Çenelerde sık görülen odontojenik kist ve tümörlerin klinik ve radyolojik bulguları ile ki-67 ve p53 protein ekspresyon oranları arasındaki ilişkinin değerlendirilmesi

Ağız diş ve çene cerrahisinde ana konulardan biri kistik lezyonların ayırıcı tanısıdır
çünkü bazı odontojenik kistler (OK) agresif davranış ve nüks etme eğilimindedir. Ek
olarak, en uygun cerrahi tedavi ve takibi belirlemek için kist ve çene tümörlerinin
ayırıcı tanısını koymak önemlidir. Patolojinin tanısı klinik ve radyolojik özelliklere
dayanmaktadır, ancak kesin tanı histopatolojiye dayanmaktadır. Mevcut retrospektif
çalışma 2011-2016 arasında yapıldı ve değerlendirmeler histopatolojik raporlar ve
panaromik filmler üzerinden ölçümlemeler ile gerçekleştirildi. Çalışmaya Başkent
Üniversitesi Çene Hastalıkları ve Cerrahisi Kliniğinde tedavi edilen hastalardan 111
odontojenik kist ve tümör örneği alındı. Bu örnekler kist tipi, yaşı, cinsiyeti ve
immünohistokimyasal boyama dağılımı için analiz edildi ve radyolojik bulgularla
karşılaştırıldı. Bu çalışmanın sonuçlarına göre, Ki-67 ve p53 gen ekspresyon
değerlerinin odontojenik keratokist grubunda anlamlı derecede yüksek olduğu
bulundu. Odontojenik keratosistlerin radiküler kistlere ve dentigeröz kistlere göre
daha yüksek malign transformasyon potansiyeli gösterdiği ortaya çıkmıştır. Ki-67 ve
p53 gen ekspresyon oranları ile radyolojik ölçümler arasındaki ilişki kist-tümör
gruplarına göre karşılaştırıldığında, sadece dentigeröz kist grubunda istatistiksel
olarak anlamlı ilişki bulunmuştur. Panoramik filmlerde dentigeröz kistlerin boyutsal
karşılaştırmalarından sonra, agresif potansiyelin ilgili kist grubundaki ekspresyon
hızı ile radyolojik ölçüm arasındaki anlamlı ilişki nedeniyle artan çapla arttığı
söylenebilir. Bu, etkilenen dişlerle ilişkili dentigeröz kistlerinin klinik uygulamada en
kısa sürede ortadan kaldırılması gerektiğini göstermektedir.
One of the main issues in dental and oral maxillofacial surgery is the differential diagnosis of
cystic lesions because few odontogenic cysts (OC) tend to aggressive behavior and relapse. In
addition, it is important to make differential diagnosis of cysts and jaw tumors in order to
determine the most appropriate surgical treatment and follow-up. The diagnosis of pathology is
based on clinical and radiological features, but the definitive diagnosis is based on
histopathology. The current retrospective study was conducted between 2011 and 2016 and
evaluations were performed on histopathologic reports. 111 biopsy specimens of odontogenic
cysts and tumors were retrieved from patients who were treated at the Maxillofacial Surgery
Department of Başkent University were included into the study. These specimens were analyzed
for the type of cyst, age, sex and immunohistochemical staining distribution and compared with
radiologic findings.According to the results of this study, Ki-67 and p53 expression values were
found to be significantly higher in the odontogenic keratocyst group than the other groups, and it
was revealed that keratocysts showed higher malignant transformation potentials than radicular
cysts and dentigerous cysts.When the relationship between Ki-67 and p53 expression rates and
radiological measurements were compared according to cyst-tumor groups, there was a
statistically significant relationship only in dentigerous cyst group. After the dimensional
comparisons of dentigerous cysts in panoramic films, it can be said that the aggressive potential
increases with increasing diameter due to the significant relationship between the expression rate
in the relevant cyst group and radiological measurement. This suggests that dentigerous cysts
associated with impacted teeth should be eliminated as soon as possible in clinical practice.
