dc.description.abstractOtojen büyüme faktörleri içeren Trombositten Zengin Plazma (TZP), yumuşak doku iyileşmesini hızlandırdığı düşünülen bir materyaldir. Bu çalışmanın amacı, dişeti çekilmelerinin tedavisinde koronale pozisyone flep (KPF)+TZP kombine tekniğinin etkinliğini klinik olarak değerlendirmek ve sonuçlarını sadece KPF uygulanan grupla karşılaştırmaktır. Çalışmaya çift taraflı Miller Sınıf I veya II dişeti çekilmelere sahip, sistemik olarak sağlıklı 17 birey dahil edilmiştir. Öncelikle, yapışık dişetinin yetersiz olduğu Miller II çekilmelere serbest dişeti grefti uygulanarak yapışık dişetinin arttırılması amaçlanmıştır. Üç ay sonra, bölgeler rastgele KPF veya KPF+TZP gruplarına ayrılmıştır. Operasyon öncesi, 1., 3. ve 6. aylarda klinik parametreler kaydedilmiştir. Ondördüncü günde yara iyileşme indeksi değerlendirilmiştir. Her iki grupta da cep derinliği, çekilme derinliği, klinik ataşman seviyesinde başlangıca göre pozitif yönde gelişme olmuştur. Keratinize dişeti genişliği kontrol grubunda değişmezken; test grubunda anlamlı şekilde artmıştır. Ondördüncü günde yara iyileşmesinin test grubunda daha iyi olduğu ve postoperatif komplikasyonların daha az olduğu kaydedilmiştir. Bu randomize kontrollü klinik çalışmanın sonuçlarına göre Miller Sınıf I dişeti çekilmelerinin tedavisinde, yara iyileşmesini hızlandırmada ve daha başarılı klinik sonuçlar elde etmede TZP; KPF tekniğine ilave katkılar sağlayabilir. Platelet-rich plasma (PRP), containing autologous growth factors, has been thought to accelerate soft tissue healing. Therefore, the aim of this study is to evaluate the clinical effectiveness of CAF+PRP combined technique and also to compare its results with CAF alone in the treatment of gingival recession. Seventeen systemically healthy patients with bilateral Miller Class I/II gingival recessions participated in this study. Firstly, Miller Class II recessions were treated with free gingival grafts in order to improve inadequate keratinized tissue. After three months, sites were randomly assigned into CAF (control) or PRP+CAF groups. Clinical parameters were recorded at baseline, and at 1, 3 and 6 months. Wound healing at postoperative 14th day was also evaluated. Probing depth, recession depth and clinical attachment levels improved for both study groups. The increase in keratinized tissue width was statistically significant in the test group while it didn’t change in the control group. Wound healing was better and complication occurrences were less than the control group on 14th day. Based on the results of this randomized controlled study, PRP may provide additional clinical benefits and accelerate wound healing compared to CAF alone in the treatment of Miller Class I recessions.en_US
