Özalay, MetinŞentürk, İhsan2015-04-182015-04-182008http://hdl.handle.net/11727/2171Çalısmamızda biseps kronik tenosinoviti nedeniyle biseps tenodezi ve biseps tenotomisi yapılan hastaların klinik ve izokinetik karsılastırması yapıldı. 2001 ve 2007 yıllar arasında Baskent Üniversitesi Adana Uygulama ve Arastırma Merkezinde kronik biseps tenosinoviti tanısı konan; konservatif tedaviye yanıt vermeyen yirmi hastaya artroskopik biseps tenotomisi veya artroskopi yardımlı veya artroskopik biseps tenodezi yapıldı. Ek cerrahi girisim olarak 18 hastaya rotator manset tamiri ve akromioplasti ve 2 hastaya akromioplasti uygulandı. On hastaya artroskopik biseps tenotomisi (5 kadın, 5 erkek; ortalama yas 62,90; dağılım 53-75), sekiz hastaya artroskopi yardımlı ve iki hastaya artroskopik biseps tenodezi (4 kadın, 6 erkek; ortalama yas 56,90; dağılım 49-66) uygulandı. Hastalar, ortalama 3,1 yıl (dağılım 1-8 yıl) takip edildi. Artroskopi yardımlı biseps tenodezi bir hastada anahtar deliği tekniği ile ve yedi hastada ise dikis kancası tekniği ile yapıldı. Artroskopik biseps tenodezi biyolojik eriyebilen interferans vidası tekniği ile yapıldı. Tüm hastalar ameliyat öncesi ve ameliyat sonrası Constant ve Ucla skorları ile değerlendirildi. Hastalar, ortalama 3,1 yıl (dağılım 1-8 yıl) takip edildi. Đzokinetik olarak. Cybex(Biodex 3, Cybex Biomedical System, NY, USA) izokinetik dinamometre cihazı yardımıyla dirsek fleksiyonu ve ön kol supinasyonu karsılastırıldı. Hastaların ameliyat öncesi Constant skorları tenotomi grubunda ortalama 64,40, tenodez grubunda ortalama 62,80 iken ameliyat sonrası tenotomi grubunda ortalama 89,50, tenodez grubunda 86,70 bulundu. Ameliyat öncesi Ucla değerleri ortalama 23,20, tenodez grubunda ortalama 22,60 iken ameliyat sonrası tenotomi grubunda ortalama 30,00, tenodez grubunda ortalama 29,20 bulundu. Her iki grupta da tedavi öncesine göre Ucla ve Constant skorlarında anlamlı düzelme bulundu (p<0,01). Gruplar arası karsılastırmada Constant, Ucla skorları ve izokinetik değerler arasında istatiksel anlamlı fark bulunmadı (p>0,05). Tenotomi uygulanan hastalarda tendonun distale migrasyonu veya popeye deformitesi görülmemistir. Kronik biseps tenosinovitinde biseps uzun bası tenotomisi; klinik, fonksiyonel, izokinetik ve kozmetik açısından biseps tenodezi kadar iyi sonuçlar vermektedir. The purpose of this study to compare clinical and isokinetic results of patients in whom biceps tenodesis and biceps tenotomy were done for chronic tenosynovitis. Arthroscopic biceps tenotomy, arthroscopy asisted or arthroscopic biceps tenodesis were done in 20 patients whose diagnosis was chronic tenosynovitis and in whom conservative treatment was not helpful. Rotator cuff repair and acromioplasty for 18 patients and acromioplasty for two patients were done in addition to biceps surgery. Arthoscopic biceps tenotomy was done in 10 patients (5 female, 5 male; mean age 62.90, distrubition 53-75), arthroscopy assisted biceps tenodesis was done in 8 patients (4 female, 6 male; mean age 56.90, distrubution 49-66). All patients were evaluated with Constant and Ucla scores preoperatively and postoperatively. The average follow-up of the patients 3.1 years (between 1-8 years). Isokineticly elbow flexion and forearm supination were compared with the Cybex(Biodex 3, Cybex Biomedical System, NY, USA) machine. Preoperative average constant scores of tenotomy and tenodesis groups were 64.40 and 62.80 respectively, whereas postoperative scores 89.50 and 86.70 respectively. Preoperative average Ucla scores were 23.20 in tenotomy group and 22.60 in tenodesis group, whereas postoperative Ucla scores of tenotomy and tenodesis groups were 30.00 and 29.20 respectively. In both groups statistically significant improvement of Ucla and Constant scores were seen in respect to pretreatment clinic (p<0.01). Comparision between Constant, Ucla scores and isokinetic measurements of both groups show no significant difference statistically (p>0.05). Any complications were not seen in all patients. In the treatment of chronic tenosynovitis, like biceps tenodesis, biceps long head tenotomy has good clinic, functional, isokinetic and cosmetic results.
