Item Vascular Variations and Anastomosis Techniques in Renal Transplant Donors(2015) Haberal, Mehmet; Arer, Ilker Murat; Yabanoglu, Hakan; Caliskan, Kenan; Parlakgumus, Alper; Yildirim, Sedat; Moray, GokhanPurpose: We aimed to share our experience about vascular variations and anastomosis tecniques in renal transplant donors. Material and Methods: 128 donor nephrectomy performed in our hospital between February 2010 and June 2014 were included in our study. Donors were retrospectively analyzed according to age, sex, comorbidity, operation history, site of nephrectomy, vascular variation and anastomosis techniques. Results: 21 (% 16,4) patients have left-sided, 19 (% 14,8) have right-sided and 13 (% 10,2) have bilateral vascular variation. 29 (% 21,2) unilateral double renal artery, 8 (% 6,3) unilateral double renal artery and vein, 4 (% 3,2) unilateral triple renal artery, 3 (% 2,4) early branching renal artery, 2 (% 1,6) unilateral double renal vein and 2 (% 1,6) polar artery are vascular variations observed. of 92 (% 71,9) recepients anastomosis type was end to side between renal artery and external iliac artery and end to side between renal vein and external iliac vein, 32 (% 25) end to end between renal artery and external iliac artery and end to side between renal vein and external iliac vein, 4 (% 3,1) end to side between renal artery and external iliac artery and end to side between renal vein and external iliac vein and end to side between 2nd renal artery and external iliac artery. Conclusion: Preoperative evaluation of renal vasculature of transplant donors is an important issue in means of decreasing peroperative vascular complications and decision for nephrectomy site.Item Organ Sparing Surgery for a Giant Liposarcoma(2015) Aytac, Huseyin Ozgur; Torer, Nurkan; Ozen, Merve; Arer, Ilker MuratLiposarcomas those are malignant soft tissue tumors often occur in large sizes in the retroperitoneum and abdomen due to their silent clinic. Excision with negative margins is the gold standard of treatment. A case operated on for a giant intraabdominal liposarcoma is being reported. A giant soft tissue tumor filling the whole abdomen was determined in the computed tomography scan. Core biopsy was obtained and demonstrated a well-differentiated liposarcoma. A 15 kg of mass 44x30x14 cm in size was excised en-bloc. Pathological examination of this tumor showed a well-differentiated liposarcoma with mixoid parts. No recurrence was observed in two years of follow-up despite any adjuvant therapy. This is to be one of the largest retroperitoneal sarcomas in the literature.Item Laparoscopic Repair of Morgagni Hernia(2015) Arer, Ilker Murat; Yabanoglu, Hakan; Aytac, Huseyin Ozgur; Caliskan, Kenan; Torer, NurkanMorgagni hernia is a congenital herniation of abdominal contents into the thoracic cavity through a retrosternal diaphragmatic defect and make up about 1 % - 5 % of all types of congenital diaphragmatic hernias. Surgical repair of Morgagni hernias is usually indicated when patients are symptomatic and have a high risk of strangulation or incarceration of the contained viscera. 71-year-old male patient admitted to emergency department with a 2-day history of abdominal pain, vomiting and obstipation. Laparoscopic repair for Morgagni hernia was performed. Laparoscopic repair for Morgagni hernia with mesh repair is secure, satisfactory and easily performed.Item Acute appendicitis during pregnancy: case series of 20 pregnant women(2016) Arer, Ilker Murat; Alemdaroglu, Songul; Yesilagac, Hasan; Yabanoglu, Hakan; 0000-0002-1161-3369; 0000-0003-4335-6659; 0000-0002-1365-9256; 28074461; AAJ-7865-2021; AAY-2668-2021; AAI-8400-2021; AAJ-6068-2021BACKGROUND: Acute appendicitis (AA) is the most common cause of acute abdomen during pregnancy. Most of the signs of appendicitis are also found during normal pregnancy period, however, and diagnosis of appendicitis during pregnancy remains challenging. The aim of the current study was to report our clinical experience of AA during pregnancy and investigate optimal management of this difficult situation. METHODS: Records of 20 pregnant women with diagnosis of AA who underwent appendectomy between 2005 and 2015 were included in this study. Data were collected retrospectively. Patients were evaluated according to age, signs and symptoms, gestational age, physical findings, serum white blood cell count, ultrasound (US) findings, pathology reports, surgical technique, operation time, and complications. RESULTS: Of 20 patients, 16 (80%) underwent open appendectomy and 4 (20%) underwent laparoscopic appendectomy. Mean age of patients was 29.6 +/- 5.6 years. Most common symptom was abdominal pain (95%). Six (30%) patients were in first trimester, 9 (45%) patients were in second trimester and 5 (25%) patients in were in third trimester. US findings consistent with AA were found in 12 (60%) patients. Negative appendectomy rate was 30%. Maternal complication was seen in only 1 (5%) patient. No fetal complication was observed. CONCLUSION: Accurate diagnosis and prompt surgical treatment of AA in pregnant women should be performed due to high rates of maternal and fetal complications.Item Can red cell distribution width be used as a predictor of acute cholecystitis?(2017) Arer, Ilker Murat; Yabanoglu, Hakan; Caliskan, Kenan; 0000-0003-0268-8999; 0000-0002-1161-3369; 0000-0002-8767-5021; 28740954; AAJ-7865-2021; AAJ-7201-2021; AAK-2011-2021Objective: Acute cholecystitis is a common disease requiring accurate markers for diagnosis and proper treatment. The aim of this study was to investigate the role of red cell distribution width (RDW) in acute cholecystitis. Material and Methods: 299 were included in the study. The subjects were divided into 2 groups; group 1 (n: 46) acute cholecystitis group and group 2 (n:253) chronic cholecystitis group. The patients were compared with respect to demographic characteristics, white blood cell count, C-reactive protein, and red cell distribution width. Results: A statistically significant difference was observed between groups with respect to gender, white blood cell count, C-reactive protein, and red cell distribution width level (p<0.05). The mean red cell distribution width level of group 1 and 2 was 14.19 +/- 2.02% and 15.03 +/- 2.51%, respectively. Conclusion: Red cell distribution width level can be used as a predictor of acute cholecystitis. Multicenter prospective studies should be performed to elucidate the exact role of RDW level in acute cholecystitis.Item Retrospective Analysis of Patients with Synchronous Primary Breast and Thyroid Carcinoma(2018) Arer, Ilker Murat; Yabanoglu, Hakan; Kus, Murat; Aldur, Aydincan; Avci, Tevfik; 0000-0002-8726-3369; 0000-0002-1161-3369; 0000-0001-5225-959X; 0000-0001-6529-7579; 29774315; AAA-3068-2021; AAJ-7865-2021; AAF-1698-2021; AAJ-7870-2021Objective: Breast and thyroid cancers are commonly encountered malignancies. Increased risk of breast cancer in follow-up period of thyroid cancer or vice versa has been reported. However, they have some associations, synchronous presentation of these tumors is rare. We presented 12 patients diagnosed as breast and thyroid cancer and treated at the same time. Materials and Methods: Mastectomy and thyroidectomy were performed in 19 patients at the same time. 7 patients were excluded because of benign thyroid pathology. Therefore 12 patients who had diagnosis of synchronous breast and thyroid cancer were included. Data regarding clinical, pathological, treatment and prognostic factors was retrospectively analyzed. Results: Total thyroidectomy was performed in all patients. The mean age of patients was 54 years (min. 44-max. 70). Only one patient was male. Thyroid pathology was detected preoperatively by FDG PET-CT scan in 11 patients. Breast reconstruction was performed in three patients. The most commonly seen thyroid malignancy was papillary thyroid carcinoma. Postoperative complication rate was 33.3%. Adjuvant chemotherapy was given in 11 patients whereas one patient received adjuvant radiotherapy. Conclusion: Although synchronous presentation of breast and thyroid cancer is rare, surgical treatment of both of these tumors can be safely performed at the same time. Association of these tumors should be evaluated by large scaled studies.
