    A Case of Idiopathic Pulmonary Hemosiderosis Presenting with Signs and Symptoms Mimicking Hemolytic Anemia

    Idiopathic pulmonary hemosiderosis is primarily a disorder of childhood, which is characterized by hemoptysis, iron deficiency anemia, and diffuse parenchymal infiltrates on chest x-ray secondary to recurrent attacks of alveolar hemorrhage. It can be diagnosed by showing hemosiderin laden macrophages in bronchoalveolar lavage fluid after other specific causes of diffuse alveolar hemorrhage are definitely excluded. A 5-year-old male patient was admitted to our clinic with sudden-onset pallor during iron therapy given for anemia. While he was being investigated for clinical and laboratory signs mimicking hemolytic anemia, he developed cough and dyspnea. He had infiltrates on chest x-ray and scattered patchy infiltrates in both lungs on high-resolution computed tomography. Hemosiderin laden macrophages were identified in fasting gastric juice and bronchoalveolar lavage fluid. The patient was diagnosed with idiopathic pulmonary hemosiderosis and started corticosteroid therapy.

    Oxygen Saturation/Minute Heart Rate Index: Simple Lung Function Test for Children

    BackgroundThe severity of airway obstruction can be accurately determined on spirometry in children with asthma. Other assessments may include peak expiratory flow and pulse oximetry. In the present study, we evaluated the validity and reliability of oxygen saturation/minute heart rate (SpO(2)/MHR) index in the prediction of degree of severe airway obstruction in children with asthma. MethodsThis was a retrospective study of children aged 7-17 followed for asthma at Mersin Women and Children's Hospital. The study compared SpO(2)/MHR ratio with forced expiratory volume in 1 s (FEV1) measured on spirometry, an important indicator of small airway obstruction. A total of 296 patients were included in the study, and classified either as having normal FEV1 (FEV1 > 80% of predicted, n = 178) or severely reduced FEV1 (FEV1 < 60% of predicted, n = 118). Positive and negative predictive values (PPV and NPV), sensitivity and specificity of SpO(2)/MHR index in predicting low FEV1 were calculated on receiver operating characteristics analysis. ResultsAn SpO(2)/MHR ratio cut-off <0.90 was associated with a PPV of 83.14%, NPV of 71.77%, sensitivity of 80.34% and specificity of 75.42% in predicting low FEV1. ConclusionsSpO(2)/MHR ratio appears to be a highly useful index to assess airway obstruction in older children with asthma. Thus, it can be used as a marker of airway obstruction severity when spirometry is not available.

    Vitamin D Status in Infants with Two Different Wheezing Phenotypes

    Objective To investigate vitamin D levels in patients with recurrent wheeze at early ages of childhood. Methods In the present cross-sectional study, serum 25-hydroxy vitamin D [25 (OH) D], levels which is known as an indicator of vitamin D adequacy, was examined in infants with three or more wheezing attacks. Results A total of 186 infants with recurrent wheezing were included in the study along with 118 healthy control peers. The recurrent wheezing study participants were classified into two groups according to Asthma Predictive Index (API) positivity and compared to control subjects regarding their serum vitamin D status. The API negative group had the lowest mean serum 25 (OH) D level (n = 121; 22.71 +/- 10.76 ng/ml) followed by API positive group (n = 65; 24.08 +/- 9.02 ng/ml) compared to healthy group (26.24 +/- 11.88 ng/ml) (p < 0.05). In addition, higher vitamin D deficiency was observed in infants in API negative group (52.1 %; p < 0.01) and API positive group (38.5 %; p < 0.05) than control group (31.4 %). Conclusions Low levels of 25 (OH) D were detected in infants with two different phenotypes of recurrent wheeze. Vitamin D deficiency may play a role in the pathogenesis of infants with recurrent wheezing.
