Item Clinical features and immunoglobulin replacement therapy outcomes of adults with common variable immunodeficiency: a single centre experience(2021) Musabak, Ugur; Erdogan, Tuba; 0000-0002-3143-2442; 34022776; ABH-5354-2020Background/aim: Common variable immunodeficiency (CVID) characterized by defective immunoglobulin production is the most prevalent form of symptomatic primary immunodeficiency (PID) in adults. We aimed to reveal the clinical features of adults with CVID and to evaluate the effects of immunoglobulin replacement treatment (IRT) on hemato-immunological findings. Materials and methods: This study included 26 adult patients receiving IRT. Two measurements of complete blood counts and major immunoglobulin levels obtained at the beginning-end of follow up period were used for comparisons. Lymphocyte subsets and B-cell subgroups were measured only at the time of presentation. Results: The most common complications were related to respiratory and digestive systems and organomegaly. Chronic diarrhoea and low body weight were positively correlated with the percentage of CD8(+) T cells (p = 0.019 and p = 0.003, respectively) but negatively correlated with the CD4/CD8 ratio and the percentage of CD19(+) B cells (p = 0.019 and p = 0.005 for both parameters, respectively). At the end of period, the distribution of haematological parameters significantly improved, and immunoglobulin M (IgM) level increased to detectable levels (p = 0.035). Conclusions: There are apparent relationships among chronic diarrhoea and low body weight, and deterioration of T and B cell immunity in adults with CVID. IRT improves the whole blood parameters and stimulates immunoglobulin M (IgM) production. It is endemic in more than 70 countries, and more than 200 million people worldwide are infected with Schistosoma. Pulmonary hypertension (PHT) is one of the chronic complications of schistosomiasis. The exact pathogenesis of schistosomiasisassociated pulmonary hypertension (S-PHT) remains unclear, although several mechanisms such as parazitic arterial embolisation, pulmonary arteriopathy, and portopulmonary hypertension have been suggested. Pathological pulmonary vascular changes in S-PHT were found similar to those in idiopathic pulmonary arterial hypertension (IPAH). The fact that schistosomiasis is one of the most common causes of pulmonary arterial hypertension (PAH), particularly in the developing countries, underlines the importance of enhancing our knowledge on this disease. Developments in the treatment of PAH have resulted in improved prognosis and significant increase in life expectancy and quality of life in the last two decades, which has enhanced the importance of S-PHT. Schistosomiasis is treated with praziquantel. Nevertheless, there is limited evidence that this treatment is effective for PHT. Although antihelmintic medications do not lead to significant improvement, they have beneficial effects and may slow down disease progression. Using PAH-specific treatments in the patients with schistosomiasis-associated PAH (S-PAH) can improve prognosis. However, inadequate clinical studies and limited sources in the endemic regions restrict extensive usage of these expensive medications. In this quantitative research, a descriptive model that describes the situation was used. The study was conducted in Ankara, and 424 teachers joined the study. To get data from teachers, two scales were used: 21st Century Educational Administrators' Skills Scale and Strategic Leadership Scale. In order to determine the level of school principals' 21st century skills and their strategic leadership, descriptive analysis was used as well as Pearson Correlation Analysis and Regression analysis. There were significant positive relationships between all dimensions of educational administrators' 21st century skills and strategic leadership behaviors. Besides, education managers' 21st century skills were found to be a significant predictor of their strategic leadership behaviors. As a result of the research, it was revealed that information literacy, technology literacy, accountability, leadership and responsibility were significant predictors of managerial leadership behavior. School administrators need to increase their levels of knowledge literacy, technological literacy, accountability, leadership, and responsibility skills in order to improve their managerial leadership behaviors.Item QTc interval is associated with increased inflammatory markers (neutrophil-to-lymphocyte ratio and LDH level) in COVID-19 patients(2021) Torun, Serife; Ozer, Nurtac; Celik, Olgun; Akcay, M. Sule; 0000-0002-7190-5443; AAD-5477-2021Purpose: This study aimed to investigate the association between QTc interval and laboratory parameters in COVID-19 patients before and after the treatment. Materials and Methods: Forty-three COVID-19 patients who had baseline and follow-up ECG findings and laboratory reports were evaluated and 40 patients were included in the study. Results: Among 40 patients, 16 were women and 24 were men. The neutrophil-to-lymphocyte ratio (NLR) and corrected QT (QTc) interval were significantly higher in females than males. After the treatment, a significant fall in CRP and ferritin values, and significantly prolonged QTc interval were seen. A significant positive correlation was observed between QTc interval and age, LDH levels, neutrophil and leukocyte count, NLR, magnesium levels, and heart rate of the patients prior to treatment. A positive correlation was observed between increased QTc interval and decreased LDH levels and NLR after treatment. Conclusion: QTc prolongation was associated with increased inflammatory markers, increased NLR and LDH levels before and after treatment in COVID-19 patients. The increase in the QTc interval was correlated with the reduction in LDH levels and NLR with treatment.
