Yanığa Bağlı Marjolin Ülser; Olgu Sunumu ve Literatür İncelemesi

Abstract

Özet Marjolin ülseri terimi; yanık skarı, kronik yara, inflamasyon ve irritasyon alanlarından köken alan deri kanserlerine verilen isimdir. Değişik deri kanser türleri bildirilmiş olmasına rağmen en sık görülen türü skuamöz hücreli karsinomdur. Çok agresif klinik gidiş, yüksek metastaz oranı ve kötü prognoz gibi nedenlerle erken evrede teşhis etmek ve erken cerrahi tedavi yapmak önemlidir. Cerrahide, lezyonun geniş eksizyonu ve bölgesel lenf nodu diseksiyonu önerilir. Cerrahi tedavi öncesinde hastalığın yaygınlığını saptamak, tedavi planlaması için önemlidir. Bu çalışmada yakın bölgesel lenf bezlerinde yayılım olmaksızın uzak bölge lenf bezlerinde metastaz oluşturmuş yanık skarına bağlı Marjinal ülser olgusu sunulmuş ve bu hastalığa ait klinik, tanı ve tedavi yöntemleri son literatür eşliğinde gözden geçirilmiştir.

Summary Marjolin Ulcer Developing in Burn Scar Tissue: A Case Report and Review of the Literature A Marjolin ulcer is skin cancer that emerges on the surfaces of burn scars, chronic wound lesions, and inflamed and irritated skin. Although different types of skin cancers have been reported, the most-common type is squamous cell carcinoma. Early diagnosis and prompt surgical treatment are essential because of the aggressive nature of this tumor, its high metastasis rate, and its poor prognosis. Wide excision of the lesion and regional lymph node dissection are recommended. Before treatment is planned, however, the whole body must be scanned to determine the extent of the disease. In this case report, a Marjolin ulcer, originating from a burn scar that had spread to distant lymph nodes without regional lymph node metastases is reported. Clinical diagnosis and treatment of this disease are evaluated; a review of the recent literature is also included.
