Türkiye’de Diyaliz ve Transplantasyon Maliyeti ve Maliyet Düşürücü Önlemler

Abstract

2003 yılında Türkiyede 28,960 hasta renal replasman tedavisi görmektedir. Hemodiyaliz için yıllık gider 22,759 dolardır. Sürekli ayaktan periton diyalizi (SAPD) için ise yılda 22,350 dolar harcanmaktadır. Transplantasyonun birinci ve ikinci yıl maliyetleri sırasıyla 23,393 ve 10,028 dolardır. Diyaliz masrafının en büyük kısmı nı diyalizerler ile eritropoetin ve diğer ilaçlar oluşturmaktadır. Türkiye’de hem hemodiyalizin hem de SAPD’nin masrafı batı ülkelerinden daha azdır. Diyalize yapılan harcamadan tasaruf etmek için alınması gerekli bazı önlemler şöyle sıralanabilir: “membran reuse”u, eritropoetin ve diğer ilaçların kullanımının sıkı denetlenmesi, başta kadaverik olmak üzere transplantasyonun artırılması.

Current Costs of Dialysis and Transplantation in Turkey, and Possible Ways to Reduce Expenses In the year 2003, 28,960 patients in Turkey were on renal replacement therapy. The current yearly cost for hemodialysis is $US 22,759, and for continuous ambulatory peritoneal dialysis is $US 22,350. In comparison, the annual costs of patient care in the first and second year after transplantation are $US 23,393 and $US 10,028, respectively. The largest proportion of dialysisrelated expenses is represented by costs of dialyzers, recombinant human erythropoietin and other drugs. In Turkey, the annual costs for dialysis (both hemodialysis and continuous ambulatory peritoneal dialysis) are lower than those in the West; however, they are still very high relative to Turkey’s economic means. In order to lower the cost of dialysis, drastic measures should be considered. Ideas for this include re-use of dialyzer membranes and strict control of the use of recombinant human erythropoietin and other drugs. Also, further efforts to expand transplantation, particularly cadaver donation, are needed.
