Uysal; Mehmet HaberalIn this case report, we present the management of a dramatic and rare combination of injuries: high-voltage electrical injury, chemical burn, and open tibial and calcaneal fractures resulting from a fall from height. The patient underwent early fasciotomies. After treatments administered by a multidisciplinary team consisting of plastic surgeons, general surgeons, orthopedic specialists, anesthesiologists, and physiotherapists, the patient was discharged without extremity loss after a 135-day hospital stay. This case report discusses the importance of early fasciotomy and debridement in management of extensive burns and fall-related injuries, the potential for extended hospitalization for reconstruction, and the necessity of a multidisciplinary approach.Item Akademik Başarıda Artış Skolyoz Varlığını Etkiler Mi? Üniversite Öğrencileri Üzerinde Kesitsel Bir Araştırma(Başkent Üniversitesi, 2024-12-30) Tezeren,Halime Ceren; Özünlü Pekyavaş,NihanAmaç: Kötü postüral alışkanlıklar ve modern yaşamın getirdiği fiziksel inaktivite nedeniyle fonksiyonel aktivitelerde asimetrik duruş artan bir problemdir. Bu çalışmada çalışmamızın amacı Fizyoterapi ve Rehabilitasyon bölümünde okuyan üniversite öğrencilerinde akademik başarıda artışın skolyoz varlığına etkisini araştırmaktır. Yöntem: Çalışmaya Fizyoterapi ve Rehabilitasyon bölümünde okuyan 56 üniversite öğrencisi (7 erkek, 49 kadın: yaş ortalaması 21.8±1.5yıl) dahil edildi. Katılımcılar Grup 1 (not ortalaması 3.00 ve üzerinde olanlar) ve Grup 2 (not ortalaması 2.99 ve altında olanlar) şeklinde gruplandı. Katılımcıların demografik bilgileri, okul ortalaması, ders çalışma postürleri ve bu postürde ne kadar süre kaldıkları ile ilgili sorular soruldu. Ayrıca katılımcılara posterior yönden postürografik analiz ve gibozite testi (forward bend test) yapıldı. Değerlendirmeler tek bir fizyoterapist tarafından yapıldı. Bulgular: Bireylerden toplamda 26 kişide skolyoz varlığı tanımlandı (grup 1; 16/28) (grup 2; 10/28). Gibozite değerlendirmesi sonucunda toplamda 27 kişide gibozite saptandı (grup 1; 15/28) (grup 2; 12/28). Gruplar arasında skolyoz varlığı (p=0,079) ve gibozite (p=0,183) açısından istatistiksel olarak herhangi bir anlamlı fark bulunmadı. Ayrıca değerlendirme parametreleri arasında istatistiksel açıdan anlamlı ilişki bulunmadı (p>0.05, r<0.500). Sonuç: Çalışmamız sonucunda not ortalaması yüksek olan öğrencilerde skolyoz varlığı olmayabileceği ve not ortalaması ile skolyoz arasında ilişki bulunmayabileceği görüldü. Bunun nedeni olarak Fizyoterapi ve Rehabilitasyon bölümü öğrencilerinin pratik ders yoğunluğunun fazla olması, masa başında geçirilen zamanın yapılan pratik derslerle ve egzersiz eğitimi ile azalıyor olması ve öğrencilerin bu konuda bilinçlenmesi gösterilebilir.Item Anne ve Bebek Sağlığında İntestinal Mikrobiyota ve Probiyotikler(Başkent Üniversitesi, 2021-12-30) Tunçer , Nihal; Öngün Yılmaz,Handeİntestinal mikrobiyota üzerine yapılan çalışmalar son yıllarda giderek artmaktadır. İntestinal mikrobiyota özellikle erken neonatal yaşamda metabolik ve immün fonksiyonları etkileyen bir dizi fizyolojik etkiye sahiptir. Literatürde yeni doğan mikrobiyotasının anne karnında oluşmaya başladığını gösteren çalışmalar olsa da; kanıtlar daha çok, doğum sırasında anneden bebeğe geçen mikroorganizmalar ve bebeğin beslenme şekli ile bebek mikrobiyotasının şekillendiği yönündedir. Perinatal ve erken postnatal dönemde probiyotik takviye kullanılarak bebek intestinal mikrobiyota kompozisyonunun yeniden şekillenmesi hastalık riskini azaltmak için olası bir diyet stratejisi olarak önerilebilir. Gebelik sırasında kullanılacak probiyotik takviyenin; preterm doğum riskini azaltabileceği, immünolojik yanıtı iyileştirebileceği, gestasyonel diyabet riskini azaltabileceği ve yeni doğanın sağlığı üzerinde olumlu etkileri olabileceğini gösteren pek çok çalışma bulunmaktadır. Toplumlarda, gıda ve beslenme okuryazarlığının öneminin anlaşılması ve yaygın hale gelmesi sağlık, iyilik hali, sağlıklı beslenme ve gıda maddelerinin seçiminin iyileştirilmesi ve sürdürülebilirliğine yardımcı olabilecektir. Bilgi, yetenek ve tutuma odaklanan, dogmalara dayanan ve etkili eğitim metotlarının kullanıldığı beslenme okuryazarlığı programlarının planlanması ve uygulanmasında bütün alanlardan uzmanların iş birliği yapmalarının faydalı olacağı düşünülmektedir. Bu derleme yazıda, beslenme okuryazarlığı tanımı, hedefi, gerekli yetenek ve okuryazarlığı etkileyen faktörlerin incelenmesi sağlanarak literatüre katkıda bulunması amaçlanmıştır.Item Beyin Bağırsak Aksının İştah Mekanizmasını Düzenleyen Hormonlar ve Nörotransmitterler Aracılığıyla Hedonik Açlık Üzerinde Etkisi(Başkent Üniversitesi, 2023-08-30) Cankar,Bilge; Beyhan,YaseminBeyin-bağırsak aksı; santral sinir sistemi (SSS), enterik sinir sistemi (ENS) ve bağırsak mikrobiyotası arasındaki çift yönlü iletişimi ifade eder. Bu iletişim nöro-endokrin faktörler (adrenal medulla ve korteksten üretilen), SSS, sempatik, parasempatik ve ESS’i kapsayan karmaşık bir ağ tarafından sağlanır. Beyinbağırsak aksı ardındaki mekanizmalar hâlâ belirsizdir fakat beyin-bağırsak aksı arasındaki çift yönlü iletişim metabolizma homoeostazı, enerji dengesi, iştah regülasyonunun yanı sıra motivasyonel süreçler ve gıda ödül sinyalinin devreye girdiği hedonik beslenme davranışını da düzenlemektedir. Herhangi bir enerji ihtiyacı olmaksızın haz amacıyla sadece lezzetli besinlerin tüketiminin söz konusu olduğu beslenme davranışı ‘Hedonik Güdümlü Yeme Davranışı’ olarak tanımlanmaktadır. Hedonik beslenme davranışını etkileyen birçok faktör bulunmakla birlikte beyin-bağırsak aksının herhangi bir evresinde oluşabilecek düzensizlikler, normal koşullar altında gıda alımını düzenleyen faktörleri (mezolimbik dopaminerjik yolak, gastrointestinal ve adipoz doku hormonları, endokannabioid sistem) olumsuz etkileyerek hedonik güdümlü yeme davranışının artışına böylelikle aşırı yeme durumuna sebep olabilir. In this review article, we aim to synthesize and analyze the existing literature on the role of prolactin in burn injuries, focusing on its impact on inflammation, immune response, tissue repair, and potential neuroprotective effects. Bioelectrical impedance analysis is an advanced technique that helps to assess the detailed information on body composition, including basal metabolic rate, fat percentage, muscle mass, total body protein, bone health, segmental distribution of muscles and fat, and extra- and intracellular water. Patients with burns and high percentage of total burn surface area are known to be in a hypermetabolic state; a prolonged period of hypermetabolism with insufficient nutrient supply can lead to catabolism and cachexia. Finding the patient's nutritional status at the beginning of hospitalization helps to prevent further complications and to support faster recovery. Here, we present a nutritional risk analysis by using bioelectrical impedance in a patient with catabolic symptoms and major burns for management of nutrition with successful outcome.Item Burn Cachexia: Diagnosis and Nutrition Support(Başkent Üniversitesi, 2023-03) Pratibha Sharma; Rajpal Singh; Shree Prakash Jaiswal; Sunil ChandiwalABSTRACT Abstract Burn cachexia is a condition in which patients with major burns, especially patients with >20% total burn surface, present with significant rapid weight loss, muscle loss, and fat loss. Burn cachexia is also known as wasting syndrome and is characterized by severe weight loss, muscle and fat loss, fatigue, and anorexia. The current definition of burn cachexia includes association of an underlying disease condition characterized with muscle loss and with or without reduction in fat mass. Cachexia is driven by the molecular pathway known as the ubiquitin-proteolytic pathway. Cytokine activity plays a crucial role in the pathogenesis of cachexia; the consequent inflammation is the main cause of cachexia in major burns, and inflammatory cytokines are upregulated by reactive oxygen species that are triggered by nuclear transcription factor NF-κB. Some other well-known pathways are mitochondrial dysfunction and autophagy, endoplasmic reticulum stress, and insulin resistance. Burn cachexia is inflammation that induces protein degradation and increases cell death. Identification of burn-induced cachexia is a difficult challenge and, if untreated, can lead to death. Nutrition support is an extremely important part of burn management with cachexia and requires special attention. In this case report, we share the practical aspects of burn cachexia diagnosis and application of nutrition to prevent mortality. Our case involved a young male patient with electric burn injury who was diagnosed with burn-induced cachexia who had a successful outcome.Item Burn Injury Following Noninvasive Radiofrequency Lipolysis: A Rare Complication Highlighting Regulatory and Medico-legal Issues(Başkent Üniversitesi, 2024-06) Mohammad R. Goodarzi; James E. T. WokesABSTRACT OBJECTIVES: The escalating demand for nonsurgical body contouring and skin rejuvenation has led to a surge in the use of radiofrequency techniques. Despite their popularity, scientific investigations of devices and parameters remain inadequate, resulting in a lack of evidence-based clinical trials. In this case report, we have highlighted the potential hazards of noninvasive radiofrequency fat reduction by presenting a severe burn case. Our objective was to underscore the importance of scientific rigor in establishing treatment parameters and the need for regulatory oversight in the aesthetic industry. MATERIALS & METHODS: A 47-year-old female underwent radiofrequency therapy at an unregulated aesthetic clinic, resulting in a full-thickness deep tissue burn. We have outlined the patient’s history, treatment details, and subsequent care, emphasizing the absence of medical supervision during the procedure. RESULTS: The patient developed a 6 cm × 6-cm full-thickness burn, necessitating operative management. Despite initial nonoperative measures, the burn progressed, leading to a permanent scar and contour irregularity. CONCLUSIONS: This case highlights the urgent need for improved regulation, training, and safety protocols in the aesthetic industry, especially concerning radiofrequency devices. The Montgomery principle emphasizes the necessity of informed consent, and the complexities of medical device regulations and indemnity considerations warrant increased regulatory oversight. Collaborations among legal experts, regulatory bodies, medical professionals, and nonmedical practitioners are essential to ensure patient safety in an evolving aesthetic landscape.Item Burn Prevention Strategies at Baskent University Facilities(Başkent Üniversitesi, 2021-06) Mehmet HaberalItem CASE REPORT Role of Burn Teams in Management of Inflammatory Skin Reactions: Report of 2 Cases(Başkent Üniversitesi, 2022-06) A. Toxic epidermal necrolysis is an acute skin inflammatory reaction. It is generally triggered by upper respiratory tract infections or by medications such as nonsteroid anti-inflammatory drugs, antibiotics, and anticonvulsants. It is associated with a significant mortality of 30% to 50% and long-term sequelae. Here, we present 2 cases of toxic epidermal necrolysis who were treated by a multidisciplinary team involving burn professionals.Item Chemical Contact Burn With Ready-Mixed Concrete: A Case Report(Başkent Üniversitesi, 2021-06) Emin Turk; Erdal Karagulle; Halil İbrahim Taşcı; Mehmet HaberalChemical burns, and particularly alkali burns, constitute a very small fraction of burn unit admissions. It is generally unknown that ready-mixed concrete can cause chemical burns. However, prolonged contact with ready-mixed concrete may cause serious burns. Raising awareness of the need for taking necessary measures against burns from ready-mixed concrete among workers would be effective for the prevention of this type of burn. Kute,; Sanshriti Chauhan; Ruchir DaveThe COVID-19 pandemic has grossly affected the transplant communities with no exception to any geographic area. Burn injury remains an important public health problem, even during the pandemic. The management of concomitant burn in an organ transplant recipient has been rarely described. Here, we report a kidney transplant recipient who was admitted for moderate COVID-19 symptoms and then sustained a second-degree inhalational burn injury at home on day 2 after discharge from COVID-19. The patient was shown to be COVID-19 positive and was isolated initially in the burn unit. After a second negative report on day 3 of hospital admission, the patient was shifted to the general ward. He required a hemodialysis session and modification of immunosuppression, which was done through telemedicine from our center. The wound was complicated by a rejection episode, which was treated by regrafting. He recovered on day 45 after injury with baseline immunosuppression restored. His serum creatinine reached a nadir of 3.2 mg/dL, and he has continued on regular follow-up. In summary, our case highlights the difficulty in managing burn cases during the pandemic. The added problem of balancing immunosuppression in organ transplant recipients with burn injuries is a daunting task for transplant physicians.Item Covid-19 Pandemisinde Diyabet Yönetimi ve Diyabet Hemşirelerinin Sorumlulukları(Başkent Üniversitesi, 2021-12-30) Polat , Gökşen; Ünsal Avdal,ElifCOVID-19 enfeksiyonu geçiren diyabetli bireyler sağlık açısından oldukça risk altındadır. Mevcut diyabet covid-19 enfeksiyonun prognozunu kötüleştirmektedir. Pandemi döneminde de hastaların rutin kontrolleri ve takipleri aksatılmamalıdır. Hastalar hemşireler tarafından tedavi planlarına uygun bakımı almaya devam etmelidir. Ancak hastaların hastaneye gitmeleri pandemi döneminde oldukça riskli bir durumdur. Hemşirelere bu dönemde oldukça büyük sorumluluklar düşmektedir. Pandemi döneminde Covid-19’lu diyabetik bireyin öz yönetimini sağlamalarına yardımcı olmalı, hastalık döneminde primer takipçileri olmalı ve hastalara gerekli yönlendirmeler yapılmalıdır. Bu derleme makalesinin amacı COVID-19 pandemisinde diyabet yönetimine ilişkin bilgiler vermek ve diyabet hemşirelerine yol gösterici bir kaynak oluşturmaktır.Item De Novo Inflammatory Bowel Disease After Pediatric Orthotopic Liver Transplant: A Case Report(Başkent Üniversitesi, 2009-09) Dehghani, Seyed Mohsen; Malek-Hosseini, Seyed Ali; Geramizadeh, Bita; Kakaei, Farzad; Bahador, Ali; Eshraghian, AhadObjectives: The improvement of pre-existing inflammatory bowel disease after orthotopic liver transplant might be anticipated. However, both the exacerbation of inflammatory bowel disease and de novo inflammatory bowel disease after orthotopic liver transplant (despite sufficient allograft immunosuppressive therapy) have been described. Materials and Methods: We present a case of ulcerative colitis in a pediatric liver transplant recipient. Results: A 13-year-old boy with cryptogenic liver cirrhosis received an orthotopic liver transplant from a deceased donor. Five months later, he presented with watery diarrhea and abdominal distention. He was treated with the immunosuppressive agents tacrolimus (0.15 mg/kg/d) and mycophenolate mofetil (20 mg/kg/d). A general physical examination revealed a boy with stable vital signs and without fever. The only positive finding was enlargement of the abdomen without tenderness. Many pus cells and a few red blood cells were detected in the patient’s stool, but the results of a stool culture for bacteria were negative. Because of his chronic diarrhea, this patient underwent colonoscopy, which revealed diffuse erythematous mucosa, multiple ulcers, exudate, and pseudo¬polyps with a diffuse loss of vascularity. Those findings are indicators of colitis. The results of histopathologic examination of the colonic mucosa suggested ulcerative colitis. The patient was treated with mesalamine and prednisolone, and a repeat colonoscopy revealed an improvement in his bowel disease. Conclusions: De novo inflammatory bowel disease should be considered in patients in whom chronic diarrhea develops after an orthotopic liver transplant. We suggest that colonoscopy and biopsy should always be performed if other causes of diarrhea have been excluded.Item Delayed Perforator Flaps for Reconstruction of Extensive Defects of the Posterior Trunk(Başkent Üniversitesi, 2024-03) Burak Ozkan; Abbas Albayati; Suleyman Savran; Cagri A. UysalABSTRACT OBJECTIVES: The delay phenomenon involves neovascularization of tissue after ischemic preconditioning. When employed, the delay phenomenon promotes flap survival and increases length-to-width ratio in conventional flaps. Although well-planned perforator flaps cover defects without tension, the success rate is unpredictable in extensive defects that are closed because of the unknown vascularity of perforasomes. We aimed to increase perforator flap size by utilizing the delay phenomenon for broad defects of posterior trunks. MATERIALS AND METHODS: Between 2019 and 2020, 10 patients (6 female, 4 male) underwent posterior trunk reconstruction with delayed perforator flaps. We retrospectively analyzed etiology, defect size, flap size and type, postoperative complications, and long-term results. The etiology of defects was meningomyelocele, soft tissue sarcoma, and pressure ulcers in 3, 3, and 4 patients, respectively. Defects were covered with delayed lumbar, intercostal, and thoracodorsal perforator flaps. Flaps were planned unilateral or bilaterally. Perforators were located preoperatively with a handheld Doppler. In the first session, delaying incisions and pedicle dissection were performed. The delayed incision was primarily sutured. Perforator flaps were elevated 1 week later, and defects were closed. RESULTS: Mean defect size was 375 cm2, and mean flap size was 420 cm2 (202-625 cm2). The donor site was closed primarily for 7 patients. Two patients required split-thickness skin grafts. One patient had donor site closed with secondary healing. No flap losses among patients were shown. Hematoma was seen in 2 patients in the immediate postoperative period. After 2 weeks, seroma and donor site dehiscence were shown in 2 and 1 patient, respectively. CONCLUSIONS: The delay phenomenon can be utilized in perforator flaps as in random pattern local flaps. Perforasome areas can be extended with delayed incision. In addition, broad defects of the posterior trunk can be safely covered with delayed perforator flaps.
