. In addition, histopathological analysis was performed on lesions in Groups A and B. Lesions were classified as either cystic lesions or granulomas. Width, height, depth, surface area and volume of lesions were measured using the built-in softwares of the appropriate imaging modalities. Measurements were compared by Wilcoxon and paired sample t tests. Ultrasound and histopathological findings were compared with lc and Mc Nemar. Statistical significance was set at p < 0.05. Results: kappa coefficient (0.667; p = 0.002) suggested good agreement between ultrasound and histopathology. No statistically significant differences were found among periapical radiography, CBCT and ultrasound in the pre-treatment measurements of lesion width ( p = 0.308) or between CBCT and periapical radiography in the pre-treatment measurements of lesion height ( p = 0.863). In all cases, mean measurement values for all variables were lower for ultrasound than for CBCT. Conclusion: Ultrasound provided useful information for the diagnosis and assessment of granulomas and radicular cysts.

    Biomechanical evaluation between orthodontic attachment and three different materials after various surface treatments: A three-dimensional optical profilometry analysis

    Objectives: To determine the best bonding method of orthodontic attachment among monolithic zirconia, feldspathic porcelain, hybrid porcelain, and the impact of surface-conditioning methods using a three-dimensional optical profilometer after debonding. Materials and Methods: 56 feldspathic porcelain, 56 monolithic zirconia, and 56 hybrid porcelain samples were divided into four surface treatment subgroups: (1) hydrofluoric (HF) acid etch + silane, (2) Al2O3 sandblasting + silane, (3) silicoating (SiO2), and (4) diamond bur + silane. The specimens were tested to evaluate shear bond strength (SBS). Residual composite was removed after debonding. Three-dimensional white-light interferometry was used to obtain quantitative measurements on surface roughness. Results: The highest SBS value was found for the HF acid-etched feldspathic porcelain group. The average surface roughness values were significantly higher in all material groups in which diamond bur was applied, while roughening with Cojet provided average surface roughness values closer to the original material surface. Conclusions: Variations in structures of the materials and roughening techniques affected the SBS and surface roughness findings.

    Comparison of periapical radiography, panoramic, and cone-beam CT in the detection of dental caries in dog teeth

    The aim of this study was to compare the effectiveness of panoramic, periapical and two different Cone Beam Computed Tomography (CBCT) devices in the detection of dental caries of dog teeth ex vivo. A total of 880 teeth were investigated, 33 of which were with caries, whereas; 33 healthy teeth were the controls. Periapical, panoramic and CBCT scans were made for the assessment of the teeth. All images were evaluated separately by two observers experienced in image interpretation. The presence or absence of occlusal caries was scored using a 5-point scale. Kappa values were calculated to assess intra and interobserver agreement. Receiver Operating Characteristic (ROC) analysis was performed to compare the effectiveness of different imaging methods in the detection of dental caries. For both observers, the order of success of the image sets in the estimation of the caries tooth was CBCT Morita, CBCT Iluma, periapical and panoramic radiograph (Area Under Curve (AUC): 0.929, 0.882, 0.861, and 0.704 for observer 1, AUC: 0.927, 0.896, 0.875, and 0.693 for observer 2, respectively). CBCT was found to be the best imaging method for the ex vivo detection of caries in dog teeth. In addition, panoramic images performed worse than all other modalities.

    Modified Haas Expander for the Treatment of Anterior Openbite and Posterior Crossbite Associated with Thumb Sucking-A Case Report: 3-Years Follow-Up

    Thumb sucking is an abnormal habit that occurs in childhood and can cause several malocclusions if it persists for a long time. Malocclusions caused by oral habits require proper treatment timing to maintain a normal growth and should be treated at an early age. This case report shows the management of thumb sucking and early correction of anterior open bite and posterior crossbite by a modified Haas expander. Three-year follow-up results showed the effectiveness of this special designed appliance.

    Riga-Fede disease like ulcers in old age: A case report

    Riga-Fede disease (RFD) is a traumatic, reactive benign disorder characterized by persistent ulceration on the tip or ventral surface of the tongue, seen mainly in infants and children. Lesions tend to develop after the eruption of natal or primary incisors, resulting from repetitive traumatic damage due to backward and forward movements of the tongue over the lower incisors. A literature survey has revealed a very limited number of reported cases of RFD in adults. Herein we reported a 70-year-old female patient who developed RFD-like ulcers on the tongue and buccal mucosa during the previous two months, while under treatment of dental implants. Histopathological examination and direct immunofluorescence of the ulcers and periulcer area did not yield a specific diagnosis. The lesions were resistant to systemic steroid treatment, however, after applying for a soft dental plate nightly for protection of the tongue and buccal mucosa, all ulcers completely healed in two months. With regard to the presented patient, we have reviewed the cases of RFD or RFD-like ulcers reported in adults and discussed the factors contributing to ulcer formation in our patient.

    Periodontal effects and survival rates of different mandibular retainers: comparison of bonding technique and wire thickness

    Objectives: This non-randomized prospective cohort study aimed to compare the periodontal effects and success rates of mandibular canine-to-canine fixed retainers having different bonding techniques and wire thicknesses. Materials and methods: Hundred patients requiring retention after orthodontic treatment were assigned to five study groups (n = 20 in each group, 61 females/39 males, median age range 16.5-18.0 years). Retention was provided by 0.0215"/direct, 0.0215"/indirect, 0.0175"/direct, 0.0175"/indirect bonded multistranded wires and removable Essix appliances. The primary and secondary outcomes were periodontal effects and success rates. The patients were examined in 1 week, 1 month, 3 months and 6 months follow-up appointments. Plaque index, gingival index, probing depth, marginal recession, bleeding on probing, failure rate per retainer wire and survival of retainer wires were analysed by Kruskal-Wallis H, Mann-Whitney U and chi-square tests. Results: Significant differences were observed between the fixed retainer (FR) and Essix (E) groups in gingival index scores at 1 month [mean FR: 1.13 (95% confidence interval (CI): 0.81-1.44), mean E: 0.40 (95% CI: 0.14-0.69), mean difference: 0.73, P < 0.01], 3 months [mean FR: 0.97/1.01 (95% CI: 0.65-1.30/0.72-1.30), mean E: 0.52 (95% CI: 0.25-0.82), mean differences: 0.45/0.49, P < 0.05], 6 months [mean FR: 0.94 (95% CI: 0.62-1.27), mean E: 0.35 (95% CI: 0.15-0.58), mean difference: 0.59, P < 0.05] and in bleeding on probing scores at 1 month [mean FR: 3.05 (95% CI: 2.12-3.98), mean E: 1.15 (95% CI: 0.42-1.88), mean difference: 1.90, P < 0.01]. The survival rates of retainer wires were 85 per cent for the 0.0215" direct/indirect and 90 per cent for the 0.0175" direct/indirect groups for the 6 months follow-up. Limitations: Six months follow-up period, which demonstrates only short-term outcomes. Conclusions: The periodontal outcomes or survival rates of mandibular fixed retainers were not affected by bonding technique or wire thickness, whereas gingival health improved with Essix retainers but not with fixed retainers.

    Evaluation of the Relationship between Type II Diabetes Mellitus and the Prevalence of Apical Periodontitis in Root-Filled Teeth Using Cone Beam Computed Tomography: An Observational Cross-Sectional Study

    Objective: This study aimed to investigate the prevalence of apical periodontitis (AP) in patients with type II diabetes mellitus (DM) with either optimal glycemic control (OGC) or poor glycemic control (PGC) compared with nondiabetics using cone beam computed tomography (CBCT). Subjects and Methods: The periapical status of 75 teeth with adequate root canal treatment performed at least 1 year ago that could be visualized in the field of view of CBCT images of 43 DM patients (29 females, 14 males) was compared with a control group consisting of 162 teeth of 86 nondiabetics (56 females, 30 males). In addition, the DM group was divided into subgroups according to the patients' mean glycated hemoglobin level as either having OGC or PGC. The periapical status of the teeth was evaluated using the CBCT periapical index (CBCTPAI), and AP was diagnosed as a CBCTPAI >= 1. chi(2), Fisher's exact test, and independent-samples t tests were used for the statistical analysis, and p < 0.05 was considered significant. Results: Significant differences between the DM group and the control group were observed (p< 0.05) in terms of AP (the frequencies of both CBCTPAI >= 1 and CBCTPAI >= 3) and the frequency of cardiovascular disease, while there were no significant differences between the DM subgroups (p > 0.05). Conclusion: The prevalence of AP and severe bone destruction in periapical tissues was significantly higher in the DM patients compared with the nondiabetic patients.
