    Effects of Creative Arts Intervention on Anxiety, Depression and Sleep Quality Among Bone Marrow Transplantation Patients During Protective Isolation

    BackgroundProtective isolation remains part of the standard of care for patients undergoing hematopoietic stem cell transplant in many places of the world. The consequences of protective isolation include increased anxiety and depressive symptoms.ObjectiveThe aim of this study was to determine the effects of creative interventions performed during the protective isolation period on anxiety, depression symptoms, and sleep quality of patients who underwent bone marrow transplantation (BMT).Interventions/MethodsThis study was a randomized, controlled, pre-post pilot design. A total of 20 patients made up the sample group. Participants were randomly assigned to the experimental (n = 9) or control group (n = 11). Hospital Anxiety and Depression Scale was applied to patients on the first day of admission to the unit and on the day of discharge. Questions prepared by researchers on sleep quantity and quality were administered every day during the patients' hospitalization. The patients in the experimental group carried out creative arts intervention for a total of 60 minutes a day, 3 to 4 days a week, while they were in the unit.ResultsThere was no difference in depression, anxiety, and sleep quality scores between the groups after intervention. However, depression and anxiety scores significantly decreased after the intervention in the experimental group, and sleep quality scores improved significantly.ConclusionCreative arts intervention may be beneficial for anxiety, depression, and sleep problems among patients undergoing BMT.Implications for PracticeCreative arts intervention is effective in coping with anxiety, depression, and sleep problems that patients may experience due to social isolation during the BMT process.

    Hemodiyaliz tedavisi alan hastaların pozitif ruh sağlığı, öz yönetim ve tedavi uyumu arasındaki ilişkinin incelenmesi

    Bu çalışma hemodiyaliz tedavisi alan hastalarda pozitif ruh sağlığı, öz yönetim ve tedavi uyumu arasındaki ilişkinin incelenmesi amacıyla yapılan tanımlayıcı ve ilişki arayıcı bir araştırmadır. Araştırma Ocak- Haziran 2024 tarihleri aralığında Başkent Üniversitesi Hastanesine bağlı diyaliz merkezlerinde tedavi alan 205 hasta ile gerçekleştirilmiştir. Araştırma verileri sosyodemografik bilgi formu, Pozitif Ruh Sağlığı Ölçeği, Kronik Hastalık Öz Yönetim Ölçeği ve Son Dönem Böbrek Yetmezliği Uyum Ölçeği ile toplanmıştır. Verilerin analizi; tanımlayıcı istatistikler, Student t testi ve ANOVA testi ile gerçekleştirilmiştir. İleri istatistiksel analiz için Bonferroni düzeltmesi yönteminden yararlanılmıştır. İlişki analizleri için Pearson Korelasyon analizi ve sonrasında Lineer Regresyon analizi yapılmıştır. Araştırmanın sonuçlarına göre hastaların Pozitif Ruh Sağlığı Ölçeği toplam puan ortalaması 139,66±10,30, Kronik Hastalık Öz Yönetim Ölçeği Kendini Damgalama puan ortalaması 11,97±3,02, Damgalama ile Baş Etme puan ortalaması 21,03±3,76, Sağlık Bakım Etkinliği puan ortalaması 17,40±2,98 ve Tedavi Uyumu puan ortalaması 18,52±2,22 bulunmuştur. Katılımcıların Son Dönem Böbrek Yetmezliği Uyum Ölçeği toplam puan ortalaması 965,02±108,92 olarak belirlenmiştir. Katılımcıların cinsiyet, medeni durum, eğitim durumu, çocuk varlığı ve başka kronik hastalık varlığı gibi sosyodemografik ve hastalıkla ilgili özelliklerine göre Pozitif Ruh Sağlığı, Kronik Hastalık Öz Yönetim ve Son Dönem Böbrek Yetmezliği Uyum Ölçeği puan ortalamaları arasında fark olduğu belirlenmiştir. Pozitif Ruh Sağlığı Ölçeği ile Damgalama ile Baş Etme, Sağlık Bakım Etkinliği ve Tedaviye Uyum puanları arasında pozitif yönlü ve orta düzeyde; Kendini Damgalama puanları ile negatif yönlü ve orta düzeyde ilişki bulunmuştur. Hastaların pozitif ruh sağlığı düzeyleri yükseldikçe kendini damgalama düzeyleri düşmekte; damgalama ile baş etme, sağlık bakım etkinliği ve tedaviye uyum düzeyleri yükselmektedir. Katılımcıların kronik hastalık öz yönetim düzeylerinin pozitif ruh sağlığı tarafından belirli oranlarda yordandığı saptanmıştır. Sonuç olarak hemodiyaliz tedavisi alan hastalarda pozitif ruh sağlığının kronik hastalık öz yönetimi ve tedavi uyumu üzerinde anlamlı bir etkisi olduğu belirlenmiştir. Bu sonuçlar doğrultusunda hemodiyaliz tedavisi alan hastalarda pozitif ruh sağlığını geliştirecek eğitim ve destek hizmetleri konusunda danışmanlık verilmesi önerilmektedir. This study is a descriptive and correlational research conducted to examine the relationship between positive mental health, self-management and treatment adherence in patients receiving haemodialysis treatment. The study was conducted with 205 patients receiving treatment in Ümitköy, Çiğdem and Yenikent Dialysis Centres of Başkent University Hospital between January and June 2024. The research data were collected with sociodemographic information form, Positive Mental Health Scale, Chronic Disease Self-Management Scale and End Stage Renal Failure Adjustment Scale. Data were analysed using descriptive statistics, Student t and ANOVA tests. Bonferroni correction method was used for further statistical analysis. Pearson Correlation analysis and then Linear Regression analysis were performed for relationship analyses. According to the results of the study, the mean total score of positive mental health of the patients was 139.66±10.30, the mean score of Chronic Disease Self-Management Scale Self-Stigma was 11.97±3.02, the mean score of Coping with Stigma was 21.03±3.76, the mean score of Health Care Effectiveness was 17.40±2.98 and the mean score of Treatment Compliance was 18.52±2.22. The mean score of the End Stage Renal Failure Compliance Scale was found to be 965.02±108.92. It was determined that there was a difference between the mean scores of positive mental health, chronic disease self-management and end-stage renal failure adjustment scale according to sociodemographic and disease-related characteristics of the participants such as gender, marital status, educational status, presence of children and presence of other chronic diseases. A positive and moderate relationship was found between positive mental health and coping with stigmatisation, health care effectiveness and treatment compliance scores, and a negative and moderate relationship was found with self-stigmatisation scores. As the positive mental health scores of the patients increased, self-stigmatisation scores decreased; coping with stigmatisation, health care effectiveness and treatment compliance scores increased. It was found that participants' chronic disease self-management levels were explained by positive mental health at various levels. As a result, it was determined that positive mental health had a significant effect on chronic disease self-management and treatment compliance in patients receiving haemodialysis treatment. In line with these results, it is recommended to provide counselling on training and support services to improve positive mental health in patients receiving haemodialysis treatment.

    The Effect Of Itching On Sleep Quality And Comfort In Patients With Hemodialysis And Renal Transplantation: A Multi-Center Cross-Sectional Study

    Background: The aim of this study was to determine the effect of itching on sleep quality and comfort in hemodialysis (HD) and renal transplant (Tx) patients with itching. Methods: This descriptive, correlational seeking and cross-sectional study was conducted in four hospitals affiliated with a private health group in Istanbul between April and June 2021. The sample of the study consisted of 42 patients receiving HD treatment and 49 patients with renal transplantation. Data were collected using the Patient Information Form, 5-D Itch Scale, Richards-Campbell Sleep Questionnaire and General Comfort Questionnaire. Results: There was a significant difference between the HD and renal transplantation groups in the Direction (p = 0.01) and Disability dimension (p = 0.002) of the 5- D Itch scale in favor of the renal transplant group. The mean sleep quality and comfort scores of the groups were similar and moderate. Itching negatively affects sleep quality in patients receiving HD treatment, and 22 % of the change in sleep quality is explained by disability of itching (R2 = 0.22; p = 0.002). Itching negatively affects comfort in both groups. Itching explains 27 % of the change in comfort level in the HD group (R2 = 0.27; p = 0.002) and 25 % of the change in comfort level in the renal transplantation group (R2 = 0.25; p = 0.001). Conclusions: According the study results during the hemodialysis treatment process, itching is more intense in terms of distribution and disability and affects the quality of sleep of patients. Itching has a significant impact on patients' perception of comfort. Patients experience itching both during HD treatment and after renal transplantation, which significantly impairs their comfort. Therefore, itching should be carefully monitored during renal replacement therapy and its effects on patients should be evaluated.
