Koroner arter bypass greftleme cerrahisi geçiren hastalarda görülen gastrointestinal sistem değişiklikleri
Özet
Kardiyopulmoner bypass (KPB) uygulanan hastalarda gastrointestinal sistem (GİS)
komplikasyon insidansı %0.3-2 arasında değişmektedir. Ancak GİS komplikasyonunun
görüldüğü hastalarda mortalite oranı oldukça yüksektir (%11-59). Çalışmamızın amacı, KPB
uygulanan ve uygulanmayan hastalarda GİS’de meydana gelen olumsuz değişiklikleri tespit
etmek ve intestinal yağ asidi bağlayıcı protein (I-FABP) ile GİS değişiklikleri arasında ilişki
olup olmadığını belirlemektir.
Başkent Üniversitesi Klinik Araştırma ve Etik Kurulu ve hasta onamları alındıktan sonra
elektif koroner arter cerrahisi uygulanacak 50-70 yaşları arasında ASA III hastalar çalışmaya
alındı. KPB uygulanan 35 hasta birinci grup, uygulanmayan 16 hasta ikinci grup olarak kabul
edildi. Çalışma prospektif, kontrollü düzende yapıldı ve çalışma koşullarını sağlayan hastalar
çalışmanın devam ettiği süre içinde ardışık olarak alındı. Hastaların preoperatif dönemde ve
postoperatif 24. saatte aspartat aminotransferaz, alanin aminotransferaz, total ve direkt
bilirubin, amilaz, kan üre azot, kreatinin, laktat, lökosit ve hemoglobin düzeylerine bakıldı.
Hastaların operasyon öncesi, operasyon sonu, postoperatif 12. ve 24. saatlerde I-FABP ve
intraabdominal basınç değerlerine de bakılarak arteriyel kan gazları alındı. Gruplar arası
değişkenler Student’s t ve Mann Whitney U testleriyle gruplar içi değişkenler ise Wilcoxon
testi ile değerlendirildi. p<0.05 değeri istatistiksel olarak anlamlı kabul edildi.
Her iki gruptaki hastaların demografik özellikleri benzerdi. Operasyon süreleri KPB
uygulanan grupta daha uzun bulundu (p<0.001). Greft sayısı KPB uygulanan grupta 3.8 ±
1.16, KPB uygulanmayan grupta ise 2.1 ± 0.88 idi (p<0.01). Hastaların ekstübasyon
zamanları ve hastanede kalış süreleri arasında fark bulunmazken, KPB uygulanan grupta
yoğun bakım ünitesinde kalış süresi daha uzundu (p<0.001). Postoperatif KPB uygulanan
grupta bir hastada nörolojik komplikasyon izlendi (p>0.05). Sekiz hastada postoperatif atriyal
fibrilasyon görüldü (p<0.043). KPB uygulanan hasta grubunda idrar çıkışı daha fazla olurken
(p<0.001), daha fazla eritrosit süspansiyonu (p<0.001) ve kristaloid (p=0.007) kullanıldı. KPB
uygulanmayan grupta ise kolloid kullanımı anlamlı olarak daha fazla idi (p=0.0035). KPB
uygulanmayan grupta ilk oral alım daha erken olurken (p>0.05), her iki grubun defekasyon
zamanları benzerdi (p>0.05). KPB uygulanan ve uygulanmayan hastaların tamamında
operasyon öncesi değerlerine göre postoperatif 24. saatte aspartat aminotransferaz, total ve
direkt bilirubin ve lökosit düzeylerinde anlamlı yükselme olurken hemoglobin düzeylerinde
anlamlı düşme görüldü. Her iki grupta da hastaların dört farklı zamanda bakılan
intraabdominal basınç değerleri arasında fark yoktu. KPB uygulanan grubun I-FABP
düzeylerinde operasyon sonunda operasyon başlangıcına göre anlamlı yükselme olurken
(p<0.005), postoperatif 12. ve 24. saatlerde operasyon bitişine göre anlamlı düşme görüldü
(sırasıyla p<0.001, p<0.001). KPB uygulanmayan grupta ise operasyon sonunda operasyon
başlangıcına göre yükselme görülürken (p<0.001), postoperatif 24. saatte operasyon
başlangıcına göre anlamlı düşme görüldü (p<0.001).
Sonuç olarak, KPB uygulanan veya uygulanmayan hastalarda GİS komplikasyonu
görülmemekle beraber her iki grupta da postoperatif AST, total ve direkt bilirubin, laktat
değerlerinde yükselme oldu. Her iki grubun I-FABP düzeyleri cerrahi bitiminde preoperatif
değerlere göre anlamlı şekilde yükseldi. Postoperatif dönemde preoperatif dönemdeki
değerlerine geri döndü. KPB uygulanan veya uygulanmayan hastalarda GİS değişikliklerinin
takibinde I-FABP kullanılabileceğini düşünüyoruz.
The incidence of gastrointestinal system complications in patients undergoing
cardiopulmonary bypass, changes between 0.3-2%, which is rare. However, mortality rate is
considerably high at patients that GIS complication is observed (%11-59). The aim of our
study was to determine the incidence of GI complications in patients undergoing open heart
surgery with or without CPB and to investigate the corelation between GI complications and
plasma intestinal fatty acid binding protein (I-FABP) levels.
After Başkent University Clinical Research and Ethics Committee and approval informed
consent obtained, ASA III patients undergoing elective coronary artery bypass surgery greft
aging between 50-70 were studied prospectively. CABG surgery was performed with or
without CPB in 35 (group I) and 16 (group II) patients respectively preoperative and 24 hours
postoperative. Patients with chest pain, hepatic or renal failure, ejection fraction less than
40%, needing valve surgery or reoperation were excluded from the study. Preoperative and
24th hour of postoperative period aspartate aminotransferase, alanine aminotransferase, total
and direct bilirubin, amilase, blood urea nitrogen, creatinin, leukocyte and hemoglobin levels
of the patients were recorded. Arterial blood gases, plasma I-FABP levels and intraabdominal
pressures were measured before and immediately after the operation and 12 and 24 hours
postoperatively. The variables between and within the groups were analized with Student’s t
and Mann Whitney U and Wilcoxon test respectively.
The demographic properties were similiar. The duration of operation was longer CPB group
(p<0.001). The number of greft was 3.8±1.16 and 2.1±0.88 in group I and II respectively
(p<0.01). Although duration of mechanical ventilation and hospital stay was similar in both
groups, intensive care unit was longer in the CPB group (p<0.001). In the CPB group had
neurological complication postoperatively (p>0.05). Atrial fibrilation was observed in 8
patients postoperatively (p<0.043). Urine output the amount of red blood cell transfusion and
cristalloid infusion were significantly higher in the CPB group. The amount of colloid
infusion was higher in group II (p=0.035). First oral intake time was earlier in the group II
(p<0.05). Defacation time of both groups were similar (p>0.05). Plasma AST, total and direct

bilirubin and white blood cell levels were significantly high and hemoglobine levels were
significantly low at 24th hour postoperatively in both groups when compared to preoperative
values. Intraabdominal pressure values of both groups were similar during the study. Plasma
I-FABP levels of both groups increased significantly at the end of the operation (p<0.05).
This levels decreased in the CPB group at postoperative 12 and 24 hrs.
In conclusion, we did not observe any GIS complication in our patients. However,
postoperative AST, total and direct bilirubine, lactate levels increased significantly in both
groups. Plasma IABP levels of both groups increased significantly at the end of the operation
and return to preoperative levels. Therefore we consider plasma IABP levels may be used to
verify GI changes in patients undergoing open heat surgery.
