METHODS: eighty -eight eyes of 88 patients with primary pterygium were included. Pterygia were graded preoperatively from type 1 to type 3 (type 1 atrophic, type 3 inflamed) according to the inflammatory status. The eyes were preoperatively randomized to receive topical steroid and antibiotic treatment (group 1, 46 eyes) and additional topical bevacizumab (5 mg/mL; group 2, 42 eyes) in the postoperative period. All eyes underwent pterygium excision and LCAT. Medications were tapered and discontinued at one month. Postoperative complications and recurrence rates were recorded. RESULTS: The mean follow -up duration was 29.3 +/- 4.2mo (24-52mo) and 28.5 +/- 3.4 (24-48mo) in group 1 and 2, respectively (P>0.05). There were no statistically significant differences regarding the age or gender between groups (P>0.05). Also, the difference between groups with respect to pterygium type was not significant. During the follow -up period, recurrence developed in 2 eyes (4.3%) in group 1, whereas in one eye (2.4%) in group 2. No statistically significant difference between groups was found in recurrence rates (P>0.05). No re-operation for recurrence was necessary during the follow-up period in both groups. Methods: In this prospective case-control study, choroidal thicknesses of 20 newly diagnosed migraine patients and 20 age-and sex-matched healthy subjects were measured using a high-speed, high-resolution frequency domain (FD) OCT device (lambda = 840 nm, 26.000 A-scans/s, 5 mu m axial resolution). All patients underwent a complete ophthalmic examination before the measurements. OCT measurements were taken at the same time of day (9: 00 AM), in order to minimize the effects of diurnal variation. Results: There was a statistically significant difference in median choroidal thickness between the migraine patients (277.00 [interquartile range (IQR) 85.75] mu m) and controls (301.00 [IQR 90.50] mu m) (p = 0.012). There were significant differences at all measurement points (p<0.05 for all). Conclusions: The decreased choroidal thickness of patients with migraine might be related to the vascular pathology of the disease. METHODS: In this observational, cross-sectional study, choroidal thicknesses of 23 newly severe OSAS patients and 23 body mass index- age- and sex-matched healthy subjects were measured using a high -speed, high resolution frequency domain-OCT device (lambda=840 nm, 26 000 A -scans/s, 5 pm axial resolution). All patients underwent a complete ophthalmic examination before the measurements. OCT measurements were taken at the same time of day (9:00 a.m.), in order to minimize the effects of diurnal variation. RESULTS: There was a statistically significant difference in median choroidal thickness between the OSAS patients (201 pm; range 145 -237 pm) and the controls (324 pm; range 296 -383 mu m; P <0.001). There were significant differences at all measurement points (P <0.001 for all). The apnea -hypopnea index (AHI) values were more than 30 in all OSAS patients and the mean AHI was 48.57 +/- 6.54. The interexaminer intraclass correlation coefficient (ICC) for the mean choroidal thickness was 0.938 (95%CI, 0.908-0.985) and ICC was greater than 0.90 for all measurement points. CONCLUSION: The decreased choroidal thickness of patients with severe OSAS might be related to the the autonomic disregulation associated with this disease. Further studies are needed to evaluate the etiopathologic relationship between choroidal thickness and OSAS.Item Metastatic Embryonal Conjunctival Rhabdomyosarcoma in a 4-Year-Old Boy(2017) Sezenoz, Almila Sarigul; Karalezli, Aylin; Arat, Yonca Ozkan; Coban, Gokcen; Kiratli, Hayyam; Terzi, Aysen; 0000-0002-4010-2883; 0000-0002-7030-5454; 0000-0002-6578-7444; 26505233; P-7533-2014; AAJ-4860-2021The authors report the case of a 4-year-old boy who presented with unilateral ptosis and a mass lesion of palpebral conjunctiva of the left upper eyelid, that had been present for 2 weeks, and had rapidly enlarged. The lesion was salmon colored and was easily distinguished from the conjunctiva. There was no obvious orbital extension in the MRI studies. Excisional biopsy was performed through a conjunctival approach. The histopathology was consistent with embryonal rhabdomyosarcoma. Thoracoabdominal CT scans revealed nodules in both lungs, indicating stage 4 disease. The patient received chemotheraphy and intensity-modulated radiation therapy. Rhabdomyosarcoma confined to the conjunctiva and distant metastasis without orbital involvement is rare. It should be included in the differential diagnosis of any atypical conjunctival mass lesions in children, and histopathology is necessary to establish proper treatment. As the case indicates, detailed systemic evaluation and careful systemic follow up of these patients are mandatory.Item Pseudoduplication of the optic disc in moderate myopia(2016) Ercan, Zeynep Eylul; Karalezli, Aylin; Coban, Gokcen; 0000-0002-9915-3781; 28003787; AAQ-3136-2020We report a case of optic disc pseudoduplication without pathological myopia. A 79-year-old woman presented with gradually decreased vision. Fundus examination showed macular drusen in both eyes and subretinal haemorrhage in the left eye. Both optic discs appeared to be tilted and the left optic disc was seen to be inferiorly duplicated. Pseudodoubling of the optic discs can be caused by optic disc and peripapillary chorioretinal colobomas and pathological myopia. Considering that our patient did not have enough axial length to merit a diagnosis of pathological myopia and no pathological fundus lesions typical of elongated sclera, we suggest that acquired pseudoduplication is not a condition unique to pathological myopia, but can occur in all myopic patients. The clinical materials, including ophthalmological examination, fundus fluorescent angiography and MRI scan of the orbit, were reported, and its pertinent literatures were reviewed.Item Topiramate-Induced Changes in Anterior Chamber Angle and Choroidal Thickness(2016) Karalezli, Aylin; Koktekir, Bengu Ekinci; Celik, Guner; 26020486; GOE-6067-2022Purpose: To investigate the acute effects of topiramate on the anterior chamber angle (ACA) and choroidal thickness in patients with migraine. Methods: This prospective study included 15 eyes of 15 patients with migraine who have been scheduled to start topiramate therapy. All patients underwent complete ophthalmic examination including measurement of the ACA and choroidal thickness using a spectral domain optical coherence tomography device (Optovue Inc.) and refractive status evaluation with an autorefractokeratometer (KR-8100; Topcon) at the baseline and 1 week after starting therapy. The patients were asked to report any pain or discomfort in their eyes during therapy at the follow-up visit. Results: None of the patients experienced pain or discomfort in their eyes. The mean ACA significantly decreased at the first week of the therapy compared with the baseline levels (40.34 +/- 7.06 degrees and 36.89 +/- 6.87 degrees, respectively) (P=0.001). However, the mean choroidal thickness increased from 277.33 +/- 95.60 mu m at the baseline to 323.40 +/- 84.50 mu m at the first week (P=0.01). There was a nonsignificant increase in the mean refractive error (from -0.25 +/- 0.54 diopter [D] at the baseline to -0.38 +/- 0.49 D after 1 week) (P=0.06). Conclusions: Topiramate can acutely decrease the ACA and increase the choroidal thickness. Methods: A total of 26 eyes of 26 patients with primary pterygium were randomly assigned after pterygium excision as having either fibrin glue (13 eyes) or Vicryl sutures (13 eyes) for CAT. Anterior segment ICGA findings were evaluated postoperatively at 1, 7, and 15 days and the percentages of graft vascularization in both groups were compared using pixel analysis software program. Results: The mean +/- SD age of patients in the suture and fibrin glue groups was 52.1 +/- 12.7 years and 57.1 +/- 9.82 years, respectively. There was no statistically significant difference between the groups regarding age, sex, or follow-up (p<0.05 for all). Also, the mean intraoperative defect size was not significantly different between the groups, which was measured as 20.11 +/- 10.44 mm(2) in the suture group and 23.44 +/- 12.34 mm(2) in the fibrin glue group (p = 0.343). The mean percentage of vascularized graft area at postoperative day 1 and 7 was 18.1 +/- 7.8% and 25.3 +/- 8.6% in the suture group and 34.8 +/- 10.2% and 66.1 +/- 17.8% in the fibrin glue group. The difference between the groups was statistically significant (p<0.01 for both). At postoperative day 15, all grafts were 100% perfused in both groups. Conclusions: Fibrin glue fixation of conjunctival autografts led to more vascularization in the early postoperative period than suture fixated grafts, which in turn may have significance in terms of graft health and pterygium recurrence.
