Benign paroksismal pozisyonel vertigoda anksiyete, depresyon, uyku ve yaşam kalitesi düzeylerinin değişimi
Özet
Benign paroksismal pozisyonel vertigo (BPPV), çok sık görülen bir periferik vestibüler
sistem hastalığıdır. Bu sebeple BPPV hasta grubunun belirlenmesi ve kişiye uygun sağlık
bakımın yapılmasıyla, bu kişilerin hızlı bir şekilde günlük yaşama dönmesi sağlanmış
olacaktır. BPPV’li bireylerde şiddetli baş dönmesi şikayeti sebebiyle anksiyete, depresyon,
uyku ve yaşam kalitesi seviyelerinde azalma görülebilir veya bu faktörlerin az olması
sebebiyle BPPV ortaya çıkabilir. Bu faktörlerden herhangi birinin değişimi diğer faktörleri
de etkileyebilir. Bu sebeple tüm değişkenler kendi içinde değerlendirilmeli ve kişiye
yönelik doğru tedavi uygulanmalıdır. Bu çalışma BPPV’li bireylerde bu değişkenleri
değerlendirmek ve uygun tedavi ile birlikte bu değişkenlerin düzeylerinde bir farklılık olup
olmadığını görmek amacıyla yapılmıştır. Bu amaca göre BPPV hastalarına atak döneminde
ve iyileştikten sonraki dönemde Richard-Campell Uyku Kalitesi Ölçeği (RCUÖ), Baş
Dönmesi Engellilik Envanteri (BDEE), Beck Anksiyete Ölçeği (BAÖ) ve Beck Depresyon
Ölçeği (BDÖ) uygulanarak, kişilerin iki farklı zamandaki depresyon, anksiyete, uyku ve
yaşam kalitesi seviyeleri arasındaki farklılığa bakılmıştır.
Çalışmaya 18-65 yaş aralığında 32’si kadın (%76,2) ve 10’u erkek (%23,8) toplamda 42
BPPV tanısı konulmuş katılımcı dahil edilmiştir. BPPV tanısı videonistagmografi cihazı ile
pozisyonel testlerde pozitif bulgu görülen kişilere konulmuştur. Bu kişilere atak
döneminde ve iyileştikten sonra aynı koşullarda olacak şekilde ilgili envanter ve ölçekler
uygulanmıştır.
Çalışmada, BPPV tanısı konulmuş kişilerin atak döneminde ve bu kişiler iyileştikten sonra
yapılan değerlendirmeler sonucunda iki farklı zamandaki uyku kalitesi, yaşam kalitesi,
depresyon ve anksiyete düzeyleri arasında istatistiksel olarak farklılık bulunmuştur
(p<0,05). Bu farka rağmen her iki zamandaki anksiyete, depresyon ve uyku kalitesi
seviyeleri normal sınırlarda elde edilmiştir. Aktif baş dönmesinin olduğu zaman yapılan BDEE sonuçlarında kişilerin günlük yaşamlarında orta derece engelliliğe sahip olduğu ve
anksiyete, depresyon ve uyku kalitesinden bağımsız olarak baş dönmesine bağlı yaşam
kalitelerinin düştüğü bulunmuştur.
Uyku kalitesi, depresyon ve anksiyete seviyelerinin bir bütün olarak değerlendirilmesi
gerektiği ve bunların yaşam kalitesini önemli derecede etkileyebilecek faktörlerden
oldukları unutulmamalıdır. Çalışmanın sonucunda, kullanılan envanter ve ölçeklerin hasta
değerlendirme bataryasına eklenmesi önerilir.
Benign paroxysmal positional vertigo (BPPV) is a very common peripheral vestibular
system disease. For this reason, by identifying the BPPV patient group and providing
appropriate health care to the individual, it is ensured that these people return to daily life
quickly. BPPV patients may experience decreased levels of anxiety, depression, sleep and
quality of life due to severe dizziness. On the contrary, BPPV may occur due to the lack of
these factors. Changes in any of these factors can affect other factors as well. For this
reason, all variables should be evaluated within themselves and the right treatment should
be applied to the individual. This study was conducted to evaluate these variables in BPPV
patients and to see if there is a difference in the levels of these variables with appropriate
treatment. For this purpose, Richard-Campell Sleep Questionnaire (RCSQ), Dizziness
Handicap Inventory (DHI), Beck Anxiety Inventory (BAI) and Beck Depression Inventory
(BDI) were applied to BPPV patients during the attack period and after recovery. The
difference between anxiety, sleep and quality of life levels was examined.
A total of 42 participants, 32 female (76.2%) and 10 male (23.8%) aged between 18-65
years, diagnosed with BPPV were included in the study. The diagnosis of BPPV was made
in people who showed positive findings in positional tests with the videonystagmography
device. The relevant inventory and scales were applied to these individuals in the same
conditions during the attack period and after recovery.
In the study, as a result of the evaluations made on BPPV patients, a statistical difference
was found between sleep quality, quality of life, depression and anxiety levels at two
different times (p<0.05). Despite this difference, anxiety, depression and sleep quality
levels at both times were within normal limits. In the BDEE results performed when active
dizziness is present, it has been found that people are moderately inhibited in their daily lives. It has been found that the quality of life of people due to dizziness has decreased,
independent of anxiety, depression and sleep quality.
It should not be forgotten that sleep quality, depression and anxiety levels should be
evaluated as a whole and these are factors that can significantly affect quality of life. As a
result of the study, it is recommended to add the used inventories and scales to the patient
evaluation battery.
