“Modifiye Piggy Pack Tekniği” ile Karaciğer Transplantasyonlarında Erken Dönem Sonuçlarımız

Abstract

Amaç: Bu çalışmada Türkiye Yüksek İhtisas Eğitim ve Araştırma Hastanesi Gastroenteroloji Cerrahisi Kliniğinde modifiye piggyback (MPB)-side to side cavocavostomy tekniği ile gerçekleştirilmiş dört karaciğer transplantasyonu ameliyatının sonuçları literatür incelemesi eşliğinde sunulmaktadır. Karaciğer transplantasyonunda cerrahi tekniklerin oturmuş olduğu günümüzde bu yöntem artan bir sıklıkta uygulanmakta olup İngiltere, Fransa ve Belçika’da bir çok merkezde bu teknik rutin hale gelmiştir. 2002ve 2003 yılları içinde dört olguya MPB tekniği ile karaciğer transplantasyonu gerçekleştirildi. Postoperatif takip süresi 15-26 ay arasında olan dört olgu sağ ve poliklinik kontrolü altındadır. 1 olguda Hepatit C rekürrensi mevcuttur. Retrocaval disseksiyon yapılmayan ve venöz devamlılığın korunduğu MPB tekniği güvenle uygulanabileceğini düşündüğümüz bir yöntemdir.

Liver Transplantation with Modified Piggyback Technique: Early Results We present the early results of 4 liver transplantations performed in the Department of Gastrointestinal Surgery of Türkiye Yüksek İhtisas Teaching and Research Hospital in Ankara, Turkey, using the modified piggyback/side-to-side cavocavostomy technique. This technique is frequently used in many centers in England, France, and Belgium. During 2002 and 2003, we performed 4 liver transplantations using the modified piggyback technique, with a postoperative follow-up of 15-26 months. The 4 patients reported are alive at the current time and under the observation of outpatient clinic. One of the patients has recurrent hepatitis C virus infection. We believe that the modified piggyback/side-to-side cavocavostomy technique is a safe and easy technique, which does not require retrocaval dissection and protects the normal venous return.
