    Nursing Care of a Patient After Burn According to Orem’s Self-Care Theory: A Case Report

    ABSTRACT OBJECTIVES: Burn injuries result in lifelong physical and psychological scars, cause pain, and affect mental health, quality of life, ability to return to work, and death. Although the medical point of view is important for the burn nurse to contribute to patient care, theoretical nursing care should be encouraged. In this case report, we describe a patient with severe burns who received nursing care according to Orem ’s Self-Care Insufficiency Theory. MATERIALS AND METHODS: The 18-year-old male patient received second-degree and third-degree burns of 75% of total body surface area while using thinner to light a heater at work. Burn injuries covered his legs, arms, front body, back body, and certain parts of the face. The patient was transferred to Ankara Baskent University Hospital Burn Intensive Care Unit where he was hospitalized for 85 days. During this period, the patient underwent many surgical treatments, including escharotomy, debridement, and graft surgeries. RESULTS: Thirteen nursing diagnoses were made during his hospital stay. These diagnoses were risk of infection, fear, self-care deficit, risk of bleeding, acute pain, anxiety, impaired physical mobility, disturbed body image, deficient knowledge, impaired skin integrity, sleep pattern disturbance, deficient fluid volume, and imbalanced nutrition of less than body requirements. Contractures developed in the patient ’s legs, preventing him from walking upright. The patient was discharged with continuation of physical therapy, which included pain control during training, maintenance of movement and exercise, giving information about outpatient follow-up, skin care, protection from sun, and nutrition. CONCLUSIONS: With the nursing care applied and in accordance with Orem’s Self-Care Insufficiency Theory, apart from the surgical treatments, the patient ’s lack of knowledge was eliminated, his anxiety level was reduced, his fear was eliminated, his diet was adjusted, and his self-confidence was increased. At discharge, the patient was confident of his care.

    Nursing Care of a Patient Followed in the Intensive Care Unit in the Acute Period After Electrical Burn According to Gordon’s Functional Health Patterns Model: A Case Study

    ABSTRACT OBJECTIVES: The World Health Organization reported about 265 000 deaths per year from fires alone worldwide, with more deaths from electrical burns, scalds, and other causes of burns. Electrical burns are an important public health problem all over the world and in our country of Turkey. Nursing care for patients with electrical burn injuries and support for the patient’s family throughout recovery involve complex and demanding expertise. In this study, we have presented the nursing care of a patient with electrical burn who was followed in the intensive care unit according to Gordon’s Functional Health Patterns Model. MATERIALS AND METHODS: A 44-year-old male patient was admitted to the burn unit after he contacted electrical wires while changing a sign on the highway. He had second-degree and third-degree burns on his arms, hands, and feet (28% of total body surface area). The goals during this phase were saving a life, maintaining and protecting the airways, restoring hemodynamic stability, promoting healing, and preventing complications. In the intensive care unit, we provided care, follow-up, and treatment according to Gordon’s Functional Health Patterns Model and multidisciplinary patient care plans. RESULTS: Nursing care was planned according to effective management of the individual, who had imbalanced nutrition of less than body requirements, deficient fluid volume, constipation risk, self-care deficits, a disturbed sleep pattern, acute pain, interrupted family processes, sexual dysfunction, and anxiety. Nursing diagnoses for interventions were applied, and evaluations were made. CONCLUSIONS: Nurses can positively affect all phases of patient care. In the case described here, Gordon ’s Functional Health Patterns model formed the basis of nursing care, resulting in achievement of goals of the planned nursing care.

    Policy of Burn Care at the Baskent University Ankara Hospital During the COVID-19 Period

    OBJECTIVES: Although hospital admissions and elective surgeries were recommended to be reduced during the COVID-19 pandemic period, the same could not be done for burn patients. It is vital that burn treatment starts immediately and continues without interruption. With this in mind, burn centers should take added precautions during this pandemic. In the present study, we present our experiences in our burn center during the pandemic period. MATERIALS AND METHODS: From March 2020 to February 2021, 457 patients were successfully treated at our center. We implemented training immediately for all hospital staff, with all staff provided with personal protective equipment. We minimized the risk of contamination by completely isolating burn patients and reducing the number of beds. We determined a 3-degree protection protocol according to the general condition of the patients and the diagnosis of COVID-19. During the pandemic period, we continued to treat burn patients by adhering to our protection protocols. RESULTS: During pandemic period, 381 patients (83.4%) were treated as outpatients and 76 patients (16.6%) were hospitalized. The most common cause of burns was scalding, with 172 adult patients (61.4%) and 152 pediatric patients (85.8%). Mean total burn surface area of hospitalized patients was 25.34% in adult patients and 9.95% in pediatric patients. During the treatment process, only 2 patients were diagnosed with COVID-19, and burn treatments were successfully completed, with patients discharged without any problems. CONCLUSIONS: Ensuring isolation among patients in our burn center and determining protection protocols for burn center health care workers played key roles in the continuity of uninterrupted care during the pandemic period.

    Preservation of the Extremity Length With Free Microvascular Tissue Transfers

    ABSTRACT OBJECTIVES: Patients who had unhealed amputations stumps have generally poor vascular condition due to vasculopathies or crush injuries. Tendons and bones are superficial and covered with a thin skin and amputation stump should be revised of changed to a more proximal level MATERIALS AND METHODS: 10 patients were reviewed retrospectively between 2020-2023. Patients were evaluated according to demographics, etiology of the amputations, location of the defects, flap type, recipient vessels, anastomosis fashion, early and late complication rates and hospitalization length . RESULTS: The mean age of the patients was 58 years. Nine of 10 extremity length were successfully preserved with free flap transfers. One patient was referred to orthopedic department for higher amputation level. CONCLUSIONS: Free microvascular tissue transfers are important reconstructive options for extremity salvage.
