Item Evaluation of the Genetic Analysis Results in Infertile Patients with Non-Obstructive Azoospermia(2023) Sen, Erhan; Kizilkan, Yalcin; Duran, Mesut Berkan; Turunc, Tahsin; Sahin, Feride Iffet; Ozkardes, Hakan; 0000-0001-7308-9673; 0000-0002-7277-449X; AAC-7232-2020; AAH-1052-2020Objective: To evaluate the genetic analysis results of patients who referred to our clinic infertility and whom semen analysis revealed non -obstructive azoospermia (NOA).Materials and Methods: Among 994 patients who underwent a microscopic testicular sperm extraction (micro-TESE) operation for NOA, 497 patients who were tested for karyotype analysis and 450 patients who were tested for chromosome Y microdeletion were included in our study. The rates of Klinefelter syndrome (KS) and Y chromosome microdeletion, sperm retrieval rates (SRR) in these genetic anomalies and the factors affecting them were investigated. Additionally, the association between the age, duration of infertility, testicular size, serum follicle stimulant hormone (FSH) and testosterone levels of patients and sperm extraction rates of micro-TESE operations were also evaluated.Results: The overall SRR of NOA patients who underwent micro-TESE was 47.5%. Among 104 patients with KS, sperm was successfully found after micro-TESE in 22 (21.2%). Fourteen patients were diagnosed with the Y chromosome microdeletion and sperm was successfully found in 4 (28.6%) of them; while the duration of infertility did not affect the SRR after micro-TESE (p=0.712); age, testicular volume serum FSH and testosterone levels had a significant effect on the SRR (p<0.005).Conclusion: In this study, the SRR of patients who have chromosome Y microdeletion or KS, was found to be lower than other studies in the literature. This difference could be derived from the genetically tested population's structure, variance in the gene areas used for scanning and different demographic characteristics of different regions.Item Prostate Biopsy in the Elderly: Histologic Findings and Treatment Necessity(2014) Akman, Ramazan Yavuz; Koseoglu, Hikmet; Oguzulgen, Ahmet Ibrahim; Sen, Erhan; Yaycioglu, Ozgur; 25374232The aim of this study is to determine results of high prostate specific antigen (PSA) or abnormal digital rectal examination driven prostate biopsies performed in our Department in men aged 75 or more and to show the characteristics of pathology results. The hospital records of the patients who had high PSA or abnormal digital rectal examination driven prostate biopsy in two common university based research hospitals have been reviewed retrospectively. Patients aged 75 years or older at the date of biopsy whose records provided pathology results and full medical history were evaluated for the study. A total of 103 patients were evaluated with a mean age of 79.4 +/- 3.4 years. More than half of the patients (55.1%) were in their seventh decade and the rest were in the eighth decade. Median PSA value was 15.0 (range 2.1-4500) ng/ml. In most of the biopsies (67%), PSA levels were lower than 20 ng/ml. In almost half of the patients (48%), digital rectal examination was abnormal. In 68.9% of the patients, there were at least one or more associated co-morbid diseases. Gleason scores were 7 or higher in 73%, and 8 or higher in 37% of the patients with prostate cancer. Four of the 70 (6%) patients had bone metastases. Castrations were applied to most of the patients with prostate adenocarcinoma (% 79). High percentage of high grade (Gleason 7 or more) prostate adenocarcinoma in the elderly refutes the perception of prostate cancer in this age group as clinically insignificant. Therefore, it is to be kept in mind that prostate cancer in the elderly an be clinically significant and prostate biopsies are to be performed when necessary.Item Prostatic Melanosis: A Case Report(2014) Cicek, Tufan; Erinanc, Hilal; Gonulalan, Umut; Sen, Erhan; Ozturk, Bulent; 0000-0003-1944-1722; 0000-0003-1401-6356; AAJ-8576-2021; AAL-1268-2021Prostatic melanosis is characterized by melanocytic proliferation in prostatic stroma and the epithelium. The pathogenesis of the lesion is uncertain. In literature it has been reported as a case associated with prostatic adenocarcinoma before. Primary and metastatic malignant melanoma should be considered in differantial diagnosis of the lesion.Item Transpaeritoneal Laparoscopic Simple Nephrectomy: A Single Center Experience(2014) Cicek, Tufan; Sen, Erhan; Gonen, Murat; Gonulalan, Umut; Kosan, Murat; Ozturk, Bulent; 0000-0003-1944-1722; AAJ-8576-2021; AAA-7140-2021Purpose: In this study, we planned to evaluate the efficacy and perioperative outcomes of transperitoneal laparoscopic simple nephrectomy at Baskent University Konya Application and Research Center. Material and Methods: The demographic and perioperative data of patients who underwent transperitoneal laparoscopic simple nephrectomy at Baskent University Konya Application and Research Center Urology clinic between February 2005 and November 2013 were analyzed retrospectively. Operative time, perioperative laboratory results, the durations of hozpitalization and follow-up, perioperative complications and demographic characteristics of each patient were recorded. Results: Overall, 58 patients were included. 27 patients were male (46%) and 31 patients were female (54%). The mean age was 45.3 +/- 17.12 (6-79). The mean operative time was 104.4 +/- 36.5 (40-185) minutes. The mean hospitalization duration was 2.7 +/- 0.7 (2-5) days. The mean preoperative and postoperative hemoglobin levels were 13.7 +/- 1.74 and 12.4 +/- 1.73 mg/dl, respectively. Postoperative hemoglobin levels were statistically significatly lower than preoperative hemoglobin levels (p<0.001). 4 patients (%6.9) needed blood transfusion. 5 patients had grade I, 4 patients had grade II and 2 patients had grade III complications according to modified Clavien classification system. Conclusion: Transperitoneal laparoscopic simple nephrectomy seems to be a safe and efficacious treatment option as having low complication rates.
