Item The Effect of Serum 25(OH)D3 Level on Breast Cancer-Related Lymphedema(2019) Analan, Pinar Doruk; Kaya, Emine; 31140909Background: The aim of this study was to evaluate the effect of serum 25-hydroxyvitamin D3 [25(OH)D3] levels on the presence and severity of lymphedema, and on the levels of pain, disability, and function in patients with breast cancer-related lymphedema (BCRL). Methods and Results: This controlled study included 71 patients diagnosed with breast carcinoma. Participants were divided into two groups. The BCRL group included 37 breast cancer patients with lymphedema and the control group included 34 breast carcinoma patients without lymphedema. Demographic information, dominant extremity, affected breast, duration of malignancy, and serum 25(OH)D3 levels were recorded for all patients. The 25(OH)D3 levels were then compared between groups. The correlations between serum 25(OH)D3 levels and the visual analog scale (VAS) and Quick Disabilities of the Arm, Shoulder, and Hand Questionnaire (Q-DASH) scales and the volumetric and diametric differences between the upper extremities were analyzed in the BCRL group. Serum 25(OH)D3 levels did not show statistically significant differences between groups (p > 0.05). There was no correlation in the BCRL group between 25(OH)D3 levels and the VAS and Q-DASH scores or the diametric and volumetric differences of extremities (r <= 0.3; p > 0.05). Conclusions: Serum 25(OH)D3 levels do not appear to affect the presence or severity of lymphedema, pain, disability, or physical function in BCRL patients. In routine clinical practice, evaluation of this vitamin level does not appear to be necessary for lymphedema in BCRL patients.Item Is There a Relationship Between Insulin Resistance and Breast Cancer-Related Lymphedema? A Preliminary Study(2021) Doruk Analan, Pinar; Kaya, Emine; 33761281Background: High blood insulin levels, insulin resistance (IR), and obesity are components of metabolic syndrome (MetS). The literature has indicated a high risk of breast cancer in patients with MetS. However, no studies have been conducted evaluating the relationship between breast cancer-related lymphedema (BCRL), one of the most frequently encountered postbreast cancer treatment conditions, and IR. Therefore, the aim of this study was to evaluate whether there is a relationship between BCRL and IR. Methods and Results: A total of 28 patients diagnosed with breast carcinoma were included in this preliminary study. Patients were divided into BCRL (n = 15; mean age: 55.2 +/- 11.2 years) and non-BCRL (control) groups (n = 13; mean age: 55.17 +/- 6.57 years). Body mass index (BMI), waist and hip circumference, and fasting blood glucose and blood insulin levels of all patients were recorded. The Homeostasis Model Assessment (HOMA) test was used for the calculation of IR measurement with a value of 2.5 taken as an indicator of IR. Parameters were compared between groups. BMI, waist circumference measurements, blood insulin, and HOMA-IR levels were statistically significantly higher in the BCRL group than the control group (p < 0.05). HOMA-IR values >2.5 were found in 14 patients in the BCRL group. In the control group, only three patients had IR based on HOMA-IR criteria (p = 0.000). Hip circumference measurements and fasting blood glucose levels were similar between the groups (p > 0.05). Conclusions: BCRL appears to be associated with waist circumference, fasting blood insulin level, and HOMA-IR levels. In routine clinical practice, evaluation of IR may be important in the follow-up of this patient population.Item Meme kanseri ilişkili lenfödem hastalarında “Quality of life measure for limb lymphoedema-arm” anketinin türkçe geçerlilik ve güvenilirliğinin araştırılması(Başkent Üniversitesi Tıp Fakültesi, 2017) Kaya, Emine; Adam, MehmetMeme kanseri ilişkili lenfödem; lenfatik sistemin kanser hücreleri ve/veya kanser tedavisi tarafından tahrip edilmesiyle proteinden zengin interstisiyel sıvının jeneralize veya bölgesel olarak birikimiyle karakterize kronik bir durumdur. Lenfödem kişiye ve etkilenme boyutuna göre değişen ölçülerde fiziksel, psikolojik ve sosyal sorunlara neden olarak hastanın yaşam kalitesini olumsuz etkilemektedir. Lenfödem hastalarının fonksiyonel düzeylerini ve yaşam kalitelerini değerlendirmek, sorunları saptayıp tedaviyi şekillendirmek amacıyla hastalığa özgü hasta bildirimli anketler kullanılmaktadır. Çalışmada orjinal adı „Quality of life measure for limb lymphoedema-Arm (LYMQOL-Arm)‟ olan anketin Türkçe geçerlilik güvenilirliğinin saptanması amaçlandı. Literatürde belirlenen prosedürlere uygun olarak Türkçe‟ye çevrilip adaptasyonu yapılan LYMQOL-Arm anketi meme kanseri ilişkili lenfödem tanısı olan, Türkçe okuma-yazma bilen 110 hastaya uygulandı. Güvenilirlik analizleri için iç yapı tutarlılığı ve test-tekrar test güvenilirliği ölçüldü. Ölçekteki her madde için ayrı ayrı ve tüm alt bölümler için Cronbach alfa katsayısı ve madde-toplam puan korelasyonları hesaplandı. Test-tekrar test güvenilirliği için, 33 hastaya başlangıçta ve iki hafta sonra olmak üzere ikişer kez ölçek uygulandı. Geçerlilik için „birlikte geçerlilik‟ araştırıldı. Birlikte geçerlilik için daha önce Türkçe geçerliliği belirlenmiş olan Q-DASH (Quick Disabilities of the Arm, Shoulder and Hand) ölçeği ile karşılaştırma yöntemi kullanıldı. Güvenilirlik analizleri için iç yapı tutarlılığında Cronbach alfa katsayısı ölçeğin alt bölümleri olan fonksiyon, dış görünüş, semptom ve emosyonel durum için sırasıyla 0,933- 0,872-0,863-0,905 olarak ve tüm anket için 0,900 olarak hesaplandı (p<0,05).Test-tekrar test analizi sonucunda her iki anket için de maddelerde pozitif yönde güçlü korelasyon saptandı (p<0,001). Yapı geçerliliği için faktör analizinde Kaiser-Meyer-Olkin (KMO) değeri 0,888 saptanarak örneklem sayısının yeterli ve maddelerin birbirleri ile korele olduğunu bulundu. Q-DASH ölçeği ile LYMQOL-Arm ölçeği arasında pozitif bir ilişki saptandı (R=0,906). Sonuç olarak LYMQOL-Arm ölçeği Türkçe versiyonunun lenfödemli hastalar için geçerli ve güvenilir bir yaşam kalitesi ölçeği olduğu ve ülkemizdeki klinik çalışmalar ile rutin klinik uygulamalarda kullanılabileceği belirlenmiştir. Breast cancer-related lymphedema; is a chronic condition characterized by generalized or regional accumulation of protein-rich interstitial fluid by the destruction of the lymphatic system by cancer cells and / or cancer therapy. Lymphedema negatively affects the quality of life of a patient as it causes physical, psychological and social problems in a way that varies depending on the person and the size of the affected area. Disease-specific questionnaires are used to evaluate the functional levels and quality of life of patients with lymphedema and also diagnose problems and determine treatment. The aim of this study was to determine the validity of the Turkish version of the Life Quality Scale for limb Lymphedema, with the original name of "Quality of life measure for limb lymphoedema-arm" (LYMQOL-Arm). After the translation and adaptation to Turkish the LYMQOL-Arm questionnaire by proper procedure is indicated at the literatüre was administered to 110 patients who were literate and diagnosed with breast cancer-related lymphedema. For reliability analysis, internal consistency and test-retest method were used. Cronbach alpha coefficient and item-total statistics were calculated for each question and subscales. The questionnaire was applied twice to 33 patients with the interval of two weeks to quantify test-retest reliability. For validation, the questionnaire was compared to Quick-DASH(Quick-Disabilities of the Arm, Shoulder and Hand) which has already been validated for Turkish. For the reliability analysis, the Cronbach's alpha coefficient subscales of internal consistency were calculated to be 0.933-0.872-0.863-0.905, and 0.900 for the whole questionnaire (p <0.05), with function, appearance, symptoms and mood. As a result of the test-retest a strong correlation was found between the items of both questionnaires (p <0.001). In the factor analysis for construct validity, Kaiser-Meyer-Olkin (KMO) value was found to be 0.888, indicating that the number of samples was sufficient and the items correlated with each other. A positive correlation between Q-DASH scala and the LYMQOL-Arm scale (R=0.906) was determined. As a result, it was determined that Turkish version of LYMQOL-Arm scale is a valid and reliable quality of life scale for lymphedema patients and can be used in clinical studies and in routine clinical practices in Turkey.Item Postural Stability in Patients with Lower Limb Lymphedema(2019) Analan, Pinar Doruk; Kaya, Emine; 31329503Background: Impaired mobility and musculoskeletal dysfunctions are commonly seen in patients with lymphedema and can affect the balance system in patients in whom the lower limb is affected. This study aims at comparing postural stability between patients with lower limb lymphedema (LLL) and healthy subjects. Methods and Results: This controlled study included 36 patients with LLL (LLL group) and 36 healthy subjects (control group). Fall risk, stability index, and Fourier index measurements of Tetrax (R) Interactive Balance System were compared. There were no statistically significant differences between groups in terms of demographics parameters (p > 0.05). Some stability and Fourier index measurements were significantly higher in subjects in the LLL group than in the control group. However, the fall risk was not statistically different in patients with LLL than healthy subjects (p > 0.05). Conclusion: LLL may cause balance impairments. However, this impairment may not lead to increased fall risk. The evaluation of postural stability in LLL patients would be a useful addition to routine clinical practice due to the relationship between LLL and balance impairments.Item Unilateral peroneal paralysis after squat exercise(2016) Analan, Pinar Doruk; Kaya, Emine; 0000-0002-3528-3712; AAA-8043-2021Peroneal nerve entrapment neuropathy is the most common entrapment neuropathy in the lower extremity. Peroneal nerve may be damaged while passing over the upper neck of fibula or the tunnel shaped space on the origin of peroneus longus muscles. As a result of this neuropathy, walking difficulties and drop-foot secondary to muscle weakness may develop in patients. In this article; clinical assessment, diagnosis and treatment planning of a patient diagnosed drop-foot after squatting exercise were discussed.
