Item Treatment Outcomes of Breast Cancer Liver Metastasis Treated with Stereotactic Body Radiotherapy(2018) Onal, Cem; Guler, Ozan Cem; Yildirim, Berna Akkus; https://orcid.org/0000-0002-2742-9021; https://orcid.org/0000-0001-6908-3412; https://orcid.org/0000-0001-6661-4185; 30296648; HOC-5611-2023; AAC-5654-2020; V-5717-2017Background: To assess the outcomes of breast cancer liver metastasis (BCLM) treated with stereotactic body radiotherapy (SBRT) and systemic treatment. Materials and methods: Patients with oligometastasis at the time of liver metastasis (LM) or who became oligometastatic (<= 5 metastases) after systemic treatment were assessed. Twenty-nine liver metastatic lesions were treated with a total of 54 Gy delivered in 3 fractions. The local control (LC), overall survival (OS), and progression-free survival (PFS) rates were calculated using Kaplan-Meier analyses. Results: A total of 22 patients with 29 liver metastatic lesions treated with liver SBRT between April 2013 and September 2017 were retrospectively analyzed. After a median follow-up time of 16.0 months (range 4.4-59.4 months), 18 patients (82%) had disease recurrence, median of 7.4 months (range 1.0-27.9 months) after completion of liver SBRT. The 1- and 2-year OS rates were 85% and 57%, and the 1- and 2-year PFS rates were 38% and 8%, respectively. The 1- and 2-year LC rates were 100% and 88%, respectively. No significant prognostic factors, including disease extension, size of metastasis, number of liver metastasis and timing of liver metastasis, hormonal status affecting OS, PFS and LC were found. No patients experienced Grade 4 or 5 toxicity; furthermore, only one patient experienced rib fracture 6 months after completion of treatment, and one patient had a duodenal ulcer. Conclusion: This study is the first to evaluate the feasibility of SBRT to BCLM patients. Liver SBRT is a conservative approach with excellent LC and limited toxicities. (C) 2018 Elsevier Ltd. All rights reserved.Item Applicability of the Vienna Sale Agreement (CISG) for Advanced Purchase Agreements of COVID-19 Vaccines in EU Member States(2022) Cortoglu Koca, SemaThe European Commission has signed advanced purchase agreements with various vaccine manufacturers for the production, purchase and supply of the COVID-19 vaccine in the EU. Some parts of the agreement texts with AstraZeneca, CurevacAg and Pfizer Inc./BioNTech have been blacked out. In these agreements, it is understood that Belgian courts are competent and Belgian law is chosen. In a dispute, it is possible for the competent courts to apply CISG since Belgium is a party to the CISG. Even if choice of law hasn't been made, the fact that the workplaces of all parties to these ageements are party to CISG, makes the CISG applicable. This study evaluates the applicability of the CISG to advanced purchase agreements (especially made with AstraZeneca) in EU Member States.Item An Examination of Medicine and Folk Medicine Elements on the Example of a Medical Text in Verse(2022) Aytac, Uyesi AsliPeople have sought cures for the diseases and ailments they have experienced since ancient times. These were sometimes the sorcerers, herbalists, embers, enchanters, atasaguns or shamans of the period, and sometimes the physicians who worked on modern medicine of the period. Anatolia, as the land where civilizations intersect, contains traces of many cultures and has a very deep structure in terms of cultural accumulation. Considering ancient medicine in Anatolia, it can be seen that sometimes modern medical knowledge and traditional folk medicine methods are used together. These methods are frequently mentioned in written texts and many works in the field of health, both in copyright and translation, have been created. The fact that they include easy treatment methods for illnesses and that they contain medical information that the public can use in daily life emphasize the importance of such texts. It is known that the oldest medical texts in Turkish written in Anatolia were created in the 14th century. Since then, many works including field information such as medicine, folk medicine, pharmacology, botany have a special importance in terms of fully revealing the vocabulary and structure of our language. The text, which is our subject of study, is registered in the collection of Ankara National Library Elazig Provincial Public Library with the number 23 Hk 2/3. The title of the work appears to be Manzume-i Tib and its author Ferdi. The text, written in "me fa' i lun me fa' i lun fe' u lun" prosody pattern and consists of 61 couplets. Although the name of the author appears as Ferdi in the manuscript, the note that the author is not known was written at the beginning of the work. Although the year of writing is not known, the text was written in Old Anatolian Turkish. The note to the head of the manuscript contains the information that it contains some information about health protection. In its content, factors affecting health and disease are emphasized. The medical knowledge until the time of writing and some practices from the field of folk medicine have been combined to include behaviors that should be done in the treatment of diseases and avoided for some diseases. Writing a text on the subject of medicine with prosody, besides the purpose of informing, it also ensures the permanence of the information transferred in a certain harmony. In the text, on the subject of diseases; Problems such as indigestion, headache, sore throat or dry throat, weakness, paralysis, sciatica, meningitis, constipation, hemorrhoids, cramps and pain were included. Among the treatments; Methods such as vomiting, laxative, cupping, and consent have been discussed. Although in small numbers, there are herbal preparations that can be used for some ailments such as tooth jaundice and bad breath. In the face of some ailments and conditions, the daily foods recommended for therapeutic purposes have been pointed out. It is seen that information about mood is also included in addition to physical health. In our study, by giving the transcription of the aforementioned manuscript, it will be pointed out the diseases, diseases and the treatment methods recommended for them. These suggestions will be examined under the headings of beneficial and harmful actions. Comparisons will be made by giving examples from other medical texts written in Anatolia. At the end of the review, there will be a glossary of terms used in the fields of medicine and folk medicine. With further examination of such texts, Old Anatolian Turkish vocabulary will be put forward in a healthier way and data that shed light on various fields such as botany, folklore, pharmacology and psychology will be obtained along with medicine.Item Evaluation of the Knowledge Level, Practice and Attitudes of the Caregivers on Ketogenic Diet Management(2022) Bayram, Sinem; Caliskan, HilalObjective: Patients with intractable childhood epilepsy may benefit from ketogenic diet treatment. Management of ketogenic diet depends on the knowledge level and practice and attitudes of their caregivers. Although advances have been made on early diagnosis and treatment options, there is still a lack of knowledge in children with epilepsy and their caregivers. Methods: In the study, 40 voluntary individuals caring for a child with intractable childhood epilepsy who is under a ketogenic diet were included. The visual analog scale was used to determine adherence to the ketogenic diet for both caregivers and children. Dietary adherence of young children (<10 years) was assessed by the caregiver. Ketogenic diet knowledge of caregivers was determined to form with 40 questions (free/prohibited foods, meal contents, any form of carbohydrate sources, label reading, side effect awareness, and management practices). Results: Availability of ketogenic products, feeling of hunger, frequent glucose, and ketone measurement were determined to be the most difficulties in adherence to the ketogenic diet. According to the visual analog scale results, the mean knowledge level of the caregivers was 3.55 +/- 3.67, ketogenic diet adherence was 7.52 +/- 2.40 for the caregivers, and 8.42 +/- 1.62 for the patients. 25% of the caregivers have high, 65% of them have low knowledge. The number of antiepileptic drugs used decreased. The number of seizures per week was 32.65 +/- 21.15 before the ketogenic diet, it decreased to 21.54 +/- 15.69. Conclusions: The effectiveness of the ketogenic diet was better in medium and high knowledge levels. As the competence increases, ketogenic diet management will be easier. Therefore, it would be beneficial to visit a dietitian during clinic visits and to seek answers to food-related problems.Item Reporting quality of animal studies published in journals listed in ULAKBIM TR index: a systematic review on compliance to the ARRIVE guidelines(2022) Aydingoz, Selda Emre; Efe, Oguzhan Ekin; Caliskan, Gokce; 0000-0001-7823-7620; 0000-0002-3243-7843; ABA-4291-2020; W-7908-2019Transparent reporting of animal studies is key to ensure reproducibility. The primary guideline for reporting animal studies is the ARRIVE (Animal Research: Reporting of In Vivo Experiments). We aimed to evaluate the compliance to the updated ARRIVE guidelines (version 2.0, date 2020) in animal studies published in journals indexed by the Turkish Academic Network and Information Centre (ULAKBIM TR Index). This was a retrospective analysis of reporting quality of animal studies published in journals listed in ULAKBIM TR Index between January 2010 and August 2021. The percentage of articles that fully reported each " of the ARRIVE Essential 10: Compliance Questionnaire was calculated, and effects of journal- and article-related factors were evaluated. Totally, 235 articles published in 89 journals were included into the analysis. The mean percentage of fully reported items was 59.1%+/- 10.9%, the least reported items being those related with bias - "blinding", "allocation to study groups", "assessment of statistical assumptions", "excluded animals", and "sample size calculation" (0.9%-10.6%). The journal's publisher, frequency, language, being indexed in the Science Citation Index Expanded, and the ratio of animal studies had no significant effect on this percentage (p>0.05). However, journals supporting the ARRIVE guidelines had significantly and higher compliance (62.1 %+/- 10.1% vs. 58.2%+/- 11.0% for supporters and non-supporters, respectively; p=0.017, 95%CI -7.0 to-0.7). Articles published after 2015 had higher compliance to the ARRIVE guidelines than those published previously (60.9%+/- 10.4% vs. 57.9%+/- 11.1%, respectively; p=0.037, 95%CI -5.8 to -0.2). Although progress has been made, compliance to the ARRIVE guidelines is still low in animal studies published in journals listed in ULAKBIM TR Index-Turkey's scientific journal database that covers over 800 peer-review journals. In order to increase the reporting quality and reproducibility of animal studies, it is imperative to raise awareness among researchers and journals, and to enforce the ARRIVE guidelines in editorial policy of journals.Item Heart failure with non-reduced ejection fraction: Epidemiology, pathophysiology, phenotypes, diagnosis and treatment approaches(2022) Cavusoglu, Yuksel; Celik, Ahmet; Altay, Hakan; Nalban, Sanem; Ozden, Ozge; Temizhan, Ahmet; Ural, Ditek; Unlu, Serkan; Yilmaz, Mehmet Birhan; Zoghi, Mehdi; 35969235Heart failure (HF) has been classified as reduced ejection fraction (HFrEF), mildly reduced ejection fraction (HFmrEF) and preserved ejection fraction (HFpEF) by the recent HF guidelines. In addition, HF with improved ejection fraction has been defined as a subgroup of HFrEF. In HFrEF, diagnostic workup and evidence-based pharmacological and device-based therapies have been well established. However, HFpEF, which comprises almost half of the HF population, represents significant uncertainties regarding its pathophysiology, clinical phenotypes, diagnosis and treatment. Diagnostic criteria of HFpEF have been changed a few times over the years and still remained a matter of debate. New paradigms including a prominent role of co-morbidities. inflammation, endothelial dysfunction have been proposed in its pathophysiology. As a complex, multifactorial syndrome HFpEF consists of many overlapping clinical and hemodynamic phenotypes. In contrast to HFrEF, clinical outcomes of HFpEF have not improved over the last decades due to lack of proven effective therapies. Although HFrEF and HFpEF have different clinical spectrums and proposed pathophysiological mechanisms, there is no clear defining syndrome postulated for HFmrEF. Clinical characteristics and risk factors of HFmrEF overlap with HFrEF and HFpEF. HFmrEF is also referred as a transitional zone for dynamic temporal changes in EF. So. HFpEF and HFmrEF, both namely HF with non-reduced ejection fraction (HF-NEF), have some challenges in the management of HF. The purpose of this paper is to provide a comprehensive review including epidemiology, pathophysiology, clinical presentation and phenotypes of HF-NEF and to guide clinicians for the diagnosis and therapeutic approaches based on the available data in the literature.Item Small genus-4 Lefschetz fibrations on simply-connected 4-manifolds(2022) Altunoz, TulinWe consider simply connected 4-manifolds admitting Lefschetz fibrations over the 2-sphere. We explicitly construct nonhyperelliptic and hyperelliptic Lefschetz fibrations of genus 4 on simply-connected 4-manifolds which are exotic symplectic 4-manifolds in the homeomorphism classes of CP2 #8<(CP2)over bar> and CP2#9<(CP2)over bar>, respectively. From these, we provide upper bounds for the minimal number of singular fibers of such fibrations. In addition, we prove that this number is equal to 18 for g = 3 when such fibrations are hyperelliptic. Recently unver and Orhan [19] have introduced the concepts of P-strong and P-statistical convergences with the help of power series methods and established a relationship between them. In the present paper, we introduce the notion of P-strong convergence with respect to an Orlicz function and prove that all these three concepts are boundedly equivalent provided that Orlicz function satisfies o2- condition. Herein, presented the final images of the patient that demonstrated a superscan appearance in the Gallium-68 PSMA positron emission tomography/computed tomography, which is a rare phenomenon.
