Parkinson hastalarında subtalamik nükleus-derin beyin stimülasyonunun odyo-vestibüler etkileri
Özet
Çalışmada; Parkinson hastalığı olan ve tedavi için derin beyin stimülasyonu uygulanan hastaların işitsel ve vestibüler yolakların değerlendirilmesi, olası etkilerin ortaya konması amaçlandı. Dışkapı Yıldırım Beyazıt Eğitim ve Araştırma Hastanesi Nöroloji ve Kulak Burun Boğaz kliniklerinde katılımcıların muayeneleri yapıldı. Parkinson hasta grubu (30 birey), subtalamik nükleus-derin beyin stimülasyonu uygulanmış grup (30 birey) ve sağlıklı grup (24 birey) olmak üzere üç grup oluşturuldu. Odyolojik değerlendirmede; işitme testi, immitansmetre, transient otoakustik emisyon testi (gürültü yokluğunda/ kontra gürültü varlığında) uygulandı. Vestibüler değerlendirmede ise; baş dönmesi handikap envanteri, videonistagmografi, modifiye denge üzerinde duyusal etkileşim klinik testi, video head impuls test uygulandı. Elde edilen değerler üç grup arasında ayrıca derin beyin stimülasyonu uygulanmış hastalarda pil açık ve pil kapalı durumlarında karşılaştırıldı. Parkinson hastaları ve derin beyin stimülasyonu gruplarında işitme eşikleri ve konuşmayı ayırt etme skorları anlamlı olarak daha düşük bulundu (sırasıyla p<0,001, p=0,002). Otoakustik emisyon değerlerinde, en az emisyon yanıtı derin deyin stimülasyonu grubunda alındı (p=0,023). Pil açık ve kapalı durumlarında bir fark izlenmedi. Videonistagmografi sonuçlarında spontan nistagmus, gaze vertikal, gaze horizontal, sakkad ile grup değişkeni arasında anlamlı ilişki saptandı. Video head impuls test sonuçlarında sağ posterior kanal, sağ anterior kanal ve sol posterior kanal değişkenleri bakımından anlamlı fark bulundu (sırasıyla p<0,001, p=0,018, p=0,039, p=0,025). Modifiye denge üzerinde duyusal etkileşim klinik testinde anlamlı fark izlenmedi. Parkinson hastalığı işitme eşiklerini olumsuz yönde olumsuz yönde etkilemiştir. Ayrıca efferent işitme sistemini de etkilediği izlemiştir. Literatürde derin beyin stiülasyonu uygulanmış hastalarda yapılmış otoakustik emisyon çalışması bulunmadığı için çalışmamız literatüre yapılan ilk katkıyı oluşturmuştur. Parkinson hastalığında vestibüler sistemin de etkilendiği görülürken, derin beyin stimülasyonunun işitme ve vestibüler sisteme olumlu etkisi ortaya konulamamıştır.
In the study; It was aimed to evaluate the auditory and vestibular pathways of patients with Parkinson's disease and to whom deep brain stimulation was applied for treatment, and to reveal the possible effects. Participants were examined in the Neurology and Ear Nose Throat clinics of Dışkapı Yıldırım Beyazıt Training and Research Hospital. Three groups were formed: Parkinson's patient group (30 subject), subthalamic nucleus-deep brain stimulation applied group (30 subject) and healthy group (24 subject). In the audiological evaluation; hearing test, immittancemeter, transient otoacoustic emission test (in the absence of noise/in the presence of contra noise) were performed. In the vestibular evaluation; dizziness handicap inventory, videonystagmography, sensory interaction clinical test on modified balance, video head impulse test were applied. The obtained values were compared between the three groups, and in patients who had deep brain stimulation, they were compared in battery on and battery off states. Hearing thresholds and speech discrimination scores were found to be significantly lower in Parkinson's patients and deep brain stimulation groups (p<0.001, p=0.002, respectively). In otoacoustic emission values, the least emission response was obtained in the deep brain stimulation group (p=0.023). No difference was observed between the battery on and off states. A significant correlation was found between spontaneous nystagmus, gaze vertical, gaze horizontal, saccade and group variable in videonystagmography results. A significant difference was found in video head impulse test results in terms of right posterior canal, right anterior canal and left posterior canal variables (p<0.001, p=0.018, p=0.039, p=0.025, respectively). No significant difference was observed in the sensory interaction clinical test on modified balance. Parkinson's disease has negatively affected hearing thresholds. It has also been observed that it affects the efferent auditory system. Since there is no otoacoustic emission study conducted in patients who underwent deep brain stimulation in the literature, our study constituted the first contribution to the literature. While it is seen that the vestibular system is also affected in Parkinson's disease, the positive effect of deep brain stimulation on the auditory and vestibular system has not been demonstrated.
