    Comparison Of Efficacy Of Oral Versus Intra-Articular Corticosteroid Application In The Treatment Of Frozen Shoulder: An Experimental Study In Rats

    Objective: The aim of this study was to compare clinical and histopathological effects of oral versus intraarticular corticosteroid application in a rat model of frozen shoulder. Methods: In this study, eighty adult Sprague-Dawley rats were used. The animals were divided into 5 equal groups. The frozen shoulder model was created by immobilizing animals' shoulders with internal fixation with sutures for 8 weeks. At the 8th week, sham(n: 16) and control (n: 16) groups were sacrificed to collect data for healthy and affected shoulders. Also, at the 8th week, 50 mg/kg methylprednisolone was started for the oral treatment group, and a single dose of 0.5mg/kg triamcinolone acetonide was injected for the intraarticular treatment group. The effect of additional steroid treatment was expected for 2 weeks, then all remaining treatment and natural course groups were sacrificed on the 10th week. Results: After sacrification, specimens taken as "en bloc" scapulothoracic disarticulation were randomly divided into two groups for a range of motion measurement and histopathological examination. The control (frozen shoulder model) group's shoulder range of motion in all directions was lower than the sham (healthy) group (P < 0.01). Natural course and intraarticular steroid groups, compared to the frozen shoulder model showed a significant increase in the direction of abduction (P < 0.05). Also, it was found for treatment groups that in all directions the range of motion was not as good as the healthy values (P < 0.01). The intraarticular treatment group showed higher degrees of abduction compared to the natural course and oral steroid treatment groups (P < 0.01). Oral steroid treatment group's range of motion was not significantly better than the disease model and had no superiority to the natural course group (P > 0.05). Histopathologically, no statistically significant difference was observed between the groups for signs of frozen shoulder which was found in the immobilized group (P > 0.05). Histopathologically, immobilization was found to cause thickening of the capsule that cannot be resolved by treatment. (P < 0.05). Conclusion: In frozen shoulder disease, intraarticular steroid injection seems to be superior in increasing the range of motion than oral steroid treatment.

    Intractable Diarrhea from Cytomegalovirus Colitis in a Case with Hereditary Spherocytosis

    Cytomegalovirus (CMV) infection is a common viral infection worldwide, with a frequency as high as 90% in developing countries. Only 10% of primary CMV infection is symptomatic in immunocompetent patients, and it rarely causes specific complications. We report CMV colitis in an immunocompetent adolescent with hereditary spherocytosis and spontaneous rupture of the spleen, who was critically ill with septicaemia and significant non-bloody diarrhoea that responded to specific CMV treatment.

    Magnetic resonance imaging findings of extraskeletal mesenchymal chondrosarcoma of wrist: a case report

    Extraskeletal mesenchymal chondrosarcomas (EMC) are high grade malignant tumors that rarely involves wrist. Published reports about them are mostly based on conventional radiography, tomography and conventional magnetic resonance imaging (MRI) findings. We did not recognize any report of dynamic-contrast enhanced MRI so far. Herein, we present dynamic contrast enhanced and Diffusion Weighted MRI findings of a case with extraskeletal mesenchymal chondrosarcoma of the wrist. In conclusion, EMC might show peripheral enhancement at early arterial phase and restricted diffusion on MRI. The knowledge of the imaging spectrum of the EMC might help us suggesting the diagnosis.

    Expression of maspin in testis tumors with germ cells and its relation with angiogenesis factors

    Background/aim: We aimed to evaluate the importance of maspin expression in testicular tumors with germ cells, its effect on prognosis, and the relation with angiogenesis factors. Materials and methods: The paraffin blocks of the orchiectomy materials of 32 patients who had undergone orchiectomy due to testicular tumors were taken within the scope of the study. The specimens of the cases included in the study group were reexamined under light microscope. Results: While just one maspin-positive sample was found in the seminoma cases, maspin stained positively in 6 of the nonseminoma germ cell tumors (NSGCTs). No statistical difference was found between maspin and tumor stage, size, alpha fetoprotein values, vascular endothelial growth factor, Ki-67, and CD31. A statistically positive correlation was only determined between maspin and p53 (P < 0.001). Conclusion: Maspin protein, whose expression in some tumors is accepted as a poor prognostic factor, is also expressed in testicular tumors with germ cells. However, according to our study, it is difficult to say whether this protein is a favorable or poor prognostic factor in testicular tumors and to understand how the effect mechanism works. The positive correlation between maspin and p53 in the NSGCTs makes us think that maspin might have displayed an effect on the p53 pathway.

    Clinical and histopathological characteristics and differential diagnosis of chordomas

    Purpose: Chordomas are rare malignant bone tumours occuring in the midline of axial skeleton. We aimed to assess these tumors which has been diagnosed in our center in terms of clinical and pathologic characteristics, and differantial diagnosis difficulties. Material and Methods: We retrospectively evaluated cinical and histopathologic characteristics of 10 chordoma patients diagnosed at pathology department of Baskent University Adana Teaching and Resarch Hospital between 2009 and 2014. Results: The patients were consisted of men (n= 9) and women (n= 1) with the age of 61.3 (38-84) years old. The localization of chordomas were the sacrococcygeal region at eight patients (80%), lumbar vertebrae at one patient ( 10%) and clivus at one patient (10%). The mean diameter was 4.68 (2.5-7.5) cm. Histopathologically, majority of cases were "conventional chordoma" while one of them was "chondroid chordoma". Surgical excision was performed for all patients, only one case has recieved radiotherapy, and chemotherapy was given after surgery for one patient. Local recurrence was detected in three patients and distant metastasis (lung) was found in a patient and he died in a short time. Conclusion: Because that chordomas are rare clinical bone tumours which can be treated efficiently by appropriate surgical excision, clinical and histopathological characteristics and differential diagnosis should be well known.

    A Rare Diagnosis in the Neck During Childhood: Congenital Chondrolipoma

    Chondrolipomas are mesenchymal tumors that found as mature cartilage tissue in a fat tissue. A 2-year-old boy was seen with a complaint of a mass of the neck. On physical examination of the child, there was a one-centimeter mass above the sternocleidomastoid muscle on the lateral neck. Pathological examination of this mass after excision was reported as chondrolipoma. Chondrolipomas are seen mostly in the breast and in the adult. They are rare tumors of the head and neck area and seen mostly the in oral cavity here. This is the first case of chondrolipoma with this age and localization combination in the literature. Chondrolipoma should therefore also be considered in the differential diagnosis of congenital masses located at lateral cervical area.
