    Antithrombotic treatment patterns and stroke prevention in patients with atrial fibrillation in Turkey: Inferences from GARFIELD-AF registry

    Objective: The corner stone of atrial fibrillation therapy includes the prevention of stroke with less adverse effects. The Global Anticoagulant Registry in the FIELD-Atrial Fibrillation (GARFIELD-AF) study provided data to compare treatment strategies in Turkey with other populations and every-day practice of stroke prevention management with complications. Methods: GARFIELD-AF is a large-scale registry that enrolled 52,014 patients in five sequential cohorts at > 1.000 centers in 35 countries. This study was initiated to track the evolution of global anticoagulation practice, and to study the impact of NOAC therapy in AF. A total of 756 patients from 17 enrolling sites in Turkey were in cohort 4 and 5. Treatment strategies at diagnosis initiated by CHA(2)DS(2)-VASc score, baseline characteristics of patients, treatment according to stroke and bleeding risk profiles, and INR values were analyzed in cohorts. Additionally, event rates during the first year follow up were evaluated. Results: AF patients in Turkey were mostly seen in young women. Stroke risk according to the CHADS(2) score and CHA(2)DS(2)-VASc score compared with world data. The mean of risk score values, including HAS-BLED score were lower in Turkey than in the world data. The percentage of patients receiving FXa inhibitor with or without an antiplatelet usage was more than the other drug groups. All-cause mortality was higher in Turkey. Different form world data when HAS-BLED score was above 3, the therapy was mostly changed to antiplatelet drugs in Turkey. Conclusion: In addition to deficiencies in available treatment options, patient care and clinical outcomes of patients with AF, the data of GARFIELD-AF provide data from Turkey about therapeutic strategies and best practices.

    Aortic flow propagation velocity, epicardial fat thickness, and osteoprotegerin level to predict subclinical atherosclerosis in patients with nonalcoholic fatty liver disease

    Objective: Nonalcoholic fatty liver disease is the most common cause of liver dysfunction in Western countries and an independent risk factor for atherosclerotic heart disease. Appropriate noninvasive parameters are lacking for optimal risk stratification of cardiovascular disease in these patients. We evaluated several recently discovered noninvasive parameters for atherosclerosis in patients with nonalcoholic fatty liver disease: epicardial fat thickness, aortic flow propagation velocity, and osteoprotegerin level. Methods: Forty-one patients (27 men and 14 women; mean age, 37.9 +/- 8.9 years) with nonalcoholic fatty liver disease and 37 control subjects (17 men and 20 women; mean age, 34.5 +/- 8.6 years) were enrolled in this observational case-control study. Patients with nonalcoholic fatty liver disease diagnosed at a gastroenterology outpatient clinic were included. Patients with cardiac pathology other than hypertension were excluded. Epicardial fat thickness and aortic flow propagation velocity were measured by echocardiography. The serum concentration of osteoprotegerin was measured using a commercial enzyme-linked immunosorbent assay kit. Results: Nonalcoholic fatty liver disease patients exhibited a significantly lower aortic flow propagation velocity (155.17 +/- 30.00 vs. 179.00 +/- 18.14 cm/s, p=0.000) and significantly higher epicardial fat thickness (0.51 +/- 0.25 vs. 0.29 +/- 0.09 cm, p=0.000) than control subjects. Osteoprotegerin levels were higher, but not significant, in patients with nonalcoholic fatty liver disease (28.0 +/- 13.0 vs. 25.2 +/- 10.8 pg/mL, p=0.244). Binary logistic regression analysis showed that aortic flow propagation velocity (OR, -0.973; 95% CI, 0.947-0.999) and waist circumference (OR, -1.191; 95% CI, 1.088-1.303) were independent predictors of nonalcoholic fatty liver disease. C Conclusion: In this study, epicardial fat thickness and osteoprotegerin level were higher and aortic flow propagation velocity was lower in patients with nonalcoholic fatty liver disease. Early detection of abnormal epicardial fat thickness and aortic flow propagation velocity may warrant a search for undetected cardiovascular disease in patients with nonalcoholic fatty liver disease.

    The effect of cold stress on right ventricular function in patients with systemic sclerosis

    Objective: Pulmonary hypertension and right heart failure are poor prognostic factors in systemic sclerosis (SSc). The effect of cold stress on pulmonary vasospasm was evaluated in some trials but the results were conflicting. The aim of our study was to determine the effect of cold stress on right ventricular (RV) function. Materials and Methods: Twenty-four patients with SSc and 24 age and sex-matched healthy controls were enrolled in the study. Transthoracic echocardiography (TIE) was performed to all patients at rest and after peripheral cold exposure. Baseline and after cold stress test TIE results were compared between the groups. Results: Cold exposure induced changes in particularly right venticular function. Pulmonary acceleration time was significantly shortened in the SSc group (118.8 +/- 11.7ms vs 111.3 +/- 13.7ms, P<0.001). Tricuspid annular plane systolic excurison and RV fractional area change were also decreased after cold exposure in the SSc group. (23.3 +/- 1.6mm vs 21.9 +/- 1.9 mm, P<0.001; 46.3 +/- 5.8 vs 44.4 +/- 5.8,P= 0.007, consecutively). Left ventricular (LV) function did not change after cold stress (LV myocardial performance index (0.42 +/- 0.04 vs 0.42 +/- 0.05), P=0.748). Conclusion: Peripheral cold exposure caused a reduction in right ventricular function indicating pulmonary Raynaud's phenomenon as a possible contributing factor.

    Learning in the Digital Age: E-Learning in Cardiology Specialization Training

    Digital cardiologists are cardiologists who can use digital media in clinical practice, education, and interaction with patients. Although digital tools are supportive of education, they cannot replace human thoughts and feelings. On the other hand, it is useful to arrange the core training program of cardiology in a way to train digital cardiologists of the future. Educators lacking this opportunity during their training should adapt quickly to e-learning. Blended use of e-learning and face-to-face learning seems to be the most effective way. in this way, the trainer will transfer his experiences on one hand and on the other hand, he will enable the learner to acquire lifelong learning habit by supporting the self-learning activity with the e-learning opportunities.

    Relationship Between Melatonin and Cardiovascular Disease

    Coronary artery disease (CAD) is one of the leading causes of morbidity and mortality worldwide. The coronary atherosclerotic process involves different pathological mechanisms; inflammation is one of the major triggers for the development of atherosclerotic plaque. Although several studies showed the favorable effects of melatonin on the cardiovascular system (CVS), melatonin seems not to take its rightful place in today's clinical practice. This review aims to point out the role of melatonin on cardiovascular disease (CVD) and its' risk factors. All data were obtained via PuhMed, Wikipedia, and Google.
