Titanyum alaşım biyomalzemelerde plazma polimerizasyon tekniği kullanılarak kemik morfogenetik protein tutuklanmasının incelenmesi
Özet
Ortopedi, omurga cerrahisi, ve diş hekimliği uygulamalarında vida, plak vb benzer
malzemelerin üretiminde titanyum alaşım içerikli metaller tercih edilirken(
mükemmel mekanik özellik, kimyasal kararlılık, biyouyumluluk vs.) bu malzemelerin,
kemik hücreleri ve dokusu ile bağlanmasındaki zayıflık bir süre sonra implantın
çıkmasına, kaynamamasına, kırılmasına ve buna bağlı ciddi klinik bulguların ve/ya
tekrarlayan revizyon cerrahilerine neden olmaktadır. Çok sık karşılaşılan eksik
kaynama problemlerinin önüne geçilmesi, maddi olarak sarfiyat azaltma, hasta
konforu vs. açısından son derece önemlidir. Bu nedenle, bel ve omurga cerrahi
uygulamalarda sıklıkla kullanılan yöntem, kemik dokusu ile olan etkileşiminin
(osteointegrasyon) en üst seviyeye çıkartılması için, kemik dokusu gelişimini indukte
eden proteinlerin kemik morfogenetik proteinin (rhBMP-2) kemiğe sürülmesi ve
sonrasında titanyum alaşım implant malzemesinin (transpediküler vida) fikse
edilmesidir. Ancak, tümüyle doğal (bu nedenle biyouyumlu) bu proteinin, toz halinde
omurilik bölgesine dökülmesi, hem maliyeti artırmakta hem de kemikteki doz
fazlalığından dolayı hala komplikasyonlar (heterotropik ossifikasyon, osteoliz,
enflamatuar kist formasyonları vs.) oluşturmaktadır. Günümüz teknolojisinde ayrıca
dinamik stabilizasyon ve perkütan enstrumantasyon teknikleri giderek
yaygınlaşmaktadır. Dinamik stabilizasyonda omurgalar arası füzyon (füzyon:
kemiksel birleşme ile iki omur arasındaki anormal hareketin önlenmesi)
uygulanmamakta, minimal invaziv perkütan cerrahi de ise füzyon interbody füzyon
şeklinde uygulanabilmektedir. Her iki cerrahide de sistemin stabilitesi açısından vida
kemik integrasyonu çok önem kazanmaktadır.
Ülkemizde benzer amaçla, açık cerrahide kullanılabilen hidroksiapatit kaplı vidalar
mevcuttur. HA kaplamasının pahalı olması, ülkemizde yapılamaması bu tekniğin
dezavantajlarıdır. Ayrıca HA ile yapılan kaplamalarda metal ve HA ‘in yoğunluk
farkından kaynaklı kırılma problemleminden kaynaklı implantın çıkma problemleri
sıklıkla görülmektedir. Araştırmada bu problemi çözmek amacıyla, plazma
polimerizasyon tekniği ile malzeme yüzeyinde uygun kimyasal gruplar olusturulması
ve devamında (rhBMP-2) implant üzerine direkt tutuklanması yaklaşımı
kullanılmıştır. Plazma polimerizasyon tekniğinde, özgün monomer (sisteamin) ve
yük boşalım tipi (radyo frekansı), kullanılmıştır. Ayrıca yöntemdeki işlem
parametreleri (güç (W), süre (dak), monomer akış hızı (ml/dak) değiştirilerek
yüzeyde meydana gelen fiziksel ve kimyasal farklılıklar, çeşitli karakterizasyon
teknikleri ile tanımlanmıştır. Bu sonuçlar degerlendirilerek, nanomertebe film
oluşturulmasında, akış dinamik ortamında en kararlı olan plazma polimerizasyon
parametresi seçilmiştir. Kararlılığa göre en uygun parametre güç: 30W, süre: 9 dak,
akış hızı : 10 ml/dak’ dır. Seçilen işlem koşullarında plazma polimerizasyon tekniği
ile modifikasyon ve sonrasında % 2.5 v/v ‘lik glutaraldehit ile aktivasyon aşamaları
devamında % 1.5 mg/ml rhBMP-2, 2 saat süreyle tutuklanmıştır. İmplant yüzeyde
(rhBMP-2) tutuklanması ile kemik dokusu gelişimini arttırdığı histolojik testlerle
kanıtlanmıştır. Sonuç olarak, spinal implantların plazma polimerizasyon tekniğiyle
kemik yapımını arttırıcı rhBMP-2 ile kaplanması yaklaşımı, canlı dışı (in vitro) / ve
canlı içi (in vivo) ortamlarda test edilmiş ve değerlendirilmiştir.
Titanium is the material of choice in the production of implants used in orthopedics,
spine surgery and dentistry due to its excellent mechanical properties, chemical
stability and biocompatibility. The loose interaction between the implants and bone
result in implant loosening, pull-out and screw breakage. Serious symptoms and
probable revision surgeries can develop because of implant related problems. The
acceleration of new bone development, production of new implant types and use in
health area are strategically important steps. Therefore, waist and spine surgery in
commonly used method, the bone tissue and the interaction (osteointegration) to
maximize the level of bone tissue formation bone proteins which induce
morphogenetic protein (rhBMP-2) is placed on the bone and then the titanium alloy
implant material (transpedicular screws) is not fixed. However, completely natural
(and therefore biocompatible) this protein spine pouring powdered, both increases
the costs and due to dose excess of bone still complications (heterotopic
ossification, osteolysis, inflammatory cyst formation, etc.).In the millenium, dynamic
stabilisation and percutaneous instrumentation techniques have been started to be
widely used. Interbody fusion is used in minimally invasive percutaneous spine
surgery while no bony fusion was performed in dynamic stabilisation. New bone
formation around the screw minimizes the risks of screw loosening, pull-out and
screw breakage. In our country, only hydroxyapatite (HA) coated screws are
available for increasing the screw bone integration. HA coating is expensive and
cannot be performed in our country. Since HA coating is dependent on European
countries, we planned to coat the spinal implants with recombinant bone
morphogenetic protein (rhBMP) by plasma polymerisation technique.
In order to solve this problem in the research of material surface by plasma
polymerization techniques in the creation of appropriate chemical groups and
continue the bone morphogenetic protein (rhBMP-2) approach is used directly on
the implant immobilization. In the plasma polymerization technique, the original
monomer (cysteamine) and load discharge type (radio frequency) is used.
Furthermore, the method of processing parameters (power (W) time (min),
monomer flow rate (ml / min) by changing the surface physical and chemical
differences occurred were identified by various characterization techniques. The
results are evaluated in forming nanoscale film flow at the dynamic environment
stable plasma polymerization parameters were selected. Optimal parameter power
by resolution: 30 W, time: 9 min, flow rate: 10 ml / min . The modification by plasma
polymerization technique to the selected processing conditions and after the
activation (2.5% v / v, glutaraldehyde) step continued in is 2.5% mg/ml rhBMP-2
arrested for 2 hours. It is proven by histological testing the implant surface rhBMP-2
enhances bone tissue formation with the immobiliziton. As a result, spinal implants
plasma polymerization coating technique with bone formation by increasing the
bone morphogenetic protein (rhBMP-2) approach , outside the body (in vitro) / and
animal (in vivo) has been tested and evaluated.
