dc.description.abstractOrgan transplantasyonu, 20. yüzyılın en önemli gelişmelerinden biri olup son evre böbrek ve karaciğer yetmezliğinin tedavisi olarak klinik uygulamada yerini almıştır. Özellikle siklosporin-A (CSA) ve FK-506 (Tacrolimus) gibi immünosupresif ilaçların kullanılmasıyla tedavi başarısı artmış, transplantasyon yaygın bir şekilde uygulanır hale gelmiştir. Ancak bu immünosupresif ilaçlar, ciddi yan etkilere sahiptir ve transplantasyonu takiben morbidite ve mortaliteyi artırabilirler. Bilhassa nörolojik yan etkiler tedaviyi sınırlayıcı ve yaşamı tehdit edici olabilmektedir. Bu çalışma, Başkent Üniversitesi Tıp Fakültesi'inde, 1 Ocak 1994- 1 Ekim 2004 tarihleri arasında yapılan karaciğer ve böbrek transplantasyonu hastalarında retrospektif olarak major santral nörolojik yan etki oranının belirlenmesi ve yan etki- kan ilaç düzeyi arasındaki ilişkinin saptanmasına yönelik yapılmıştır. Ayrıca yan etkinin meydana geliş zamanı da değerlendirilmiştir. 505 böbrek transplantasyonu hastasından 364'ü, 91 karaciğer transplantasyonu hastasından 27'si kriterleri karşıladığından çalışmaya alınabilmiştir. Bu hastaların 249 (%63,7)'unun CSA, 61 (%15,6)'inin FK-506, 81 (%20,7)'inin farklı zamanlarda olmak üzere her iki ilacı kullandığı tespit edilmiştir. 364 böbrek transplantasyonu hastasının kayıtlarının retrospektif olarak incelenmesi sonucunda 10 (%2,7)'unda, 27 karaciğer transplantasyonu hastasının kayıtlarının incelenmesi sonucunda ise 6 (%22,2)'sında major santral nörolojik yan etki saptanmıştır. Nörotoksisite saptanan 16 hastanın 12 (7 böbrek, 5 karaciğer)'si CSA, 4 (3 böbrek, 1 karaciğer)'ü FK-506 kullanmakta idi. Hem karaciğer transplantasyonu hem de böbrek transplantasyonu olan hastalarda, her iki ilaç arasında major santral nörolojik yan etki görülme oranı açısından istatistiksel olarak anlamlı bir farklılık bulunmamıştır. Nörotoksisite- kan ilaç düzeyi arasında ilişki olup olmadığı değerlendirildiğinde ise, CSA kullanan 12 hastanın 6'sında kan ilaç düzeyi normal, diğer 6'sında ise yüksek bulunmuştur. Aynı şekilde, FK-506 kullanan 4 hastanın 2'sinde normal, 2'sinde yüksek tespit edilmiştir. Bu yüzden kan ilaç düzeyi ile nörotoksisite gelişimi arasında anlamlı bir ilişki saptanmamıştır. Ayrıca, verilerden nörotoksisite oluş zamanı- ilaç kullanım süresi değerlendirildiğinde, 16 hastanın 11 (%68,8)'inde ilk 3 ay içinde nörotoksisite meydana geldiği görülmüştür.Bu çalışma sonuçları, her iki transplantasyon tipi hakkında literatürde bildirilen sonuçlarla uyumlu bulunmuştur. The organ transplantation, one of the most important developments in the 20th century, has become a viable therapy option for patients suffering from renal and liver disease. Specifically, the chance in successful outcome in treatment has increased and transplantation has been used widely by using immunosuppressive agents such as cyclosporin-A (CSA) ve FK-506 (Tacrolimus). But these immunosuppressive agents have serious adverse "side" effects and they may increase the morbidity and mortality after transplantation. Especially, neurological side effects may limit therapy and could be life threatening. The interest of this study is to identify major neurological side effects and to determine the relationship between side effects and blood drug levels in renal and liver transplant recipients retrospectively at the University of Başkent between January 1, 1994 and October 1, 2004. In addition, the timing of the side effects was also evaluated. 364 eligible patients of 505 renal transplant recipients and 27 eligible patients of 91 liver transplant recipients, who met the criteria, were included in the study. It was established that 249 (%63,7) of these patients were receiving CSA, 61 (%15,6) of them receiving FK-506, and 81 (%20,7) of the patients had used both drugs during different periods. Retrospective examination of the patient records showed that 10 out of 364 (2.7%) kidney transplant patients and 6 out of 27 (22.2%) liver transplant patients have experienced major central neurological side effects. Among 16 patients having neurotoxicity effects, 12 (7 kidney patients and 5 liver patients) were treated with CSA whereas 4 (3 kidney patients and 1 liver patient) of them were treated with FK-506. Among the patients who underwent either kidney or liver transplants, no significant statistical difference was observed between two drugs as far as the major central neurological side effects are concerned. When assessing the relationship between neurotoxicity and blood drug level for the patients treated with CSA, it was found that 6 out of 12 patients had normal and the other 6 patients had high blood drug level. Similarly, 2 out of 4 patients treated with FK-506 had normal and the other two had high blood drug levels. Therefore, no conclusive relationship could be found. Besides, when assessing the timing of neurotixicity due to the drug usage period by using the data, it was seen that neurotoxicity occurs in the first 3 months in 11 (%68,8) out of 16 patients. The results of this study are consistent with the results reported in the literature for both types of transplantation.en_US
