Hepatorenal sendromlu hastalarda terlipressin tedavisi
Özet
Giriş ve Amaç: Hepatorenal sendrom (HRS), akut veya kronik karaciğer hastalığı zemininde
klinik, laboratuar veya histolojik olarak gösterilebilen bir neden olmaksızın gelişen, akut
fonksiyonel renal yetmezlikle karakterize, sirozun mortalitesi yüksek bir komplikasyonudur.
Splanknik vasküler yataktaki arteriyel vazodilatasyona ikincil renal dolaşımda
vazokonstriksiyon ve vazokonstriktör sistemin hemeostatik aktivasyonu ile efektif kan
akımının azalması patogenezde sorumlu tutulan mekanizmadır. Vazopressin analogları
(terlipressin, ornipressin) sistemik ve splanknik vazokonstriksiyon yaparak efektif arteriyel
kan akımını artırır ve endojen vazokonstriktör sistemin (renin-anjiotensin sistemi ve sempatik
sinir sistemi) aktivitesini azaltır, böylece renal kan akımı ile glomerüler filtrasyon hızı artar.
Bu çalışmanın amacı hepatorenal sendrom gelişen hastalarda terlipressin tedavisinin prognoz
üzerine etkisi ve sağkalım üzerine etkili olan risk faktörlerini araştırmaktır.
Gereç ve Yöntem: Bu çalışmada Ocak 2000 – Haziran 2008 tarihleri arasında HRS tanısıyla
terlipressin tedavisi almış 52 HRS (19 tip 1 HRS ve 33 tip 2 HRS)’lu hasta değerlendirildi.
Bulgular: Terlipressin tedavisi ile tedavi sonrası serum sodyum düzeylerinde anlamlı artış
saptandı (p=0,030). Hastaların sağkalım oranı %33,3, ortanca sağkalım süresi 19 gündü.
Hastaların 30 günlük sağkalım oranı ise %25,4’tü. Tedavi sırasında ve sonrasında 7 hastaya
karaciğer nakli yapıldı. Tedavi süresince hemodiyalize alınan ve alınmayan hastaların
sağkalım oranları arsındaki fark anlamlıydı (p=0,002). Ölüm üzerine en etkili risk faktörleri
kontrast madde kullanımı (p=0,006, HR=2,8), hemodiyaliz (p=0,025 HR=2,8) olarak
saptanırken terlipressin tedavi süresinin artışının ölüm riskini azalttığı saptandı (p=0,002
HR=0,89).
Sonuç: HRS’da ölüm üzerine en etkili risk faktörleri kontrast madde kullanımı, hemodiyaliz
gerekliliği ve terlipressin tedavi süresindeki artıştır. Hepatorenal sendromun tedavisi karaciğer
naklidir. Ancak karaciğer nakline kadar geçen sürede yaşamı uzatabilmek için terlipressin
özellikle nakil için bekleyenlerde iyi bir alternatiftir.
Introduction and Objective: Hepatorenal Syndrome(HRS) developes on a background of
acute or chronic hepatic disease, without a cause which can be shown as clinic, laboratory or
histological, characterize by an acute functional renal failure, a complication of cirrhosis
which has a high mortality. Arterial vasodilation of the splanchnic vascular bed,
vasoconstriction in secondary renal circulation and with the hemostatic activation of
vasoconstrictor system decreased effective blood flow that is the mechanism responsible for
pathogenesis. Vasopressin analogues(terlipressin, ornipressin) cause vasoconstriction of the
splanchnic and systemic circulation, improve effective arterial blood flow and attenuate
activation of the endogenous vasoconstrictor system(renin-angiotensin and sympathetic
nervous system) and thereby increase renal perfusion and glomerular filtration rate. In this
study, effects of treatment with terlipressin on survival and risk factors for death of patients
with HRS was searched.
Material and Method: In this study, from January 2000 to July 2008, 52 patients(19 type 1
HRS patients and 33 type 2 HRS patients) diagnosed as HRS treated with terlipressin are
evaluated.
Findings: Treatment withTerlipressin and after the treatment serum sodium levels were
determined to be significantly increased (p=0.030). Patients survival rate 33.3% , median
survival time 19 days. Patients’ survival rate at 30 days was 25.4% . During the treatment and
after treatment liver transplantation was performed in 7 patients.
During the treatment, between both patients who were treated with hemodialysis and who
were not treated with hemodialysis survival rate were significantly (p= 0.002) different. When
most effective risk factors for death determined as using contrast agents (p=0.006, HR=2.8),
hemodialysis (p=0.025 HR=2.8); the risk of death decreased with increasing terlipressin
treatment time was determined (p=0.002 HR=0.89).
Conclusion: Using contrast agents, the necessity for hemodialysis and increasing terlipressin
treatment time are the most effective risk factors on death of the HRS. The definitive
treatment for hepatorenal syndrome is liver transplantation. However until the time of liver
transplantation for extending the survival especially terlipressin is a good alternative to those
who were waiting for liver transplantation.
